Background: Quality of life is a foundational component of overall health and is negatively impacted by post-traumatic stress disorder (PTSD). In the last decade, acupuncture treatment has become a popularized treatment option for PTSD, especially for Veterans. Research evidence is needed to provide support for acupuncture as a treatment for PTSD which leads to an enhanced quality of life. Objectives: This paper investigated the changes in health-related quality of life in Veterans with combat PTSD who enrolled in a 5-year randomized (two-arm, parallel-group, prospective, single-blinded) clinical trial, completing either a control (Sham) or acupuncture (Verum) treatment (N = 93). Methods: Health-related quality of life was measured by the Veterans RAND 12-item Health Survey (VR-12). Paired t tests between and among participants’ differences were calculated for the Verum acupuncture group, Sham acupuncture group, and Total group. Results: Veterans who completed acupuncture treatment in either randomized arm (Verum and Sham) had a self-perceived improvement in quality of life when assessing physical and mental health symptoms. Specifically, those who were randomized and completed Verum acupuncture treatment reported the greatest improvement in mental health quality of life. Conclusions: Study results support prior foundational findings that acupuncture has a positive effect on self-reported quality of life by reducing PTSD symptoms in Veterans with combat PTSD, with Verum acupuncture eliciting the greatest improvement on mental health symptoms. Chronic pain significantly impairs functional performance in patients with cancer. Although acupuncture is effective for cancer-related pain, its impact on pain-related functional interference remains unclear. This secondary analysis of the PEACE randomized clinical trial included patients with prior cancer diagnoses and musculoskeletal pain for ≥3 months. Participants were randomized to groups undergoing 10 weeks of electro-acupuncture, auricular acupuncture, or a waitlist control. Functional performance was assessed using the Quick-Disability Arm/Shoulder/Hand (Q-DASH) for upper limbs and the Western Ontario and McMaster Universities Osteoarthritis (WOMAC) subscale for lower limbs (higher scores = worse function). Linear mixed models compared changes over time between groups, with week 12 as the primary endpoint. Functional changes were also compared between pain responders and non-responders in the acupuncture arms. Among 360 patients (mean [SD] age, 62.1 [12.7] years; 69.7% women), mean baseline Q-DASH and WOMAC scores were 33.2 (19.8) and 33.3 (20.3). At week 12, both electro-acupuncture and auricular acupuncture significantly improved function versus waitlist: Q-DASH by −7.18 and −9.64 points, respectively, and WOMAC by −6.89 and −7.61 points (all p < 0.001). No differences were found between the two acupuncture groups. Treatment effects on Q-DASH diminished during follow-up, while improvements on WOMAC persisted. Within the acupuncture groups, pain responders achieved greater functional gains than non-responders (Q-DASH, −6.74; WOMAC, −6.16; both p < 0.001). Electro-acupuncture and auricular acupuncture improved upper and lower extremity function in cancer patients with chronic pain. These findings support acupuncture as a potential adjunct in functional rehabilitation for cancer survivors. Full article

Objective: This study aimed to evaluate, through a network meta-analysis, the short- and long-term efficacy of both Western medical therapies and traditional Chinese medical therapy (acupuncture) in improving symptoms of shoulder pain. Methods: A comprehensive computer-based search was conducted in Embase, Cochrane Library, Web of Science, and PubMed databases for randomized controlled trials (RCTs) related to Western and Chinese medical treatments for shoulder pain measured by visual analogue scale (VAS) scores. All researchers independently screened and selected studies, extracted data, and assessed the risk of bias. Studies that met quality standards were analyzed using Stata 16.0 and Review Manager 5.4 software. Results: A total of 269 articles were retrieved, and 15 were ultimately included in the network meta-analysis, covering nine types of Western and Chinese medical therapies. The total sample size was 1114 cases, with 557 in an experimental group and 557 in a control group. In terms of reducing VAS scores at 4 weeks after treatment, sham acupuncture was significantly less effective than acupuncture (MD: 19.39; 95% CI: 0.66–38.12), indicating that acupuncture had a better short-term effect on pain relief at 4 weeks. In terms of reducing VAS scores at 12 weeks after treatment, sodium hyaluronate (hyaluronate) was more effective than physical therapy (PT) in reducing long-term pain (MD: −19.57; 95% CI: −37.23–−1.90); suprascapular nerve block (SSNB) (MD: −9.11; 95% CI: −16.02–−2.20) and arthroscopic capsular release (MD: −16.07; 95% CI: −30.16–−1.97) were also more effective than PT. The top three treatments in terms of clinical efficacy for painful shoulder conditions were hyaluronate, SSNB, and arthroscopic capsular release. Conclusions: For the treatment of shoulder pain, hyaluronate, SSNB, and arthroscopic capsular release showed greater potential long-term efficacy in pain reduction than PT, with hyaluronate showing the best effect. Chronic gastritis (CG) is a prevalent digestive disorder. It progresses through multiple stages, has an insidious onset, and can lead to severe complications if untreated. Modern treatments primarily aim to eradicate Helicobacter pylori and relieve symptoms. However, drug resistance and adverse effects often limit their effectiveness. As a primary traditional Chinese medicine (TCM) therapy, acupuncture treats CG through multi-target mechanisms. This review systematically outlines the classification and pathology of CG. It also comprehensively analyzes animal and clinical studies on acupuncture for CG from the past decade. The study summarizes the mechanisms of acupuncture and related therapies for CG, covering gastric mucosal function, metabolism, intestinal flora, gastrointestinal hormones, apoptosis, inflammation, and oxidative stress. It further explores the relationships among diseases, interventions, acupoints, and molecular pathways. Additionally, it compares the therapeutic profiles of different external therapies. The review also examines the current state of clinical research, including the selection of acupoints, treatment duration, and outcome assessment. The results demonstrate that external therapies effectively alleviate common CG symptoms such as abdominal distension, acid reflux, and stomach pain. These treatments also improve gastric mucosal health and modulate serum levels of inflammatory factors, oxidative stress markers, and gastrointestinal hormones. In vivo experiments using chronic non-atrophic gastritis (CNAG) and chronic atrophic gastritis (CAG) models confirm these benefits, showing changes in key biomarkers and elucidating potential mechanisms. Nevertheless, future high-quality, large-sample clinical trials are still needed to firmly establish efficacy. Further mechanistic studies are also needed to validate the interconnections among relevant signaling pathways. Full article

Polycystic ovary syndrome (PCOS) is the most common cause of anovulatory infertility, with ovulation induction remaining the first-line treatment approach. Although letrozole has emerged as the most effective monotherapy, treatment resistance, side effects, and patient preferences have led to increasing interest in adjunctive or alternative approaches. This narrative review summarizes the current evidence for ovulation induction in patients with PCOS, including conventional pharmacologic agents, such as clomiphene citrate, letrozole, gonadotropins, and insulin-sensitizing agents, as well as complementary therapies, such as acupuncture and Chinese herbal medicine. We also examine emerging adjuvants, such as vitamin D, omega-3 fatty acids, sildenafil, and antioxidants that may enhance clinical pregnancy rates or improve endometrial receptivity. While robust evidence supports the use of letrozole as a first-line agent, complementary and integrative therapies may offer additional benefits, particularly in treatment-resistant or preference-driven contexts. Further high-quality studies are needed to clarify the role of combined therapeutic strategies in optimizing fertility outcomes for women with PCOS. Full article

Background/Objectives: Mild cognitive impairment (MCI) represents an intermediate stage between normal ageing and dementia, with a high annual progression rate. Despite its clinical relevance, no pharmacological treatment has been definitively approved for this condition; however, multiple pharmacological and non-pharmacological strategies have been investigated for their potential benefits. This systematic review assessed the effectiveness of both types of interventions in adults with MCI, aiming to identify effective strategies to preserve cognitive function. Methods: A systematic search (2017–2025) was conducted in PubMed, Scopus, ScienceDirect, SpringerLink, and WOS, following PRISMA guidelines. Randomised controlled trials and quasi-experimental studies involving adults aged ≥ 50 years with a diagnosis of MCI were included. Outcomes were evaluated in terms of cognitive, functional, behavioural, and quality-of-life improvements. Risk of bias was assessed using the RoB 2 and ROBINS-I tools. Results: Of 108,700 records screened, 40 studies were included. Non-pharmacological interventions, such as cognitive training (conventional, computerised, or virtual reality-based), consistently improved memory, attention, and executive functions (e.g., MoCA: +3.84 points; p < 0.001). Transcranial magnetic stimulation combined with physical exercise also demonstrated significant benefits (p = 0.025). Among pharmacological treatments, only vortioxetine and choline alfoscerate showed modest improvements; cholinesterase inhibitors had limited effects and frequent adverse events. Complementary therapies (yoga, probiotics, and acupuncture) yielded promising outcomes but require further validation. Conclusions: Non-pharmacological strategies, particularly cognitive training and physical exercise, emerge as the most effective and safe approaches for managing MCI. The inclusion of pharmacological interventions with preliminary evidence of benefit should be considered within a personalised, multimodal approach, while recognising the current absence of approved drug treatments for MCI. Further research is needed in underrepresented populations, such as those in Latin America. Full article

Background/Objectives: Korean medicine healthcare (KMHC), a form of traditional medicine including acupuncture and herbal medicine, is widely utilized by patients with intervertebral disc disease (IVDD). With the increasing use of real-world evidence (RWE) in the medical field, this study aims to derive RWE on KMHC utilization and its associated factors in patients with IVDD. Methods: Data from 495 individuals who received outpatient healthcare for IVDD regardless of the purpose such as treatment, examination, rehabilitation, monitoring, or prescription were analyzed using the 2022 Korea Health Panel Survey (KHPS). Multinomial logistic regression analyses were performed to identify factors associated with healthcare use for IVDD. Regression models were constructed by sequentially adding predisposing, enabling, and need factors following Andersen’s behavioral model. All statistical analyses accounted for the complex survey design of the KHPS using survey sampling weights. Results: Individuals aged 45–59 years were less likely to use both KMHC and conventional medicine healthcare (CMHC) for IVDD compared to those aged 19–44 years (adjusted odds ratio [95% confidence interval], 0.28 [0.09, 0.89]). People with disabilities showed lower utilization of both KMHC and CMHC for IVDD compared to those without disabilities (0.27 [0.09, 0.81]). Individuals who were employed (2.37 [1.06, 5.3]) or perceived their health status as fair (3.05 [1.17, 8]) or poor/very poor (6.13 [2.04, 18.45]) were more inclined to use both KMHC and CMHC for IVDD. Individuals who engaged in regular physical activities (2.65 [1.19, 5.9]) or had shoulder joint diseases (3.71 [1.22, 11.29]) or other spine-related diseases (2.63 [1.16, 5.96]) were more inclined to use KMHC-only for IVDD. Conclusions: This study identified significant demographic and health-related factors influencing KMHC utilization for IVDD. These findings emphasize the need for tailored healthcare policies regarding KMHC for IVDD for effective resource distribution. Full article

Canine Otitis Externa (COE) is a challenging otological disorder in dogs which causes significant irritation and discomfort. This comprehensive review provides an extensive analysis of COE with emphasis on the fundamentals of the condition, starting with the basic anatomy of the canine external ear canal, followed by pathophysiology and diagnosis of COE. Furthermore, novel therapeutic interventions, formulation considerations, and challenges, with the perspective of future directions in the field of COE management, are described. Diagnostic models, including clinical examination, cytology, and susceptibility tests, are presented to provide an overview of the processes involved in detecting and selecting appropriate therapies for the management of COE. Moreover, this paper describes the limitations of current therapies with considerations for the selection of alternative novel treatments such as aromatherapy, acupuncture, bacteriophages, nutraceuticals, and nanomedicines. This review places particular emphasis on the pharmaceutical formulation of topical products used for COE treatment. Various factors, including osmotic pressure, safety profile, viscosity, bioadhesion, and formulation pH, must be considered when developing topical preparations. These parameters are critical in formulation development to enhance therapeutic outcomes and minimise potential side effects. Finally, potential advancements in COE management are highlighted, including the integration of microbial genomics and the significance of managing the microbiota. Overall, this review serves as a valuable resource for those interested in the future of topical treatment of COE by providing a deep understanding of diagnostic, therapeutic, and medical interventions for effective management. Alzheimer’s disease (AD) is a prevalent neurodegenerative disorder characterized by progressive cognitive decline and an impaired quality of life, for which no curative treatment is currently available. Recent research indicates that chronic pulmonary conditions—including chronic obstructive pulmonary disease (COPD) and obstructive sleep apnea (OSA)—exhibit significant epidemiological associations with AD pathogenesis, suggesting that the lung–brain axis may contribute to AD development. Acupuncture, a core TCM intervention, shows promise for modulating multisystem functions and enhancing cognitive performance. This review synthesizes the current evidence regarding pulmonary diseases influencing AD through the lung–brain axis, elucidates potential mechanisms by which acupuncture may modulate pulmonary function and mitigate AD pathology, and explores future directions for lung–brain axis-targeted acupuncture interventions. Our overarching aim is to propose integrative, evidence-based strategies that combine Chinese and Western medicine for the prevention and management of AD. Full article

Background: As diabetes mellitus continues to escalate into a global health crisis, particularly in China, the limitations of conventional pharmacotherapy underscore the need for complementary interventions. This study systematically reviews two decades of research progress on acupuncture for diabetes management. Methods: A total of 391 publications met the inclusion criteria from the Web of Science Core Collection (2004–2024) using the search terms “acupuncture” AND “diabetes”. These comprised 294 original studies and 97 reviews. CiteSpace 6.3.R1 was used to perform multidimensional analyses, including co-occurrence networks, centrality algorithms, and silhouette metrics across countries/regions, institutions, authors, journals, references, and keywords. Results: The analysis shows a significant increase in publications on acupuncture for diabetes management after 2013. China and the United States lead in research output, yet collaboration between the two countries remains limited. Most researchers currently work within isolated clusters, underscoring the need for greater exchanges and cooperation. Furthermore, this study identified three key research hotspots: insulin resistance, complications, and interdisciplinary research. Conclusions: This bibliometric analysis reveals dynamic growth patterns and paradigm shifts in acupuncture and diabetes research. The findings provide valuable implications for integrating acupuncture into diabetes treatment. Full article

Utilization Pattern of Acupuncture and Its Associated Predictors for Cancer Pain in South Korea: A Cross-Sectional Study

Background: Cancer-related pain is a common and distressing symptom among patients with cancer. Although acupuncture is widely used and supported by growing evidence, its real-world use and related patient attitudes remain underexplored in South Korea. This study aimed to investigate patterns of acupuncture use and identify factors influencing its use among Korean cancer patients experiencing pain. Methods: A cross-sectional survey was conducted from October 2023 to May 2024 at six Korean medicine hospitals. A total of 201 cancer patients with pain completed a questionnaire assessing acupuncture use, pain severity and interference, and beliefs using a modified version of the ABCAM (Attitudes and Beliefs about Complementary and Alternative Medicine) instrument. Treatment concerns, logistical barriers, and social norms were analyzed. Results: Of the 201 participants, 80.6% reported using acupuncture for cancer-related pain. Pain severity was the only significant predictor of acupuncture use (OR = 1.53, 95% CI: 1.10–2.12, p = 0.01). Acupuncture users reported fewer concerns about safety and side effects, fewer logistical barriers, and stronger encouragement from family, healthcare providers, and peers. Conclusions: This is the first study to explore real-world use of acupuncture for cancer pain in South Korea. Greater pain severity, perceived safety, ease of access, and social support were associated with higher acupuncture use. Mechanical Acupuncture at ST36 Attenuates Inflammatory Pain Involving TRPV1 Signaling in Mice

We recently developed a mechanical acupuncture instrument (MAI) that applies mechanical stimulation to acupuncture points in effectively treating hypertension and addiction in animal models. However, its analgesic effect on inflammatory pain remains unclear. Here, we aimed to determine the optimal duration of MAI treatment at any given acupuncture point to improve analgesic effects. Adult male ICR mice (20–25 g, 6 weeks old, n = 6 per group) were used to evaluate whether MAI administration or TRPV1 (transient receptor potential vanilloid 1) inhibition had analgesic effects. Then, we investigated whether it affected TRPV1 expression and glial cells in the spinal cord of mice. The capsaicin test was used to identify the most effective acupoints and optimal treatment times for MAI. Additionally, we induced inflammatory pain in mice by administering a 2% carrageenan via intraplantar injection. To assess the analgesic effects of MAI treatment and TRPV1 inhibition, we evaluated pain-related behavior using von Frey filaments and a thermal stimulator applied to the hind paw. MAI treatment significantly suppressed pain-related behaviors. In particular, paw-licking duration was markedly reduced in the group treated with MAI for 60 s at ST36 compared to the capsaicin-treated group (p < 0.05), suggesting a robust analgesic effect. Additionally, MAI and capsazepine administration significantly attenuated carrageenan-induced mechanical allodynia and thermal hyperalgesia compared to the carrageenan-only group (p < 0.05 to p < 0.001). Additionally, MAI treatment and capsazepine administration effectively suppressed the carrageenan-induced upregulation of TRPV1 and glial cells in the spinal cord. In conclusion, our findings show that MAI administration at ST36 significantly alleviated inflammatory pain and was associated with downregulation of TRPV1 expression and microglial activation in the spinal cord. The present findings suggest that TRPV1 signaling is involved in the analgesic effects of mechanical acupuncture; however, a direct causal relationship has yet to be established. Full article

Osteoarthritis in dogs is commonly managed with non-steroidal anti-inflammatory drugs (NSAIDs), nutraceuticals, and other medications aimed at alleviating pain and inflammation. Although NSAIDs are generally considered the most effective option, their use is contraindicated in certain conditions due to adverse side effects. Consequently, there has been growing interest in alternative or complementary therapies, such as acupuncture, laser therapy, and their combination. Laser acupuncture involves stimulating traditional acupuncture points with low-intensity laser light, producing anti-inflammatory, analgesic, anti-edematous, and bio-stimulatory effects. In this study, 19 dogs with osteoarthritis were assigned to two groups: 10 received laser acupuncture treatment (Group T) using a dual-wavelength system delivering continuous and pulsed, synchronized emissions, while 9 served as controls (Group C). After 30 days, only Group T showed significant improvement (p < 0.01), with reduced pain and improved joint mobility. Significant reductions in LDH and CPK were observed in Group T, along with changes in inflammatory markers (IL-6 and IL-10, though not TNF-α) and a significant increase in biological antioxidant potential. These preliminary findings indicate that laser acupuncture, applied according to a standardized protocol and adapted to the site of disease, may represent a viable non-invasive therapeutic option for canine osteoarthritis, particularly in cases where pharmacological treatment is contraindicated or ineffective. Further large-scale, randomized controlled trials are warranted to confirm these results and establish long-term efficacy. Full article

Background and Objectives: Musculoskeletal pain is a global public health issue. Eight Constitution Medicine (ECM), a type of East Asian Traditional Medicine, offers personalized, minimally invasive treatment through Eight Constitution Acupuncture (ECA) and Eight Constitution Lifestyle Intervention (ECLI). Despite its clinical use, scientific evidence supporting ECM’s effectiveness remains limited. This study aimed to evaluate the effectiveness in treating musculoskeletal pain in primary care settings. Materials and Methods: This retrospective study analyzed medical records from three ECM clinics (Gangnam-Shingwang, Yeson, and Yebon) between January 2018 and August 2023. A total of 163 patients were included, with 44 providing follow-up data. Pain intensity, quality of life, and functional outcomes were assessed using validated instruments including the PainDETECT questionnaire, Korean Cancer Pain Assessment Tool (KCPAT) somatic pain scores, EuroQol 5-Dimension 5-Level (EQ-5D-5L), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), Oswestry Disability Index (ODI), Neck Disability Index (NDI), and Shoulder Pain and Disability Index (SPADI). Pre- and post-treatment scores were statistically analyzed. Results: Significant decreases were observed in KCPAT somatic pain scores (11.77 ± 4.77 to 9.77 ± 5.32) and significant improvements in EQ-5D-5L scores (0.74 ± 0.12 to 0.80 ± 0.07). WOMAC and ODI scores also showed significant improvements. However, the changes in the NDI, SPADI, and PainDETECT scores were not statistically significant. No adverse events were reported. Conclusions: ECM, through ECA and ECLI, may offer effective personalized treatment for musculoskeletal pain, improving both pain intensity and quality of life. Despite its small sample size and retrospective design, this study offers valuable preliminary evidence for ECM. Further large-scale prospective studies are needed to confirm these findings. Background and Objectives: Facial paralysis involves the complete or partial loss of facial movement due to damage to the facial nerve, leading to impaired voluntary muscle function and facial asymmetry. Given its significant physical and psychosocial impact, there is an urgent need to strengthen the evidence supporting non-pharmacological treatments. This umbrella review aims to compile the most reliable and current data to establish a consensus on the effectiveness of such interventions for patients with facial paralysis. Materials and Methods: This study is an umbrella review. A systematic search was conducted in PubMed, Embase, Scopus, and CINAHL (28 July 2024). The variables assessed included overall healing/recovery rate, facial disability, and facial function. Methodological quality was evaluated using the AMSTAR and ROBIS tools. Screening was performed independently by two reviewers, with a third reviewer resolving any discrepancies. Results: Five systematic reviews were included, all evaluating the impact of non-pharmacological interventions in facial paralysis. The findings suggest that acupuncture and electrical stimulation may improve recovery rates and facial function, although high heterogeneity and methodological limitations were noted in some studies. No definitive conclusions could be drawn regarding facial disability. Conclusions: The combination of electrotherapy with other complementary techniques, such as facial exercises or laser therapy, appears to be a safe and potentially effective approach for facial paralysis rehabilitation. Nonetheless, further research employing standardized protocols and higher methodological quality is necessary to establish more robust conclusions for physiotherapeutic practice. Full article
