The Authors presented their case report of the first patient affected from Streptococcus intermedius brain abscess, lung abscess and aortic valve endocarditis with literature review of lung abscesses and/or pleural infections due to S. intermedius.

The paper is well written and can be clearly read throughout the text. Clinical presentation is linear.

I have the following questions and critiques, which I hope may be used to further strengthen the article:

- Every case report should follow the CARE guidelines and the CARE checklist should be added as supplementary.

- I suggest adding just few information regarding the aortic valve nodule. Which was the dimension? Did it have echocardiographic features suggesting risk of embolization? Were there signs of left ventricular dilatation? These information may be clinically relevant.

- The table reports "gender" for what I understand being patients' sex. Gender and sex are not the same and should not be used interchangeably. I suggest using the word "sex".

- The table legend should add in the footnote the meaning of "mo", which I guess means "months".

I read with interest the paper Streptococcus intermedius brain abscess with lung abscess and aortic valve endocarditis.

I have two major comments:

The patient’s history needs to be more complete, and physical examination needs to be described in more detail, including the respiratory, cardiovascular, and abdominal systems.

The main interest of the report is the aortic valve endocarditis , and the introduction and literature review should focus on endocarditis and brain abscess, rather than on the lungs, which are a more frequente manifestation of S.intermedius infection.

Minor suggestions are on the file which I have attached.
