Objective: This observational cross-sectional study aimed to comprehensively evaluate sensory–cognitive performance in children diagnosed with Attention-Deficit/Hyperactivity Disorder (ADHD), with a focus on auditory processing, visual–perceptual abilities, visual–motor integration, and oculomotor function. The study further examined how hyperactivity, age, and gender may influence these domains. Methods: A total of 70 non-medicated children with clinically diagnosed ADHD (mean age = 9.1±2.4 years; 67.1% male), all with normal visual acuity, were assessed using four standardized instruments: the Test of Auditory Processing Skills, Third Edition (TAPS-3), the Test of Visual Perceptual Skills, Fourth Edition (TVPS-4), the Beery–Buktenica Developmental Test of Visual–Motor Integration, Sixth Edition (VMI-6), and the Developmental Eye Movement (DEM) Test. Statistical analyses included one sample and independent samples t-tests, one-way ANOVA, and Pearson correlation coefficients. Results: Participants demonstrated significantly above-average performance in auditory processing (TAPS-3: μ=108.4, std=7.8), average visual–perceptual abilities (TVPS-4: μ=100.9, std=7.2), slightly below-average visual–motor integration (VMI-6: μ=97.1, std=9.0), and marked deficits in oculomotor efficiency (DEM ratio: μ=87.3, std=18.1). Statistically significant differences were observed across these domains (t-values ranging from 2.9 to 7.2, p<0.01). Children with hyperactive-impulsive presentations exhibited lower horizontal DEM scores (μ=73.4, std=12.3) compared to inattentive counterparts (μ=82.9, std=16.2; p=0.009). Age and sex influenced specific subtest scores, with boys and children aged 8–9 years achieving higher outcomes in word memory (p=0.042) and visual discrimination (p=0.034), respectively. Moderate correlations were identified between auditory and visual–perceptual skills (r=0.32, p=0.007), and between visual–perceptual and oculomotor performance (r=0.25, p=0.035). Conclusions: The findings from this sample reveal a distinct sensory–cognitive profile in children with ADHD, characterized by relatively preserved auditory processing and pronounced oculomotor deficits. These results underscore the value of a multimodal assessment protocol that includes oculomotor and visual efficiency evaluations. The conclusions pertain specifically to the cohort studied and should not be generalized to all populations with ADHD without further validation. Full article

The developmental eye movement (DEM) test is a paper-based tool used to assess ocular motor skills in children. By naming numbers in a simple and easy simulated reading task, the DEM test provides an oculomotor efficiency score without complex eye-tracking equipment. Studies have shown that its usefulness can be extended to adults, despite its name suggesting that it is used primarily for developmental ages. However, for a broader application in the adult population in a clinical setting, there are no adult-specific norms. This study aimed to develop adult normative data for the Italian-speaking population and assess whether the DEM scores were influenced by age. In this study, 521 healthy Italian adults, aged 20 to 79 years, participated. Normative data were obtained by using a regression-based framework with demographic variables as predictors. Results show that age influences all sub-measures of time such as Vertical Time (VT), Adjusted Horizontal Time (AHT), and the Ratio score, but did not influence the error score. This is in line with the processing speed reduction in aging. Errors were influenced by education and gender. DEM norms, defined and scored using percentiles and equivalent scores, allow the assessment of oculomotor efficiency in adults, making this test suitable for use in all clinical settings, particularly in neuropsychological and neurological ones. The primary aim of this descriptive cross-sectional study was to examine the relationship between ocular motility and motor skills in school-age children. Participants included 142 schoolchildren (mean age: 7.08 ± 0.61 years) who completed a computerised version of the Developmental Eye Movement (DEM) test while their eye movements were recorded, and Northeastern State University College of Optometry's Oculomotor test (NSUCO). Children were classified into three groups based on their level of motor performance, which was measured by the Movement Assessment Battery for Children-2 (MABC-2). The group with typical motor performance had higher percentiles for both vertical and horizontal time, fewer errors, number of saccades, fixations, and regressions, and faster test performance. Visual test results correlate with the motor assessment outcomes; correlations are weak or moderate. Our findings emphasise the interconnectedness of motor and ocular motility. Hence, including evaluation of visual and motor proficiencies at school age would help to detect struggles in these crucial areas of development. Full article

The aim of the study was to determine the normative data of the computerized DEM test for school-age children in Latvia. The study analyzed data on the performance (test execution time, duration, number of fixations, and number of errors) of 291 children while completing the computerized DEM test. Eye movement fixations were recorded with a Tobii Pro Fusion video-oculograph (250 Hz). According to the results of the study, the performance of the computerized DEM test is 77 %. For the study, 1 SD (one standard deviation) was chosen as a criterion for determining test norms. In the study, the norms of the computerized DEM test in Latvia were developed in class groups—from 1st to 6th grade (aged 7 to 12 years), the results were summarized in a table as the minimum performance values of the computerized DEM test. An efficient mode of evaluation for eye movements is the use of objective eye tracking systems combined with subjective tests (NSUCO or DEM), which are easily applicable across all age groups and in eye care clinical settings. The objective of this study was to characterize fixations during reading in two groups: a group of children with neurodevelopmental disorders (NDDG, 24 children, age: 6–12 years) and a group of children with oculomotor anomalies but without NDD (OAG, 24 children, age: 6–12 years). The results obtained were compared with those from a control group (CG, 20 children, age: 6–12 years). Specifically, the outcomes obtained with two subjective score systems, the Northeastern State University College of Optometry’s Oculomotor (NSUCO) test and the Developmental Eye Movement (DEM) test, were compared with the objective analysis obtained through a commercially available eye tracker (Tobii Eye X, Tobii, Stockholm, Sweden). Specialized analysis software, namely Clinical Eye Tracker 2020 (Thomson Software Solutions, Welham Green, UK), was used. It was found that children with NDD had impaired oculomotor skills. A higher number of regressions, more fixations, and longer durations of fixations appear to be characteristic signs of this population group. Additionally, children with NDD took longer to complete the DEM test, as well as exhibiting more errors. The use of objective videoculographic systems for eye tracking and subjective tests like the NSUCO or DEM are good tools to assess saccadic movements, allowing the detection of oculomotor abnormalities in children with NDD. Purpose: To measure the ocular motility parameters of the Developmental Eye Movement (DEM) test objectively, with an eye tracker in subjects with intellectual disability (ID). Methods: The DEM test was performed on 45 subjects with ID, while their eye movements were recorded with an eye tracker. Some objective parameters of ocular motility were obtained through each subtest (A, B, and C) of the full DEM test. Results: There was a significant positive correlation between the saccadic speed (cc: 0.537; p = 0.001) and length (cc: 0.368; p = 0.030) of both eyes for the same subject. People with a higher percentage of ID exhibited a greater number of fixations, saccades, and errors, and took longer to perform the DEM test than those with a lower ID percentage, who had greater numbers of these parameters than subjects without ID. Subjects without ID exhibited faster saccades, with a higher amplitude, than subjects with ID. Conclusions: The eye tracker quantifies ocular motility parameters involved in the DEM test in subjects with ID. Both eyes’ movements in subjects with ID were conjugated, exhibiting saccades of the same length and speed. All parameters were different in subjects with ID compared to those in subjects without ID, so normative tables specifically for subjects with ID are necessary. Full article

Background: The purpose of this study was to evaluate ocular motility in normal young adults when performing the Developmental Eye Movement (DEM) test using an infrared eye-tracker in a sample of young subjects without visual dysfunctions. Methods: An optometric evaluation was carried out on 52 participants with a mean age of 21.00 ± 3.22 years to verify they did not have any binocular dysfunction, by completing a computerized version of the DEM test while their eye movements were recorded with an eye-tracker. A custom-written software was developed to analyse some specific parameters of ocular motility while performing each subtest (Test A, Test B and Test C) of the complete DEM test. Results: The mean duration of the fixations was shorter in Test C (243.56 ± 46.18 s) than in Test A (493.52 ± 171.41 s) and Test B (484.20 ± 156.59 s). The mean adjusted horizontal (AdjHT: 35.24 ± 6.68 s) and vertical (VT: 33.58 ± 5.56 s) times were at the 45th and at the 40th percentile, respectively. In Test C, there was a high positive significant correlation between the saccadic speed (cc: 0.77; p < 0.001) and the saccadic length (cc: 0.74; p < 0.001) of both eyes. Conclusions: The eye-tracker is an objective method to evaluate the DEM test in subjects without binocular dysfunctions, measuring and quantifying ocular motility parameters that are impossible with the traditional subjective method. The eye movements of both eyes are conjugated in each subject, having saccades of the same length and speed. Full article

Vision is one of the most complex and dominant sensory systems necessary for information feedback from the environment. Few studies have already reported a positive effect of a sport vision program on elite sport teams’ visual skills; however, few studies have focused on the effect of sport vision programs on children’s visual skills. Therefore, the aim of this study was to determine the effect of a sports vision program on Grade 4 and 5 female netball players’ visual skills. Girls (N = 25) with a mean age of 10.08 years (SD = 0.65) formed part of this study. A pre-test–post-test design was followed with a retention test. The eight-week sports vision program was executed twice a week for 60 min on the experimental group (n = 13). The Developmental Test of Visual–Motor Integration (VMI-4), the Wayne Saccadic Fixator (WSF) and the Developmental Eye Movement (DEM) test were used to evaluate the girls’ visual skills, hand–eye coordination, visual reaction time, peripheral vision and saccadic eye movements. No statistical differences were found between the two groups before starting with the sports vision program. After intervention, significant differences between the two groups were reported, with the experimental group performing better in hand–eye coordination (p = 0.001) and reaction time (p = 0.001). Results further indicated that the experimental group experienced significant improvements (p ≤ 0.05) in motor coordination, hand–eye coordination, reaction speed and visual tracking after intervention with significant lasting effects (p ≤ 0.05). The control group performed significantly worse in reaction time (p = 0.01). A sports vision program can be recommended for Grade 4 and 5 female netball players to improve certain visual skills. Eye movement disorders have been reported in patients with multiple sclerosis (MS) as saccadic disturbances. Several methods have been described for the assessment of saccades, including the K-D and DEM tests. The performance of these tests also involves attention, language, and other brain areas which have been reported to be altered in MS patients. The aim of the study was to determine how developmental eye movement (DEM) and King-Devick (K-D) tests are affected in MS-patients. It was also to analyze whether a resolved episode of optic neuritis (ON) has a negative influence. Subjects with MS showed worse outcomes (higher times) in DEM test (72 (26) s and a K-D test (56 (22) s compared to healthy subjects (64 (7) s and 55 (11) s, respectively). However, a previous ON episode did not worsen the MS-times of DEM (80 (33) s or of K-D (62 (33) s. Horizontal saccades with the DEM showed differences between subjects with MS (with and without ON) and healthy ones (p < 0.05), whereas no such differences were found in the vertical saccades. According to the DEM instructions, MS patients would present heterogeneous oculomotor and non-visual disturbances. Regarding the K-D test, only the third card (the most complex one) showed differences (p < 0.05) between groups. These tests can capture impairment of attention, language, and other areas that correlate with suboptimal brain function in addition to the oculomotor dysfunctions present in subjects with MS. Children with specific learning disorders have been associated with oculomotor problems, with their analysis even suggested to be a potential diagnostic tool. A prospective non-randomized comparative study evaluating 59 children (6–13 years old) divided into three groups was conducted: a control group (CG) including 15 healthy emmetropic children; a group of 18 healthy children with oculomotor abnormalities (OAG); and a group of 26 children diagnosed with specific learning disorders (LDG). In all groups, besides a complete eye exam, oculomotricity was characterized with two clinical tests: Northeastern State University College of Optometry’s Oculomotor (NSUCO) and Developmental Eye Movement (DEM) tests. Concerning the NSUCO test, lower ability, precision, and head/body movement associated scorings were obtained for both smooth pursuits and saccades in OAG and LDG when compared to the CG (p < 0.001). Likewise, significantly longer time needed to read the horizontal sheet of the DEM test and a higher DEM ratio were found in OAG and LDG compared to CG (p ≤ 0.003). No differences between LDG and OAG were found in the performance with the two oculomotor tests (p ≥ 0.141). Oculomotor anomalies can be present in children with and without specific learning disorders, and therefore cannot be used as diagnostic criteria of these type of disorders. Full article

Saccadic eye movement is an important ability in our daily life and is especially important in driving and sports. Traditionally, the Developmental Eye Movement (DEM) test and the King–Devick (K-D) test have been used to measure saccadic eye movement, but these only involve measurements with “adjusted time”. Therefore, a different approach is required to obtain the eye movement speed and reaction rate in detail, as some are rapid eye movements, while others are slow actions, and vice versa. This study proposed an extended method that can acquire the “rest time” and “transfer time”, as well as the “adjusted time”, by implementing a virtual reality-based DEM test, using a FOVE virtual reality (VR) head-mounted display (HMD), equipped with an eye-tracking module. This approach was tested in 30 subjects with normal vision and no ophthalmologic disease by using a 2-diopter (50-cm) distance. This allowed for measurements of the “adjusted time” and the “rest time” for focusing on each target number character, the “transfer time” for moving to the next target number character, and recording of the gaze-tracking log. The results of this experiment showed that it was possible to analyze more parameters of the saccadic eye movement with the proposed method than with the traditional methods. Full article

By transplanting the Developmental Eye Movement (DEM) test chart to a virtual reality head-mounted display (VR HMD) system, this study sought to evaluate the effectiveness of the DEM test for measuring dynamic visual acuity. Thirty-nine adults aged 20–39 years of both genders were the subjects of the study. After undergoing measurement of their visual function, through medical questionnaire, interpu-pillary distance (IPD), near point of convergence (NPC), near point of accommodation (NPA), and far and near phoria, the correlation between the tests was analyzed performing DEM vertical, horizontal test and VR HMD DEM (VHD) vertical, horizontal test. NPC and NPA decreased significantly after the VHD test, while phoria did not. The horizontal was quicker than the vertical in the DEM test, and vice versa in the VHD test. DEM was quicker than VHD in both the vertical and horizontal directions. There was no notable difference in error frequency between DEM and VHD. In terms of DEM and VHD test, there was no notable difference in the short-range IPD and subjective symptoms of the top 10 and bottom 10 subjects. The performance time for VHD, in which the chart must be read while moving the body, was longer than that of DEM. Therefore, based on the consistency of the results of both tests and the lack of a difference in error frequency and subjective symptoms, the VHD equipment proposed in this thesis is as effective as dynamic visual acuity measurement equipment.
