Fentanyl and its analogues represent a severe threat due to their extreme potency and increasing prevalence in illicit drug supplies. Even trace amounts (on the order of a couple of milligrams) can be lethal, contributing to a surge in opioid overdose deaths worldwide. Beyond the public health crisis, fentanyl has emerged as a security concern, with the potential for deliberate use as a chemical agent in CBRN scenarios. This underscores the critical need for rapid and accurate detection methods that can be deployed by security forces and first responders. Modern technology offers a range of solutions—from portable mass spectrometers and spectroscopic devices to electrochemical sensors and immunoassay kits—that enable on-site identification of fentanyl and its analogues. This review provides a comprehensive overview of detection techniques, examining their capabilities and applications in law enforcement, border control, and CBRN incident response. We highlight how integration of advanced sensors with machine learning is enhancing detection accuracy in complex field environments. Challenges such as operational constraints and the ever-evolving variety of fentanyl analogues are discussed, and future directions are recommended to improve field-deployable detection tools for safety and security applications. An advanced anodic stripping voltammetry (ASV)-based Micro Electro Mechanical System (MEMS) sensor for cadmium (Cd) detection is presented in this study, which is cost-effective and efficient for in situ water monitoring, providing a crucial early warning mechanism, streamlining environmental monitoring, and facilitating timely intervention to safeguard public health and environmental safety. The rationale behind this work is to address the critical need for an in situ monitoring system for cadmium (Cd) in freshwater sources, particularly those adjacent to agricultural fields. Cd(II) is a highly toxic heavy metal that poses a significant threat to agricultural ecosystems and human health due to its rapid bioaccumulation in plants and subsequent entry into the food chain. The developed analytic device is composed of a commercial mercury salt-modified graphite screen-printed electrode (SPE) with a custom-designed innovative polydimethylsiloxane (PDMS) flow detection cell. The flow cell was prototyped using 3D printing and replica moulding, with its design and performance validated through COMSOL Multiphysics simulations to optimize inflow conditions and ensure maximum analyte dispersion on the working electrode surface. Chemical detection was performed using square wave voltammetry, demonstrating a linear response for Cd(II) concentrations of 0 to 20 µg/L. The system exhibited robust analytical performance, enabling 25–30 daily analyses with consistent sensitivity within the Limit of Detection (LoD) set by the law of 3 µg/L. Full article

Public stigma toward psychological help-seeking is a critical barrier to mental health service utilization, particularly among university students in culturally conservative and academically demanding disciplines such as law. In China, where Confucian values emphasize social conformity and face preservation, law students may internalize societal narratives that associate mental illness with personal weakness, deterring them from accessing psychological services. This study translated and examined the psychometric properties of the Stigma Scale for Receiving Psychological Help (SSRPH) among Chinese law students. A total of 1257 undergraduate law students from five universities in China participated in the study. Exploratory factor analysis (EFA) was conducted on a randomly selected subsample (n = 628) to examine the scale’s factor structure, followed by confirmatory factor analysis (CFA) on a second subsample (n = 629). Results supported a unidimensional factor structure with strong internal consistency (α = 0.82). CFA yielded a good model fit (CFI = 0.97, TLI = 0.95). Significant negative correlations with help-seeking attitudes (r = −0.37, p < 0.001) supported discriminant validity. While further validation is warranted, the Chinese SSRPH appears suitable for assessing perceived public stigma in legal education contexts and may inform future research and program design in stigma reduction and mental health promotion. In recent years, Italy has experienced a significant increase in violence against healthcare workers, mirroring a global trend. Manifesting as verbal, physical, psychological, and material aggression, this phenomenon endangers both personnel safety and the foundational principles of the National Health Service (SSN) as outlined in Article 32 of the Italian Constitution. The escalation—most acute in emergency departments, psychiatric units, inpatient wards, and community services—affects a broad spectrum of professionals, compromising care quality and institutional integrity. Data from the FNOMCeO-CENSIS Report 2023–2024 reveal over 18,000 reported incidents in 2024, with verbal assaults disproportionately affecting female nursing staff. The COVID-19 pandemic further exacerbated systemic vulnerabilities, heightening user dissatisfaction and psychological strain among healthcare providers. In response, legislative actions—such as Law No. 113/2020 and Decree-Law No. 137/2024—aim to strengthen prevention, monitoring, and penal measures. This article examines legal, institutional, and organizational responses, including on-the-ground and hospital-based strategies to mitigate violence. Adopting a multidisciplinary perspective, it analyzes recent policy developments, regional dynamics, and victim-perpetrator profiles, arguing that safeguarding healthcare environments is both a public security priority and an ethical imperative essential to preserving the dignity of care work and the resilience of the health system. This study presents the scientific and methodological foundation of Spain’s first national framework for the ethical management of community cat populations: the Action Plan for the Management of Community Cat Colonies (PACF), launched in 2025 under the mandate of Law 7/2023. This pioneering legislation introduces a standardized, nationwide obligation for trap–neuter–return (TNR)-based management of free-roaming cats, defined as animals living freely, territorially attached, and with limited socialization toward humans. The PACF aims to support municipalities in implementing this mandate through evidence-based strategies that integrate animal welfare, biodiversity protection, and public health objectives. Using standardized data submitted by 1128 municipalities (13.9% of Spain’s total), we estimated a baseline population of 1.81 million community cats distributed across 125,000 colonies. These data were stratified by municipal population size and applied to national census figures to generate a model-ready demographic structure. We then implemented a stochastic simulation using Vortex software to project long-term population dynamics over a 25-year horizon. The model integrated eight demographic–environmental scenarios defined by a combination of urban–rural classification and ecological reproductive potential based on photoperiod and winter temperature. Parameters included reproductive output, mortality, sterilization coverage, abandonment and adoption rates, stochastic catastrophic events, and territorial carrying capacity. Under current sterilization rates (~20%), our projections indicate that Spain’s community cat population could surpass 5 million individuals by 2050, saturating ecological and social thresholds within a decade. In contrast, a differentiated sterilization strategy aligned with territorial reproductive intensity (50% in most areas, 60–70% in high-pressure zones) achieves population stabilization by 2030 at approximately 1.5 million cats, followed by a gradual long-term decline. This scenario prioritizes feasibility while substantially reducing reproductive output, particularly in rural and high-intensity contexts. The PACF combines stratified demographic modeling with spatial sensitivity, offering a flexible framework adaptable to local conditions. It incorporates One Health principles and introduces tools for adaptive management, including digital monitoring platforms and standardized welfare protocols. While ecological impacts were not directly assessed, the proposed demographic stabilization is designed to mitigate population-driven risks to biodiversity and public health without relying on lethal control. By integrating legal mandates, stratified modeling, and realistic intervention goals, this study outlines a replicable and scalable framework for coordinated action across administrative levels. It exemplifies how national policy can be operationalized through data-driven, territorially sensitive planning tools. The findings support the strategic deployment of TNR-based programs across diverse municipal contexts, providing a model for other countries seeking to align animal welfare policy with ecological planning under a multi-level governance perspective. Background: Current food systems contribute significantly to poor public health and environmental degradation. With increasing rates of chronic disease and undernutrition globally and in the United States of America (U.S.), transforming food systems toward sustainability is a critical public health priority. Objectives: This narrative review aimed to summarize U.S. policies from the past decade relevant to sustainable food systems, focusing on four domains—availability, accessibility, affordability, and desirability—proposed by the 22nd Annual Harvard Nutrition Obesity Symposium. Methods: Systematic searches were conducted using MEDLINE (PubMed), Scopus, U.S. Congress websites, and Google searches. Studies and policies published between 2013 and 2023 that addressed at least one of the four domains were included. Policies were reviewed according to their alignment with the policy lifecycle framework, encompassing formulation through implementation. Results: A review of the final 632 articles explained that, despite growing interest in sustainable food systems, there is a lack of comprehensive U.S. policies that address the four domains in an integrated manner. Most initiatives were limited in scope, often school-based, and not explicitly sustainability-focused. Food availability and accessibility policies exist but remain fragmented, while affordability and desirability domains are severely underrepresented. Few laws or bills have been enacted or evaluated for population-level or environmental outcomes. Conclusions: Transforming U.S. food systems requires more robust evidence-based policy development and evaluation. There is an urgent need for integrated multisectoral policy frameworks to ensure health, equity, and sustainability across all food system domains. Full article

Incineration at sea is a significant source of marine pollution, threatening biodiversity and public health. Although Peru ratified the 1996 London Protocol in 2018, key deficiencies persist in its domestic legal framework, particularly the absence of clear and internationally aligned definitions for “incineration” and “incinerator.” These gaps hinder effective enforcement by the National Maritime Authority (NMA-DICAPI) and limit regulatory compliance with international obligations. This study analyzes Peru’s current legislation in light of the London Protocol and includes a comparative overview of regional regulatory approaches in Latin America. Based on this analysis, the study identifies regulatory inconsistencies that compromise environmental protection and proposes three key legal reforms: (1) refining the definition of “incineration” to reflect international standards; (2) formally incorporating a definition for “incinerator”; and (3) establishing specific administrative and economic sanctions for related infractions. Through comparative analysis with Mexico and Colombia’s approaches, we propose targeted amendments including refined definitions aligned with IMO standards and explicit administrative/economic sanctions. Implementing these recommendations would enhance Peru’s legal clarity, enforcement capacity, and compliance with international maritime law, reinforcing its role as a responsible actor in marine environmental governance. The extensive use of conventional pesticides has been a fundamental strategy in modern agriculture for controlling pests and increasing crop productivity; however, their improper application poses significant risks to human health and environmental sustainability. This review compiles scientific evidence linking pesticide exposure to oxidative stress and genotoxic damage, particularly affecting rural populations and commonly consumed foods, even at levels exceeding the maximum permissible limits in fruits, vegetables, and animal products. Additionally, excessive pesticide use has been shown to alter soil microbiota, negatively compromising long-term agricultural fertility. In response to these challenges, recent advances in nanotechnology offer promising alternatives. This review highlights the development of nanopesticides designed for controlled release, improved stability, and targeted delivery of active ingredients, thereby reducing environmental contamination and increasing efficacy. Moreover, emerging nanobiosensor technologies, such as e-nose and e-tongue systems, have shown potential for real-time monitoring of pesticide residues and soil health. Although pesticides are still necessary, it is crucial to implement stricter laws and promote sustainable solutions that ensure safe and responsible agricultural practices. The need for evidence-based public policy is emphasized to regulate pesticide use and protect both human health and agricultural resources. Full article

Endometrial cancer (EC) is the most common gynecological malignancy in developed countries. Risk factors for EC include metabolic alterations (obesity, metabolic syndrome, insulin resistance), hormonal imbalance, age at menopause, reproductive factors, and inherited conditions, such as Lynch syndrome. For the inherited forms, several genes had been implicated in EC occurrence and development, such as POLE, MLH1, TP53, PTEN, PIK3CA, PIK3R1, CTNNB1, ARID1A, PPP2R1A, and FBXW7, all mutated at high frequency in EC patients. However, gene function impairment is not necessarily caused by mutations in the coding sequence of these and other genes. Gene function alteration may also occur through post-transcriptional control of messenger RNA translation, frequently caused by microRNA action, but transcriptional impairment also has a profound impact. Here, we review how chromatin modifications change the expression of genes whose impaired function is directly related to EC etiopathogenesis. Chromatin modification plays a central role in EC. The modification of chromatin structure alters the accessibility of genes to transcription factors and other regulatory proteins, thus altering the intracellular protein amount. Thus, DNA structural alterations may impair gene function as profoundly as mutations in the coding sequences. Hence, its central importance is in the diagnostic and prognostic evaluation of EC patients, with the caveat that chromatin alteration is often difficult to identify and needs investigations that are specific and not broadly used in common clinical practice. The different phases of the healthy endometrium menstrual cycle are characterized by differential gene expression, which, in turn, is also regulated through epigenetic mechanisms involving DNA methylation, histone post-translational modifications, and non-coding RNA action. From a medicolegal and policy-making perspective, the implications of using epigenetics in cancer care are briefly explored as well. Epigenetics in endometrial cancer is not only a topic of biomedical interest but also a crossroads between science, ethics, law, and public health, requiring integrated approaches and careful regulation. Natural Ventilation Technique of uNVeF in Urban Residential Unit Through a Case Study

The present study was motivated by the need to enhance indoor air quality and reduce airborne disease transmission in dense urban environments where high-rise residential buildings face challenges in achieving effective natural ventilation. The problem lies in the lack of scalable and convenient tools to optimize natural ventilation rate, particularly in urban settings with varying building heights. To address this, the scientific technique developed with an innovative metric, the urbanized natural ventilation effectiveness factor (uNVeF), integrates regression analysis of wind direction, velocity, air change rate per hour (ACH), window configurations, and building height to quantify ventilation efficiency. By employing a field measurement methodology, the measurements were conducted across 25 window-opening scenarios in a 13.9 m2 residential unit on the 35/F of a Hong Kong public housing building, supplemented by the Hellman Exponential Law with a site-specific friction coefficient (0.2907, R2 = 0.9232) to estimate the lower floor natural ventilation rate. The results confirm compliance with Hong Kong’s statutory 1.5 ACH requirement (Practice Note for Authorized Persons, Registered Structural Engineers, and Registered Geotechnical Engineers) and achieving a peak ACH at a uNVeF of 0.953 with 75% window opening. The results also revealed that lower floors can maintain 1.5 ACH with adjusted window configurations. Using the Wells–Riley model, the estimation results indicated significant airborne disease infection risk reductions of 96.1% at 35/F and 93.4% at 1/F compared to the 1.5 ACH baseline which demonstrates a strong correlation between ACH, uNVeF and infection risks. The uNVeF framework offers a practical approach to optimize natural ventilation and provides actionable guidelines, together with future research on the scope of validity to refine this technique for residents and developers. The implications in the building industry include setting up sustainable design standards, enhancing public health resilience, supporting policy frameworks for energy-efficient urban planning, and potentially driving innovation in high-rise residential construction and retrofitting globally. Background/Objectives: Antimicrobial consumption (AMC) surveillance and antimicrobial stewardship (AMS) constitute effective strategies to combat the increasing antimicrobial resistance rates worldwide. Post-Soviet countries (Armenia, Azerbaijan, Belarus, Estonia, Georgia, Kazakhstan, Kyrgyzstan, Latvia, Lithuania, Moldova, the Russian Federation, Tajikistan, Turkmenistan, Ukraine, and Uzbekistan) implemented various elements of AMC surveillance and AMS to different extents. The limited quantity and quality of data from post-Soviet countries make it difficult to assess health system performance; therefore, this region is a blind spot in global AMR monitoring. This systematic review assesses and characterises AMC surveillance and AMS implementation in post-Soviet countries. Methods: Evidence was compiled via a search in PubMed, Google Scholar, Embase, CyberLeninka, and Scopus. The eligibility criteria included AMC surveillance- and AMS-related papers in human health within defined regions and timelines. Some literature from the official websites of international and national health organisations was included in the search. Results: As a result of the searches, screening, and critical appraisal, three peer-reviewed publications and 31 documents were selected for analysis. Eleven out of fifteen countries with updated national action plans for combating antimicrobial resistance have defined AMC surveillance and AMS as strategic objectives. All 15 examined countries submitted antimicrobial consumption data to international networks and reported the existence of approved laws and regulations on antibiotic sales. However, disparities exist in the complexity of monitoring systems and AMS implementation between high-income and low-income countries in the region. Conclusions: This review provides key insights into the existing AMC surveillance and AMS implementation in former Soviet countries. Police officers work in high-stress environments that demand emotional resilience, interpersonal skills, and effective communication. Occupational stress can negatively impact their motivation, conflict resolution abilities, and professional effectiveness. Loving-Kindness Meditation (LKM), a mindfulness-based intervention focused on cultivating compassion and empathy, has shown promise in enhancing prosocial attitudes and emotional regulation. With the rise of short video platforms, digital interventions like video-guided LKM may offer accessible mental health support for law enforcement. This study examines the effects of short video app-guided LKM on police officers’ mindfulness, public service motivation (PSM), conflict resolution skills (CRSs), and communication skills (CSSs). It aims to determine whether LKM can enhance these psychological and professional competencies. A randomized controlled trial (RCT) was conducted with 110 active-duty police officers from a metropolitan police department in China, with 92 completing the study. Participants were randomly assigned to either the LKM group (n = 46) or the waitlist control group (n = 46). The intervention consisted of a 6-week short video app-guided LKM program with daily 10 min meditation sessions. Pre- and post-intervention assessments were conducted using several validated scales: the Mindfulness Attention Awareness Scale (MAAS), the Public Service Motivation Scale (PSM), the Conflict Resolution Styles Inventory (CRSI), and the Communication Competence Scale (CCS). A 2 (Group: LKM vs. Control) × 2 (Time: Pre vs. Post) mixed-design MANOVA was conducted to analyze the effects. Statistical analyses revealed significant group-by-time interaction effects for PSM (F(4,177) = 21.793, p < 0.001, η2 = 0.108), CRS (F(4,177) = 20.920, p < 0.001, η2 = 0.104), and CSS (F(4,177) = 49.095, p < 0.001, η2 = 0.214), indicating improvements in these areas for LKM participants. However, no significant improvement was observed for mindfulness (F(4,177) = 2.850, p = 0.930, η2 = 0.016). Short video app-guided LKM improves public service motivation, conflict resolution skills, and communication skills among police officers but does not significantly enhance mindfulness. These findings suggest that brief, digitally delivered compassion-focused programs can be seamlessly incorporated into routine in-service training to strengthen officers’ prosocial motivation, de-escalation competence, and public-facing communication, thereby fostering more constructive police–community interactions. Background: The rapid integration of artificial intelligence (AI) technologies into healthcare systems presents new opportunities and challenges, particularly regarding legal and ethical implications. In Saudi Arabia, the lack of legal awareness could hinder safe implementation of AI tools. Methods: A sequential explanatory mixed-methods design was employed. In Phase One, a structured electronic survey was administered to 357 clinicians across public and private healthcare institutions in Saudi Arabia, assessing legal awareness, liability concerns, data privacy, and trust in AI. In Phase Two, a qualitative expert panel involving health law specialists, digital health advisors, and clinicians was conducted to interpret survey findings and identify key regulatory needs. Results: Only 7% of clinicians reported high familiarity with AI legal implications, and 89% had no formal legal training. Confidence in AI compliance with data laws was low (mean score: 1.40/3). Statistically significant associations were found between professional role and legal familiarity (χ2 = 18.6, p < 0.01), and between legal training and confidence in AI compliance (t ≈ 6.1, p < 0.001). Qualitative findings highlighted six core legal barriers including lack of training, unclear liability, and gaps in regulatory alignment with national laws like the Personal Data Protection Law (PDPL). Conclusions: The study highlights a major gap in legal readiness among Saudi clinicians, which affects patient safety, liability, and trust in AI. Although clinicians are open to using AI, unclear regulations pose barriers to safe adoption. Experts call for national legal standards, mandatory training, and informed consent protocols. A clear legal framework and clinician education are crucial for the ethical and effective use of AI in healthcare. Full article

Planetary health is a transdisciplinary concept that erases the dividing lines between individual and community health, and the natural systems that support the wellbeing of humankind. Despite planetary health’s broad emphasis on justice, the promotion of science-based policies, and stated commitments to fairness, equity, and harm reduction, the criminal justice system has largely escaped scrutiny. This seems to be a major oversight, especially because the criminalization of mental illness is commonplace, and the system continues to be oriented around a prescientific compass of retribution and folk beliefs in willpower, moral fiber, and blameworthiness. Justice-involved juveniles and adults are funneled into landscapes of mass incarceration with ingrained prescientific assumptions. In non-criminal realms, such as obesity, there is a growing consensus that folk psychology ideas must be addressed at the root and branch. With this background, the Nova Institute for Health convened a transdisciplinary roundtable to explore the need for a ‘Copernican Revolution’ in the application of biopsychosocial sciences in law and criminal justice. This included discussions of scientific advances in neurobiology and omics technologies (e.g., the identification of metabolites and other biological molecules involved in behavior), the need for science education, ethical considerations, and the public health quarantine model of safety that abandons retribution. The Role of Mental Health, Recent Trauma, and Suicidal Behavior in Officer-Involved Shootings: A Public Health Perspective

This study uses a public health approach to identify the comorbid risk factors and protective factors that influence the likelihood of an officer-involved shooting (OIS). Methods: We analyzed 7.5 years of hospital inpatient data obtained from the state of Texas. The OIS subjects (n = 177) were civilians who were shot during a legal intervention involving law enforcement. The control group (n = 33,539) included persons who were hospitalized due to injuries from a car accident. Logistic regression models were used to identify the predictors of an OIS incident. The data included information on chronic diseases, vulnerable population status, health insurance, mental health diagnoses, substance use disorders, and recent trauma. Results: About one-fourth (24.3%) of OIS subjects had a diagnosed mental illness, compared to 8.4% of control subjects (p < 0.001). Factors that greatly increased the risk for an OIS included the following: schizophrenia (AOR = 2.7; CI: 1.6, 4.6), methamphetamine use disorder (AOR = 3.5; CI: 2.2, 5.5), and recent family bereavement (AOR = 8.5; CI: 1.8, 39.6). Six subjects (3.4%) were persons experiencing homelessness (PEH). Protective factors that lowered the risk for an OIS included commercial health insurance (AOR = 0.27; CI: 0.17, 0.45) and Medicaid insurance (AOR = 0.61; CI: 0.11, 0.93). Conclusions: These findings underscore the preventable nature of many OIS incidents, especially those that involve untreated mental illness, homelessness, substance use disorders, and recent trauma. Addressing the root causes of these incidents will likely require interdisciplinary collaboration among law enforcement, public health agencies, and social services. Full article
