Background: Stroke survivors and their family caregivers face substantial burdens resulting from a complex interplay of clinical, functional, and psychosocial factors. Community-based resources and social networking are critical for alleviating caregiver burden and improving outcomes for both caregivers and stroke survivors. Objectives: To assess the Saudi population’s readiness to use social platforms for stroke survivors’ recovery and caregiver support. Methods: A cross-sectional descriptive design was employed between March and June 2025 to collect data from community-dwelling individuals using sociodemographic and Technology Readiness Index (TRI) questionnaires. Results: A convenience sample of 576 participants was enrolled in this study. Overall, the participants showed a high level of technology readiness, with a total TRI mean score of M = 3.65, SD = 0.86. Optimism had the highest mean score, followed by innovativeness, insecurity, and discomfort. Significant differences in technology readiness were identified based on age, educational level, employment status, familiarity with modern technology, and healthcare provider status. Conclusions: This study demonstrates a high level of technological readiness across the Saudi population, indicating strong potential for integrating social connection platforms into stroke recovery and caregiver support. These findings align with Saudi Arabia’s Vision 2030 goals for digital transformation in the healthcare sector. Future research should focus on conducting feasibility studies to better understand the desirable features of e-health services and digital solutions within the Saudi community. Full article

Background: Research shows that experiences of sexual violence and the use of substances hurt mental health, predisposing individuals to the development of psychological symptoms of somatization, depression, and anxiety. Objective: The main objective of this study is to assess substance use, sexual violence, and psychological symptoms (BSI-18) in a community sample of Portuguese college students. Participants: 459 college students aged 18 to 50 years participated in this study (mean = 21.55; SD = 3.15). Methods: A survey was created online and disseminated via mailing lists and social networks, and the measurement instruments used were an informed consent form, a sociodemographic questionnaire, the Brief Symptom Inventory-18 (BSI-18), a Sexual Violence Questionnaire, and a Substance Use Questionnaire. Results: We found that men consume more substances, but women report more sexual victimization and psychological symptoms. Non-heterosexuals report significantly more sexual violence experiences and higher substance consumption and psychological symptoms than heterosexuals. Sexual victimization showed strong correlations with both substance use and psychological symptoms, while only some substances correlated with psychological symptoms. Age, gender, sexual orientation, sexual violence, and the substance “Hashish/Marijuana” were shown to be significant and strong predictors of psychological symptoms. Conclusions: As shown in previous studies, sexual and gender minorities are more at risk of experiencing sexual victimization and are more prone to consuming substances and developing psychological symptoms, except for men, who tend to consume more substances. Sexual violence and psychological symptoms are highly correlated, while the relation between substance use and psychological symptoms is not entirely clear. Our findings highlight the need for stronger educational and clinical interventions and increased mental health resources, with training on topics involving sexual violence, substance use, and addressing the specific challenges faced by minority groups, to create safer, inclusive, and more supportive Portuguese universities. A learning management system (LMS) plays a crucial role in supporting students’ educational activities by centralized platforms for course delivery, communication, and student support. Recently, many universities have integrated chatbots into their LMS to assist students with various inquiries and tasks. However, existing chatbots often necessitate human interventions to manually respond to complex queries, resulting in limited scalability and efficiency. In this paper, we present a memory-augmented large language model (LLM) framework that enhances the reasoning and contextual continuity of LMS-based chatbots. The proposed framework first embeds user queries and retrieves semantically relevant entries from various LMS resources, including instructional documents and academic frequently asked questions. Retrieved entries are then filtered through a two-stage confidence filtering process that combines similarity thresholds and LLM-based semantic validation. Validated information, along with user queries, is processed by LLM for response generation. To maintain coherence in multi-turn interactions, the chatbot incorporates short-term, long-term, and temporal event memories, which track conversational flow and personalize responses based on user-specific information, such as recent activity history and individual preferences. To evaluate response quality, we employed a multi-layered evaluation strategy combining BERTScore-based quantitative measurement, an LLM-as-a-Judge approach for automated semantic assessment, and a user study under multi-turn scenarios. The evaluation results consistently confirm that the proposed framework improves the consistency, clarity, and usefulness of the responses. These findings highlight the potential of memory-augmented LLMs for scalable and intelligent learning support within university environments. Full article

This study aimed to evaluate the effects of jump rope training on cardiovascular capacity, assessed with the Ruffier test, and muscular strength, measured using isometric dynamometry (BioFET Mustec, Almere, The Netherlands), within a university physical education program. A total of 52 undergraduate students from non-specialist faculties at the University of Oradea were randomly assigned to either an experimental group (EG) or a control group (CG). Over eight weeks, the EG performed a ten-minute jump rope training session once per week in combination with cardiovascular exercises, while the CG participated only in cardiovascular exercises. Statistical analyses revealed a significant group effect on cardiovascular response (Ruffier Index: p = 0.019; Cohen’s d = −0.271) and a substantial increase in right lower limb strength (p = 0.003; d = 1.026) in the EG compared to the CG. Furthermore, improvements were observed in upper limb strength (left arm: p = 0.010; d = 0.922) and left lower limb strength (p = 0.027; d = 0.779). These findings suggest that incorporating jump rope training into university physical education classes may represent an effective and low-cost strategy to enhance both cardiovascular efficiency and muscular strength in young adults. Given its simplicity and affordability, jump rope training appears feasible for implementation in schools and universities, even where resources are limited. Future research should investigate its long-term effects across different populations. Full article

Background: Urgent care clinics (UCCs) embedded within primary healthcare settings play a vital role in managing acute, non-life-threatening conditions in children. However, limited data exist on medication prescribing patterns in such settings in the Kingdom of Saudi Arabia (KSA), particularly regarding antibiotic use. This study aimed to describe the epidemiology of pediatric urgent care visits and identify factors associated with prescribing within a model primary healthcare (PHC) center. Methods: A retrospective chart review was conducted for all urgent care visits made by pediatric patients (<14 years) at a model PHC center in the KSA for all visits in 2024. Sociodemographic variables, visit timing, diagnosis, and prescription data were extracted from electronic health records. Multivariable logistic regression was used to analyze predictors of medication prescribing. Results: Of the 1016 pediatric urgent care visits, 62.5% resulted in medication prescriptions, and 23.62% of those visits included at least one antibiotic, primarily penicillins (71.33%). Cephalosporins and tetracyclines were not prescribed. Prescriptions were 67% more likely among adolescents and 70% less likely among infants when compared to school-aged children (95% CI = 1.04–2.67 and 95% CI = 0.15–0.61, respectively). Respiratory and ENT-related diagnoses accounted for most prescriptions. No significant sex-based differences in prescribing were observed. Conclusions: The epidemiological patterns observed indicate that respiratory and ENT conditions, as well as seasonal peaks in autumn and winter, are the main drivers of prescribing in pediatric urgent care. These findings have implications for strengthening disease surveillance, anticipating service demand, guiding preventive interventions such as vaccination and health education, and supporting evidence-based planning of primary care resources. Educator agency in the form of choice over learning experiences is widely thought to enhance educator engagement and instructional improvement, yet causal evidence is scarce. We conducted a preregistered randomized controlled trial in an online computer science course with volunteer instructors who teach students worldwide. All instructors (N = 583) received automated feedback on their instruction, with half randomly assigned to have choice over the feedback topic. Choice alone did not increase feedback engagement or yield observable changes in practice, but it raised student attendance—an effect that was strongest for instructors who voluntarily engaged with additional training resources, including training modules and teaching simulations. For this subset of instructors, having choice over feedback had significant positive impacts on their instruction and student outcomes compared to the control group. This suggests that agency in choosing feedback topics may be most effective when combined with instructors’ intrinsic motivation to pursue self-directed improvement. Our study also demonstrates a scalable method for testing design principles in educator training and underscores the need to examine when, how and for whom agency might drive improvement. Diversity, equity, and inclusion, most commonly known as DEI, serves as a framework for practices that promote the fair treatment and full participation of all members of a community. Culturally responsive teaching and critical pedagogy are commonly associated with DEI as part of the larger strategy to validate and inspire learners while improving their self-efficacy and using education to challenge oppressive systems. While DEI is becoming increasingly better known in higher education, Historically Black Colleges or Universities (HBCUs) are heralded in the literature as a model for this work. Nevertheless, there is relatively limited empirical research exploring facets of DEI and culturally responsive teaching on HBCU and other minority-serving institutions’ campuses. This paper examines the campus of an HBCU located in the Mid-Atlantic United States, with an institutional commitment to diversity, equity, and inclusion, via a comprehensive DEI climate study that included separate surveys of students and faculty/staff. More specifically, the study explored whether all community members consider the campus inclusive, whether all community members experience a culture of belonging, whether adequate resources and supports exist for all campus members to succeed, whether faculty exhibit culturally responsive teaching practices, and whether the perceptions of faculty and staff differ from those of students. The purpose of the study was to help address the gap in the DEI literature exploring the practices of minority-serving institutions. According to the results, participants found the University to be an inclusive place, expressing strong satisfaction with the campus climate and experience. Further, when the presence of culturally responsive teaching practices was explored, strong evidence was indicated. Possible areas for improvement include greater supports and resources for LGBTQIA+, Indigenous, and disabled community members. Full article

Modern technologies, tools, and services are rapidly penetrating life, requiring a radical shift in traditional education and the rapid implementation of interdisciplinary STEM (science, technology, engineering, and mathematics) learning approaches. Hence, numerous studies have been initiated to explore key aspects of modern STEM methods and existing opportunities for their evaluation, personalization, and optimization. The article presents results from qualitative research conducted through semi-structured interviews with schoolteachers who have experience in implementing innovative STEM methods in their educational practice. The focus of the study is on the qualitative analysis of modern methods and good practices applied in teaching STEM subjects in Bulgaria. The formulated research questions address the readiness to apply STEM educational methods in Bulgarian schools, focusing on prerequisites such as institutional support, availability of technology infrastructure and resources, and teachers’ competencies. Additionally, these research questions aim to explore teachers’ views on modern teaching methods and approaches currently utilized in STEM education, as well as their perceived effectiveness, engagement, and applicability. The key themes and insights that emerged from the interviews also shed light on the state-of-the-art of STEM education in Bulgarian schools and the current use of teaching methods and techniques for STEM education. The findings revealed that teachers miss time in the curriculum dedicated to STEM lessons and need more integrated learning resources and additional qualifications to apply STEM methods effectively. The interpretation of results analyzes the significance of the findings and their implications for teaching practices and policies in STEM education. Full article

Background: First-Line Nurse Managers (FLNMs) have been recognised as key contributors to achieving organisational objectives, serving as vital intermediaries between management, staff, and patients. Assessing whether the Span of Control (SOC) is appropriate and providing the necessary support for FLNMs to fulfil their responsibilities poses a considerable challenge for healthcare organisations. No tool exists in Spain to guide decisions regarding FLNM’s SOC and resource needs. The aim of this study is to design a tool for assessing the span of control of first-line nurse managers in hospitals. Methods: This study employed a tool development and content validation design to create the EASOC-Nursing instrument (Eliges Aragón SOC tool). The study was conducted in three stages: an integrative literature review, followed by a national Delphi study with 43 experts in nursing management, and finally, focus group discussions. Results: A tool was created to assess first-line nurse managers’ (FLNM) span of control (SOC) using 13 key indicators, organised into four categories: unit (operations and resources, conflicts, and logging and monitoring of activities), professional (staff and competencies), FLNM (autonomy, education, and leadership), and organisation (support systems, education, and research). It includes a total of 31 items and determines SOC adequacy by establishing cut-off points that classify it as below acceptable, appropriate, or excessive. When the SOC is inadequate, the tool provides specific recommendations for support measures, such as the provision of administrative personnel or the appointment of a co-leader. Conclusions: The EASOC-Nursing tool offers a comprehensive evaluation of the core dimensions of the FLNM role and its responsibilities in Spain. Furthermore, it delivers practical guidance on the most suitable types of support to facilitate the attainment of optimal outcomes for both patients and healthcare organisations. In light of the global nursing shortage, the availability of a robust and context-sensitive instrument to assess the SOC enables hospital nursing management to allocate resources more strategically, thereby enhancing working conditions for professionals and contributing to improved patient care outcomes. The hybrid work environment significantly undermines the social well-being of employees in the workplace. Existing research predominantly addresses academics’ well-being challenges without offering practical strategies to counter these issues. This study identifies strategies that higher education institutions must adopt to enhance the social well-being of their academics in hybrid work settings. It employs Demerouti’s Job Demands-Resources (JD-R) model and Baumeister and Leary’s theory of the need to belong as its theoretical framework. Using a cross-sectional qualitative approach, semi-structured interviews were guided by an interview schedule to gather data. The sample comprised 23 academics from three campuses of North-West University, and thematic analysis was utilized to analyse the data. The study revealed that growth strategies, such as training, development, and mentoring, are crucial for fostering a sense of belonging, strengthening work relationships, and helping academics connect in a hybrid work environment. Support strategies like providing peer support, management support, physical resources, effective communication, and improvements in job quality enhance academics’ social well-being in this setting. Relationship strategies, which entail organizing social events and promoting a positive organizational culture, are key to encouraging social well-being in the hybrid work environment. Additionally, reward strategies, such as recognition and direct compensation, are essential for reinforcing a sense of belonging, improving work relationships, and enhancing social connections in a hybrid work environment. Intentional, coach-oriented, sensible, and inclusive leadership is vital. The findings offer valuable insights for higher education institutions to adopt a more comprehensive approach to managing the well-being of academic employees. This highlights the need to focus not only on mental and psychological health but also on social well-being. Full article

Our study explores the performance gains of Retrieval-Augmented LLMs over baseline LLMs while also identifying the trade-off opportunity between smaller-parameter LLMs augmented with user-specific data to larger parameter LLMs. (2) Methods: We experimented with four different LLMs, each with a different number of parameters, to generate outputs. These outputs were then evaluated across seven lexical and semantic metrics to identify performance trends in Retrieval-Augmented Generation (RAG)-Augmented LLMs and analyze the impact of parameter size on LLM performance. (3) Results and Discussions: We have synthesized 968 different combinations to identify this trend with the help of different LLM sizes/parameters: TinyLlama 1.1B, Mistral 7B, Llama 3.1 8B, and Llama 1 13 B. These studies were grouped into two themes: RAG-Augmented LLM percentage improvements to baseline LLMs and compelling trade-off possibilities of RAG-Augmented smaller-parameter LLMs to larger-parameter LLMs. Our experiments show that RAG-Augmented LLMs demonstrate high lexical and semantic scores relative to baseline LLMs. This offers RAG-Augmented LLMs as a compelling trade-off for reducing the number of parameters in LLMs and lowering overall resource demands. (4) Conclusions: The findings outline that by leveraging RAG-Augmented LLMs, smaller-parameter LLMs can perform better or equivalently to large-parameter LLMs, particularly demonstrating strong lexical improvements. They reduce the risks of hallucination and keep the output more contextualized, making them a better choice for knowledge-intensive content in academic and research sectors. Full article

Background/Objectives: Discrimination has wide-ranging implications, affecting patients’ trust in healthcare professionals and their intentions to seek care. It can cause barriers that can affect access to care, especially among racial and ethnic minority groups in mental healthcare settings. Hence, we aim to examine the relationship between racial discrimination and reasons for using telehealth services. Method: Using the 2022 Health Information National Trends Survey (HINTS-6), we isolated 6017 participants who reported on discrimination inquiry. The primary dependent variable in this study is self-reported discrimination in healthcare, while the main independent variable is the reason for the most recent telehealth visit, and several socio-demographic variables were included as covariates, including age, sex, education, income, and marital status. Descriptive statistics were generated, and multivariable regression analysis was estimated as well, with a p-value < 0.05. Results: Non-Hispanic Black individuals had significantly higher odds of reporting discrimination compared to non-Hispanic White individuals (crude OR: 11.85, 95% CI: 7.67–18.309). Similarly, Hispanic individuals (crude OR: 4.626, 95% CI: 2.899–7.384) and individuals of other racial backgrounds (crude OR: 6.883, 95% CI: 4.04–11.729) had significantly increased odds of experiencing discrimination. Conclusions: The findings of this study offer critical insights into the determinants of perceived discrimination within telehealth services. Given the sustained integration of telehealth into healthcare delivery, it is imperative to develop and implement targeted strategies that provide education and resources to promote health equity among Non-Hispanic Black patients. The aim of this study is to analyze the representation of geological, mining, processing, and environmental processes in platform Minecraft. Based on a methodological comparison of in-platform mechanics with technological and geoscientific procedures, the article assesses the degree of accuracy, simplification, and didactic applicability of individual processes related to the extraction and use of mineral resources. The analysis is structured into seven main thematic areas covering the entire resource value chain—from geological exploration through mining, ore beneficiation and processing, to quantitative indicators (e.g., waste-to-ore ratio), fluid resources, and environmental impacts. Special attention is given to the potential of modifications that significantly enhance the complexity and accuracy of simulated processes. The results show that Minecraft, enriched with thematic mods, can serve as an accessible and flexible tool for the popularization and education of industrial and geoscientific processes, while engaging a wide range of audiences. Background/Objectives: Chronic kidney disease (CKD) requires intensive dietary and lifestyle management, yet patient engagement and access to tailored education remain limited, particularly outside clinical settings. This study describes the development and implementation of NephroGo, and evaluates its usability, user engagement, and perceived acceptability among patients with CKD. Methods: The app was developed based on clinical and dietary guidelines, incorporating personalized nutrient recommendations, dialysis tracking, and educational content. Technically, it features a Django backend, Flutter mobile frontend, and secure cloud-based hosting. User feedback was collected through one-time interviews (n = 10) and a standardized Mobile App Rating Scale (MARS) survey (n = 32). Longitudinal usage data over four years were also analyzed. Results: Initially, NephroGo was downloaded by 204 users, of whom 93.6% were considered active users based on defined behavioral engagement thresholds. Over a four-year period, the app accumulated a total of 1670 downloads. This study focuses on evaluating user engagement, usability, and perceived acceptability of the NephroGo app over a four-year period. Most users were female (52.3%) and aged 30–65. Stage 5 CKD patients and those undergoing peritoneal dialysis (PD) had the highest engagement. The most-used feature was the personalized nutrition calculator, with sodium being the most frequently exceeded nutrient. The average MARS score was 4.09 ± 0.66, with functionality rated highest (4.27 ± 0.74). App ratings were significantly higher among users referred by physicians (p = 0.039). Conclusions: NephroGo offers a scalable digital tool to support dietary management and health monitoring, with potential to complement standard nephrology care in a resource-conscious manner.
