Special Issue Information

Dear Colleagues,

We are doing a review of articles submitted from institutions to Pharmacy that deal with MTM (medication therapy management) in hospice/palliative care. We would like to offer you the opportunity to be one of these institutions and publish a paper free of charge. The article should incorporate a detailed description of how your institution utilizes/provides the service of MTM. Medication therapy management entails a thorough and extensive analysis of the patient’s drug regimen to see if there are any inappropriate or clinically irrelevant medications, drug interactions, potential serious side effects, monitoring parameters with certain medications, opportunity to limit the number of medications used, etc. The collection will be published online to help pharmacists and others in their role of providing MTM service in hospice/palliative care settings. It will emphasize the integral role that pharmacists play in this field of practice.
