The authors reviewed the differences between CD-associated and cryptoglandular perianal fistulas in epidemiology, pathogenesis, and clinical management. They showed a flow chart to guide physicians when dealing with patients displaying their first episode of perianal fistulas.

The goal of the article is to summarize the current knowledge on the epidemiology, pathogenesis, and clinical management of perianal fistula, besides to provide a flow chart to give guidance to physicians when dealing with patients displaying their first episode of perianal fistulas.

The authors included an Introduction section, classification of perianal fistulas, epidemiology, pathogenesis, and treatment. The authors also included 2 explanatory figures and 1 flow chart.

I have only one observation to the authors. The article would benefit from the inclusion of an explanatory legend for Figure 2.
