The garment manufacturing industry, being labor-intensive, has long faced challenges in automating the sewing process due to the flexibility and deformability of fabrics. This study proposes a novel strategy for automated sewing by integrating soft fingers and machine vision technology. Firstly, leveraging the flexibility and adjustability of soft fingers, combined with the motion characteristics of the sewing machine, a sewing model was established to achieve coordinated operation between the soft fingers and the sewing machine. Experimental results indicate that the fabric feeding speed and waiting time of the soft fingers are significantly correlated with the sewing speed and stitch density of the sewing machine, but not with the fabric properties. Secondly, machine vision technology was employed to inspect the quality of the sewn fabrics, achieving a classification accuracy of 97.84%. This study not only provides theoretical and technical support for the intelligent upgrading of the garment manufacturing industry but also lays the foundation for the automation of complex sewing processes such as quilting. Future research will further optimize the system’s performance and expand its applications in more complex sewing tasks. Full article

Objectives: The aim was to measure wait times for rheumatologist consultation and disease-modifying antirheumatic drug (DMARD) treatment and examine their association with travel time to primary care practitioners (PCP) and rheumatologists within a centralized intake system, respectively. Methods: Within a centralized intake system serving 4.2 million people, we measured wait time for rheumatologist consultations and DMARD treatment for an RA incidence cohort between 1 April 2015 and 31 March 2020. Wait times were reported as the median with the interquartile range (IQR). Using multivariate logistics regression models, we examined the impact of travel times to primary/rheumatology care on wait times for rheumatologist consultation (28-day benchmark) and DMARD treatment (14-day benchmark). Travel times were defined according to quantiles and pre-defined categories. Results: The median wait time was 47 days (IQR: 18–114) for rheumatologist consultations (36% meeting the benchmark) and 35 days (IQR: 1–132) for DMARD treatment (43% meeting the benchmark). Patients living >120 min away had lower odds of meeting the 28-day consultation benchmark compared with those within 30 min (OR 0.64; 95% CI: 0.42–0.97). Compared with patients driving ≤30 min, lower odds of meeting the 14-day benchmark for DMARD treatment were observed for those driving over 60 min to PCPs (OR 0.62; 95% CI: 0.39–0.99) and patients driving 30–60 min to rheumatologists (OR 0.68; 95% CI: 0.55–0.85). Conclusion: RA management was suboptimal due to low rates of meeting RA consultation and treatment benchmarks, which was significantly associated with long travel times to both primary and RA care within a centralized triage system. This highlights the need for complementary strategies (e.g., tele-rheumatology, travel support, or alternate care providers) to ensure timely RA care in rural and remote communities. Full article

Advanced heart failure (AdHF) is a progressive condition with a high morbidity and mortality burden despite optimal medical therapy. Heart transplant (HT) and left ventricular assist device (LVAD) represent the only two life-prolonging options in AdHF. Unfortunately, only a minority of AdHF patients are eligible for these life-saving therapies, and even patients who are candidates for HT usually incur prolonged waiting list times. Intermittent or continuous home-based inotropic therapy offers a potential solution to improve quality of life, reduce recurrent hospitalizations, and maintain organ function, both for the stabilization of patients who are ultimately candidates for life-saving therapies and for palliative care in those without other therapeutic options. In this review, we summarize the current literature on the role of home inotropes in managing AdHF, emphasizing the current evidence on the most adopted agents, the practical considerations for their administration, and the possible different preferred utilization of these agents. Finally, we address gaps in the literature and outline future research directions to enhance therapeutic options and outcomes. Full article

Emergency medical services confront significant challenges in delivering timely patient assessments within geographically isolated or disaster-impacted regions. While drones (unmanned aircraft systems, UAS) show transformative potential in healthcare, standardized protocols for drone-assisted patient evaluations remain underdeveloped. This study introduces the ABCD-IN-BARS protocol, a 9-step telemedicine checklist integrating patient-assisted maneuvers and drone technology to systematize remote emergency assessments. A wait-list randomized controlled trial with 68 first-aid-trained volunteers evaluated the protocol’s feasibility. Participants underwent web-based modules and in-person simulations and were randomized into immediate training or waitlist control groups. The ABCD-IN-BARS protocol was developed via a content validity approach, incorporating expert-rated items from the telemedicine literature. Outcomes included time-to-assessment, provider confidence (Modified Cooper–Harper Scale), measured at baseline, post-training, and 3-month follow-up. Ethical approval and informed consent were obtained. Most of the participants can complete the assessment with a cue card within 4 min. A mixed-design repeated measures ANOVA assessed the effects of Time (baseline, post-test, 3-month follow-up within subject) on assessment durations. Assessment times improved significantly over three time points (p = 0.008), improving with standardized protocols, while patterns were similar across groups (p = 0.101), reflecting skill retention at 3 months and not affected by injury or not. Protocol adherence in simulated injury identification increased from 63.3% pre-training to 100% post-training. Provider confidence remained high (MCH scores: 2.4–2.7/10), and Technology Acceptance Model (TAM) ratings emphasized strong Perceived Usefulness (PU2: M = 4.48) despite moderate ease-of-use challenges (EU2: M = 4.03). Qualitative feedback highlighted workflow benefits but noted challenges in drone maneuvering. The ABCD-IN-BARS protocol effectively standardizes drone-assisted emergency assessments, demonstrating retained proficiency and high usability. While sensory limitations persist, its modular design and alignment with ABCDE principles offer a scalable solution for prehospital care in underserved regions. Further multicenter validation is needed to generalize findings. Full article

This study presents an integrated assessment of anchorage-related emissions and air quality impacts in the Panama Canal region through Automatic Identification System (AIS) data, bottom-up emission estimation, and atmospheric dispersion modeling. One year of terrestrial AIS observations (July 2024–June 2025) captured 4641 vessels with highly variable waiting times: mean 15.0 h, median 4.9 h, with maximum episodes exceeding 1000 h. Annual emissions totaled 1,390,000 tons of CO2, 20,500 tons of NOx, 4250 tons of SO2, 656 tons of PM10, and 603 tons of PM2.5, with anchorage activities contributing 497,000 tons of CO2, 7010 tons of NOx, 1520 tons of SO2, 232 tons of PM10, and 214 tons of PM2.5. Despite the main engines being shut down during anchorage, these activities consistently accounted for 34–36% of the total emissions across all pollutants. High-resolution emission mapping revealed hotspots concentrated in anchorage zones, port berths, and canal approaches. Dispersion simulations revealed strong meteorological control: northwesterly flows transported emissions offshore, sea–land breezes produced afternoon fumigation peaks affecting Panama City, and southerly winds generated widespread onshore impacts. These findings demonstrate that anchorage operations constitute a major source of shipping-related pollution, highlighting the need for operational efficiency improvements and meteorologically informed mitigation strategies. A trend analysis mentioned that the global automotive Vehicle-to-Everything—also called V2X—market size will be reached at several billions in the near future. This information clearly highlights the importance of developing V2X communication. Nowadays, automobile manufacturers have introduced vehicles equipped with driver assistance and even conditional autonomous driving features. Light detection and ranging (LiDAR) components are used in sensor networks to detect objects around. Also, vehicles take advantage of LiDAR sensors to discover the neighbor cars in cognitive systems for road safety. Carrying on from our previous works, we found that organizing vehicles into groups can enhance the safety of the vehicle networks by LiDAR assistance. However, the success rate and reliability of grouping vehicles is an important issue. Also, enhancing existing Vehicle-to-Vehicle (V2V) communication mechanisms remains a key factor in ensuring that emergency messages can be transmitted both timely and accurately. To address this, in this research, a method is proposed to make vehicles on the road be self-organized well for Intelligent Transportation Systems (ITS). Also, we found that before data in each car is transmitted, the scenario that data is queued for waiting a random time exponentially distributed outperforms data being sent immediately. Full article

Online peer support platforms are vital, scalable resources for mental health, yet their effectiveness is frequently undermined by inefficient user matching, severe participation inequality, and subsequent “super-helper” burnout. This study introduces TrailMap, a novel peer-matching algorithm inspired by the decentralised foraging strategies of ant colonies. By treating user interactions as paths that gain or lose “pheromone” based on helpfulness ratings, the system enables the community to collectively and adaptively identify its most effective helpers. A two-phase validation study was conducted. First, an agent-based simulation demonstrated that TrailMap reduced the mean time to a helpful response by over 70% and improved workload equity compared to random routing. Second, a four-week randomised controlled pilot study with human participants confirmed these gains, showing a 76% reduction in median wait time and significantly higher perceived helpfulness ratings. The findings suggest that by balancing the workload, TrailMap enhances not only the efficiency but also the socio-technical sustainability of online support communities. TrailMap provides a practical, nature-inspired method for building more resilient and equitable online support communities, enhancing access to effective mental health support. Full article

Background/Objectives: While immigrants represent 21% of Canada’s total population, they represent 30% of the country’s older population. Sociocultural and economic barriers to the Canadian healthcare system have been frequently reported among older adult immigrants. These barriers are intricately linked to a vastly understudied phenomenon-transnational health practices (THP), which may involve travelling to home countries for healthcare, accessing medicine and health-related information and resources linked to home countries. This study aimed to explore the relationships among local healthcare experiences in Canada, individual characteristics and use of THP among older adult immigrants. Methods: A mixed-methods approach was used combining statistical, spatial and qualitative methods to analyze group patterns of THP and its influencing factors. Primary data was collected through surveys and focus groups of older Mainland Chinese and older South Korean immigrants residing in Toronto. They are the two largest East Asian groups in Canada, with documented transnational ties with their home country. Results: The study found that THP were sought by both groups but were more prevalent among older Chinese immigrants. By integrating quantitative and qualitative analyses, the study revealed complex relationships between THP and barriers in local healthcare access relating to wait times, cost, language, availability, spatial accessibility and quality of care, for different types of care including primary, specialist, eye and dental care. Conclusions: The study generates new knowledge on THP in Canada and adds to the growing body of literature on transnational healthcare practices and behaviours among migrants across different countries and regions. It provides implications to inform health policy and deliver care for older adult immigrants as their populations continue to increase. Full article

Background/Objectives: Outpatient surgery enhances patient comfort while reduces surgical wait times and healthcare costs compared to inpatient procedures. This study evaluates the individual feasibility of performing transoral trans muscular cordectomies with microelectrodes (TOMES) on an outpatient basis. Methods: This observational study analyses TOMES types III, IV, and V cordectomies performed from January 2002 to December 2023. Key metrics include patient demographics, procedural details, incidence of bleeding, anticoagulation and other comorbidities. Results: Of the 143 procedures, 127 were cancer-related, while 16 were due to bilateral vocal cord paralysis. The average age was 65, with a predominantly male cohort (92%). Postoperative hemorrhage occurred in four cases, primarily among oncological patients, but there was no correlation with anticoagulation therapy. A personalized predictive model for bleeding risk was developed based on patient-specific characteristics and observed outcomes. Additionally, performing the procedure on an outpatient basis decreased healthcare costs and wait times for patients with T1/T2 glottic carcinoma. Conclusions: The findings indicate that TOMES type III or higher cordectomies can be safely performed on an outpatient basis, through the use of a personalized predictive model for each case and with appropriate postoperative monitoring. This approach has the potential to lower healthcare costs and improve patient quality of life through individualized assessments and structured risk analysis. Full article

The interconnection between health and environment is increasingly recognised, as is the role of healthcare professionals in raising awareness among patients and healthcare policymakers. To explore the relevance of enhancing patients’ awareness of the links between health and environment in ambulatory care, we conducted a qualitative study on General Practitioner (GP) practices in Switzerland. Using a co-benefit approach, we designed materials on meat consumption and active mobility, which were displayed in the waiting rooms. We conducted observations in five practices and interviewed five patients and five GPs to examine patients’ understanding of the messages and assess the acceptability of discussing them during medical encounters. Patients and GPs were receptive to the co-benefit approach. However, barriers were identified in promoting co-benefits during clinical encounters, including time constraints and lack of knowledge. Patients showed reactance to the messages and questioned the reliability of messages related to climate change. GPs were reluctant to compromise relationships with patients and were ambivalent, viewing environmental discussions as political. Positive message display and community promotion were identified as facilitators. This study highlights the need to develop clear educational materials to support GPs, to adapt messages to patients’ backgrounds, and to address the dichotomy between GPs’ political and scientific perspectives. Background: Vision loss due to chronic eye diseases remains a significant public health challenge. Early detection through screening programs may reduce the burden of vision loss. This study aimed to assess the detection rate of eye diseases (glaucoma, AMD, and diabetic retinopathy), including those newly detected during opportunistic screening and ophthalmological consultations within the local government health policy program “Good Vision for Mazovians” in Mazovia, Poland. Material and methods: This study is a retrospective analysis of medical data from the registry of the Ophthalmology Department of the Międzylesie Specialist Hospital in Warsaw, which implemented the local government preventive program “Good Vision for Mazovians. Data from 1812 individuals (aged 18–92 years) participating in the “Good Vision for Mazovians” preventive program were analyzed. Results: Most participants were female (59.7%), aged over 60, and took medications regularly (62.7%). Excluding subjects with prior diagnosis of eye conditions, the detection rate was 38 suspected cases (3.8%) of glaucoma cases, 84 suspected cases of AMD (4.6%), and 21 suspected cases of diabetic retinopathy (1.2%). Most participants had not visited an ophthalmologist in the past two years (58.6%), reported low or average knowledge of eye health, had difficulty accessing ophthalmology services in their region (57%), and identified long waiting times for appointments as the main barrier to care (83.5%). Conclusions: Opportunistic screening for eye diseases in populations with limited access to eye care should be considered as a method for detecting common causes of irreversible visual impairment, particularly AMD. Older adults and individuals without higher education appear to face the greatest barriers to accessing ophthalmology services and may benefit the most from targeted opportunistic screening initiatives. Academic attention has long been accorded to Yan Zhitui and his Family Instructions for the Yan Clan; however, the Confucian philosophical dimensions of his thought remain underexplored. This article will analyze his concepts of Virtue and Happiness alongside his thoughts on the Mandate of Heaven to explore how he found his place in the chaotic landscape of the Northern and Southern Dynasties. Reacting to the contemporary trend of pursuing high-ranking posts and generous emoluments while disregarding morality, Yan Zhitui first defined the connotations of Virtue and Happiness. He then established a causal relationship between them through the correspondence between Name and Reality, an act which reestablished the central position of Virtue. To address the conflict between Virtue and Happiness, his response was to trace its root cause and divide the Mandate of Heaven into two dimensions: Virtue and Time. Transcendental assurance for the core status of Virtue and the unity of Virtue and Happiness is provided by the former dimension, while the latter’s uncontrollability, in comparison, offers an explanation for contradictory realities. Based on this understanding, Yan Zhitui’s guidance urged individuals to adhere to the Way and uphold virtue while observing the macro situation and micro signs, and to wait for the right moment for the unity of Virtue and Happiness to be realized. The flourishing of Buddhism during the Northern and Southern Dynasties was also a significant influence; therefore, Yan Zhitui’s thoughts on the Mandate of Heaven absorbed Buddhist karma theory. The severity of natural disasters is increasing every year, having an impact on many people’s lives. During the response phase of disasters, airports are important hubs where relief aid arrives while people need to be evacuated to safety. However, the airport often forms a bottleneck in these relief operations because of the sudden need for increased capacity. Limited research is carried out on the operational side of airport disaster management. Experts identify the main problems as first the asymmetry of information between the airport and the incoming flights, and second the lack of resources. The goal of this research is to gain understanding of the effects of incomplete knowledge of incoming flights with different resource allocation strategies on the performance of the cargo handling operations in an airport after a natural disaster event. An agent-based model is created, where realistic offloading strategies with different degrees of information uncertainty are implemented. Model calibration and verification are performed with experts in the field. The model performance is measured by the average turnaround time, which can be split into offloading time, boarding time and the cumulative waiting times. The results show that the effects of one unplanned aircraft are negligible. However, the waiting times and other inefficiencies rapidly increase with the more unplanned aircraft arriving. This study develops a multi-agent evolutionary game model that incorporates both retailers and heterogeneous logistics providers, extending beyond prior dyadic models that typically isolate either channel choice or logistics competition. By comparing scenarios with and without the BOPS channel, the framework captures the dynamic interactions between retailers and logistics providers. The results show that introducing In-Store Pickup significantly increases market demand and retailer revenue by reducing consumer waiting time, but it also produces a revenue crowding effect for slow logistics providers. For fast providers, the impact depends on their ability to adjust service quality: lowering service levels helps retain market share, while efficiency improvement enhances profitability. Furthermore, consumer product valuation plays a critical role in driving retailers toward dual-provider or hybrid strategies. The methodological innovation lies in integrating heterogeneous logistics service differentiation with channel strategy selection into a unified evolutionary game framework. The study contributes by proposing a dynamic “efficiency threshold–channel selection” mechanism, offering both theoretical advancement in omnichannel retailing research and managerial insights for retailers and logistics providers seeking to optimize logistics capabilities and channel collaboration. This study investigates sentiment analysis of X data about the National Health Service (NHS) during a politically charged period, using lexicon-based, machine learning, and deep learning approaches, as well as topic modelling and aspect-based sentiment analysis (ABSA). This study is distinct in its comparative evaluation of sentiment analysis techniques on NHS-related tweets during a politically sensitive period, offering insights into public opinion shaped by political discourse. A dataset of 35,000 tweets collected and analysed using various techniques, including VADER, TextBlob, Naive Bayes, Support Vector Machines, Logistic Regression, Ensemble Learning, and BERT. Unlike previous studies that focus on structured feedback or general sentiment, this research uniquely explores unstructured public discourse during an election period, capturing real-time political sentiment towards NHS policies. The sentiment distribution from lexicon-based methods depicted that the presence of stop words could affect model performance. While all models achieved high accuracy on the validation dataset, challenges such as class imbalance and limited labelled data impacted performance, with signs of overfitting observed. Topic modelling identified nine topic clusters, with “waiting list,” “service,” and “immigration” carrying negative sentiments. At the same time, words like “thank,” “support,” “care,” and “team” had the most positive sentiments, reflecting public delight in these areas. ABSA identified positive sentiments towards aspects like “useful service”. This study contributes a comparative framework for evaluating sentiment analysis techniques in politically contextualised healthcare discourse, offering insights for policymakers and researchers. The study underscores the importance of data quality in sentiment analysis. Future research should consider incorporating multilingual datasets, extending data collection periods, optimising deep learning models, and employing hybrid approaches to enhance performance.
