3            132      This is a bit confusing. You randomized at 15 seconds, but                              was the envelope system used? (This is much clearer later in                            the paper.

 

GENERAL COMMENTS:

The article is excellent. I think it would benefit from some attention to reducing the repetition of some topics.

Thank you. On page3 we have indicated that further details follow later.

We agree there is repetition and have removed some sections and sentences where they are unnecessary repeats, particularly towards the end, where in following the SPIRIT reporting framework, items were reported twice.
