We have received the reviewers’ comment regarding the need to improve the language in this manuscript.  Although we were surprised by these comments, as the paper was written by native English-speaking medical writers, we understand that some sections could have been written clearer. To this end we have invited an independent, native English medical writer to fully edit and revise the manuscript once again.
