Physico-Mechanical Properties of 3D-Printed Filament Materials for Mouthguard Manufacturing

Mouthguards are recommended for all sports that may cause injuries to the head and oral cavity. Custom mouthguards, made conventionally in the thermoforming process from ethylene vinyl acetate (EVA), face challenges with thinning at the incisor area during the process. In contrast, additive manufacturing (AM) processes enable the precise reproduction of the dimensions specified in a computer-aided design (CAD) model. The potential use of filament extrusion materials in the fabrication of custom mouthguards has not yet been explored in comparative studies. Our research aimed to compare five commercially available filaments for the material extrusion (MEX) also known as fused deposition modelling (FDM) of custom mouthguards using a desktop 3D printer. Samples made using Copper 3D PLActive, Spectrum Medical ABS, Braskem Bio EVA, DSM Arnitel ID 2045, and NinjaFlex were compared to EVA Erkoflex, which served as a control sample. The samples underwent tests for ultimate tensile strength (UTS), split Hopkinson pressure bar (SHPB) performance, drop-ball impact, abrasion resistance, absorption, and solubility. The results showed that Copper 3D PLActive and Spectrum Medical ABS had the highest tensile strength. DSM Arnitel ID 2045 had the highest dynamic property performance, measured with the SHPB and drop-ball tests. On the other hand, NinjaFlex exhibited the lowest abrasion resistance and the highest absorption and solubility. DSM Arnitel ID 2045’s absorption and solubility levels were comparable to those of EVA, but had significantly lower abrasion resistance. Ultimately, DSM Arnitel ID 2045 is recommended as the best filament for 3D-printing mouthguards. The properties of this biocompatible material ensure high-impact energy absorption while maintaining low fluid sorption and solubility, supporting its safe intra-oral application for mouthguard fabrication. However, its low abrasion resistance indicated that mouthguards made from this material may need to be replaced more frequently. Full article

Sports activities can increase participants’ risk of orofacial trauma. Therefore, it is crucial to have a comprehensive understanding of various types of sports mouthguards. This study aims to present a narrative literature review on ergonomic mouthguards, including their indications for use, classifications, materials, manufacturing methods, and the current status of additive manufacturing in their production. Research of the existing literature was performed in the Google Scholar, MEDLINE/PubMed, Web of Science, and ScienceDirect databases to identify the currently available publications on the topic of sports mouthguards from January 1951 to August 2023. The terms used were “sports mouthguard”, “mouthguards and orofacial traumas”, and “mouthguards and additive manufacturing”. A total of 920 articles were found, and 39 articles were selected and included in this review. While consensus exists regarding the significance of using sports mouthguards with optimal attributes, there is a need for standardization in the methodology for manufacturing custom-made mouthguards. These can be fashioned with conventional ethylene vinyl acetate (EVA) copolymer reinforcements. Such standardization would ensure uniform stress distribution and guarantee ample protection for oral tissues, allowing for universal reproducibility among dental practitioners. Additive manufacturing stands as an innovative method for fabricating mouthguards, displaying promising benefits. However, the materials and methodologies employed in this workflow still require refinement and characterization for a safe clinical integration. Full article

The aim of this study was to clarify the effect on the postural control function of equalizing the occlusal contact state by wearing a mouthguard. Method: Participants included 23 untrained adults, 11 gymnasts, and 10 weightlifters. Custom mouthguards were fabricated and adjusted so that all teeth were in even contact with light clenching. The standing postural control function was evaluated by the displacement of the center of foot pressure (COP) measured using a gravity center fluctuation meter. Measurement conditions were with and without a mouthguard. Statistical analyses were performed on the differences in COP displacement among participant groups and between mouthguard conditions and the difference in the reduction rate of COP displacement when wearing a mouthguard among participant groups. Results: COP significantly decreased in the order of untrained adults to weightlifters to gymnasts, irrespective of the mouthguard condition. The reduction rate in COP displacement after wearing a mouthguard was greatest in gymnasts. Conclusions: The effect of the mouthguard on postural control function was more pronounced in gymnasts than in untrained adults and weightlifters, and this study suggests that equalization of the occlusal state by wearing a mouthguard contributes to improving postural control function. Soccer players purposefully head the ball, raising concerns about reduced tolerance to concussion and potential long-term brain health. By combining qualitative video analysis with custom-fit instrumented mouthguards (iMGs), we aimed to categorize header kinematics (peak linear acceleration (PLA) and peak angular acceleration (PAA)) by header type and ball delivery method. iMGs were fitted to 10 male collegiate players for twelve matches. A total of 133 headers were verified and contextualized via video review. The most common header type (38.7%), as well as the preceding ball delivery method (47.4%), was found to be a pass. Approximately one-quarter of header impacts (27.0%) occurred below 10 g. For header type, there were no significant differences in kinematics, with shot attempts having the highest median PLA and PAA. For ball delivery methods, goal kicks had significantly greater PAA than long balls and pass attempts. The current study highlights the utility of qualitative video analysis in combination with real-time head kinematic data from iMGs to understand the mechanism and severity of header impacts. The pilot findings indicate that high-speed ball delivery methods result in higher head kinematics and should be a focus of future mitigation strategies. Full article

Custom mouthguards are used in various sports disciplines as a protection for teeth, temporomandibular joints, and soft tissues of the oral cavity from impact forces. The purpose of this research was to evaluate the mechanical properties of flexible polymeric 3D-printable materials and to select a material with the most favourable physical properties for making intraoral protectors. Four 3D-printable polymeric materials were selected for the evaluation: IMPRIMO LC IBT (Scheu-Dental, Iserlohn, Germany), Keyortho IBT (EnvisionTEC, Gladbeck, Germany), IBT (Formlabs, Somerville, MA, USA), and Ortho IBT (NextDent, Utrecht, Netherlands). A total of 176 samples (44 from each material) was 3D-printed using the stereolitography (SLA) technique. Tensile strength, flexural strength, notch-toughness, Shore hardness, sorption, and solubility tests were conducted. The materials were compared using a series of analyses of variance (one-way ANOVA) with Bonferroni post hoc tests. Statistical analyses were performed with the use of IBM SPSS Statistics 28.0.0 software (IBM, New York, NY, USA). Each material was assigned a score from 1 to 4 depending on the individual test results, and tests were given indexes according to the significance of the parameter in the mouthguard protective function. The number of points obtained by each material in each test was then multiplied by the test index, and the results were tabulated. The material with the highest result among the ones studied—most suitable for the application in mouthguard fabrication—was Keyortho IBT from EnvisionTEC. Full article

Ethylene vinyl acetate mouthguards are the most often used custom protective intraoral appliances by combat sports practitioners. However, due to the difficulties in maintaining the hygiene of such mouthguards and thinning during fabrication, resulting in difficulty in predicting final dimensions, they may not be the optimal solution. The aim of this research was to evaluate an innovative method of mouthguard formation using intraoral modeling of the mouthguard pattern, hybrid acrylic material, and the addition of ZnO nanoparticles. Seventeen mouthguards patterns and 34 custom mouthguards were evaluated: 17 hybrid and 17 nanohybrid. A total of 1122 measurements were performed: each mouthguard and pattern was measured at 22 points. Statistical analyses were performed with the use of IBM® SPSS® Statistics 27.0.0 software (IBM, Armonk, NY USA). The mean thickness of the patterns and mouthguards at all labial areas of central incisors were between 4.65 and 4.80 mm. The thickness at the buccal surface of the first molar was between 3.71 and 4 mm, and at the occlusal surface between 3.40 and 3.56 mm in the cusp area. All measurements of hybrid and nanohybrid mouthguards were strongly and highly correlated with the measurements of the mouthguard patterns. Hybrid and nanohybrid mouthguards are an advantageous alternative to thermoformed custom appliances. Full article

Instrumented mouthguards (iMG) were used to collect head acceleration events (HAE) in men’s professional rugby league matches. Peak linear acceleration (PLA), peak angular acceleration (PAA) and peak change in angular velocity (ΔPAV) were collected using custom-fit iMG set with a 5 g single iMG-axis recording threshold. iMG were fitted to ten male Super League players for thirty-one player matches. Video analysis was conducted on HAE to identify the contact event; impacted player; tackle stage and head loading type. A total of 1622 video-verified HAE were recorded. Approximately three-quarters of HAE (75.7%) occurred below 10 g. Most (98.2%) HAE occurred during tackles (59.3% to tackler; 40.7% to ball carrier) and the initial collision stage of the tackle (43.9%). The initial collision stage resulted in significantly greater PAA and ΔPAV than secondary contact and play the ball tackle stages (p < 0.001). Indirect HAE accounted for 29.8% of HAE and resulted in significantly greater ΔPAV (p < 0.001) than direct HAE, but significantly lower PLA (p < 0.001). Almost all HAE were sustained in the tackle, with the majority occurring during the initial collision stage, making it an area of focus for the development of player protection strategies for both ball carriers and tacklers. League-wide and community-level implementation of iMG could enable a greater understanding of head acceleration exposure between playing positions, cohorts, and levels of play. Full article

Background: Orofacial injuries are common occurrences during contact sports activities. However, there is an absence of data regarding the performance of hybrid occlusal splint mouthguards (HMG), especially during compressive loading. This study amid to evaluate the biomechanical effects of wearing a conventional custom mouthguard (MG) or the HMG on the teeth, bone, and the device itself. Methods: To evaluate the total deformation and stress concentration, a skull model was selected and duplicated to receive two different designs of mouthguard device: one model received a MG with 4-mm thickness and the other received a novel HMG with the same thickness. Both models were subdivided into finite elements. The frictionless contacts were used, and a nonlinear analysis was performed simulating the compressive loading in occlusion. Results: The results were presented in von-Mises stress maps (MPa) and total deformation (mm). A higher stress concentration in teeth was observed for the model with the conventional MG, while the HMG design displayed a promising mechanical response with lower stress magnitude. The HMG design displayed a higher magnitude of stress on its occlusal portion (7.05 MPa) than the MG design (6.19 MPa). Conclusion: The hybrid mouthguard (HMG) reduced (1) jaw displacement during chewing and (2) the generated stresses in maxillary and mandibular teeth. Full article

Experimental research studies have shown that wearing a mouthguard (MG) is an effective way to prevent tooth or maxillofacial trauma. However, there is a lack of scientific information regarding how the material arrangement within the mouthguard can modify its mechanical response during an impact. Hence, this study aimed to evaluate the influence of material arrangement within custom-made mouthguards on stress transmitted to anterior teeth, bone, and soft tissue after impact. Four 3D finite element models of a human maxilla were reconstructed based on the CBCT of a young patient and analyzed according to the presence or absence of a mouthguard and the type of material arrangement within those with a mouthguard: model NMG with no mouthguard; model CMG representing the conventional arrangement with a single 4 mm-thick ethylene-vinyl acetate (EVA) foil; model FMG presenting layer arrangement with two 1 mm-thick foils of EVA in the outer shell and one 2 mm-thick foil of EVA foam in the core; model HMG presenting a 1 mm-thick compact inner and outer shell of EVA and a 2 mm wide air-filled zone in the core. Linear quasi-static analysis and frontal load were used to simulate an impact with an energy of 4.4 J. Isotropic linear elastic properties were assumed for the bone and teeth but not for the mouthguard protection and oral soft tissues. The results were evaluated and compared in terms of displacement, stretches, and stresses. All the mouthguards analyzed reduced the risk of injury to teeth and bone, reducing the displacement and stress of these structures. However, the implementation of a honeycomb structured layer allowed more significant displacement and deformation of the mouthguard’s external layer, thus promoting higher protection of the anatomic structures, namely the root dentin and the bone tissue. Nevertheless, the results also indicate that improving the mouthguard flexibility might increase the soft tissue injuries. Full article

The purpose of the present systematic review was to determine the acute effects of wearing bite-aligning mouthguards on muscle strength, power, agility and quickness in athletes. A search of the current literature was performed using the electronic databases (until 1 May 2021) Web of Science, Scopus and Medline. The inclusion criteria were: (1) descriptive design studies; (2) with randomized clinical trials; (3) examining the within-subject acute effects of wearing mouthguards on functional and neuromuscular performance parameters; (4) in physical active, recreational or high-standard athletes. Twenty-seven studies met the inclusion criteria. Sixteen reported positive effects in some of the variables assessed, two reported negative effects and the rest found no significant differences. Overall, the main findings described in the literature are inconclusive concerning the neuromuscular advantages of using mouthguards in muscle strength, power, agility and quickness. These discrepancies might be related to several factors such as differences in testing protocols, poor control of the jaw magnitude and improper mouthguard designs. Despite these differences, after conducting the present systematic review, the authors speculate that jaw clenching while wearing custom-made, bite-aligning oral devices might promote beneficial effects in lower limb power actions, especially in jump ability and knee extension movements. Thus, athletes might consider the use of mouthguards, not only for their protective role but also for the potential ergogenic effects in specific actions, mainly those for which lower limb muscular power are required. Athletes’ oral health appears to be poor in numerous sport activities and different diseases can limit athletic skills, both during training and during competitions. Sport activities can be considered a risk factor, among athletes from different sports, for the onset of oral diseases, such as caries with an incidence between 15% and 70%, dental trauma 14–70%, dental erosion 36%, pericoronitis 5–39% and periodontal disease up to 15%. The numerous diseases are related to the variations that involve the ecological factors of the oral cavity such as salivary pH, flow rate, buffering capability, total bacterial count, cariogenic bacterial load and values of secretory Immunoglobulin A. The decrease in the production of S-IgA and the association with an important intraoral growth of pathogenic bacteria leads us to consider the training an “open window” for exposure to oral cavity diseases. Sports dentistry focuses attention on the prevention and treatment of oral pathologies and injuries. Oral health promotion strategies are needed in the sports environment. To prevent the onset of oral diseases, the sports dentist can recommend the use of a custom-made mouthguard, an oral device with a triple function that improves the health and performance of athletes. During training, the sports dentist must monitor the athletes and the sports examination protocol must be implemented with the inclusion of the clinical examination, quantitative and qualitative analysis of saliva and instructions on the use, cleansing and storage of the mouthguard. Background/Aims: There is a lack of data regarding the influence of different laminates for mouthguard reinforcement in the mechanical response during an impact in the orofacial region. The aim of this study was to verify the influence of the laminate framework on the stresses and strains of the anterior teeth and displacement of ethylene-vinyl acetate (EVA) custom-made mouthguards during a simulated impact. The null hypotheses was that the different laminates reinforcement would present the similar effect in maxillary structures, regardless the elastic modulus. Methods: A finite element model of human maxillary central incisors with an antagonist contact was used. A linear quasistatic analysis was used to simulate the force exerted during an impact. A total of 5 different layers were simulated inside the mouthguard at the labial portion according to the Elastic Modulus 1 MPa (Extremely flexible), 9 GPa (Low modulus reinforcement), 18 GPa (Without reinforcement), 50 GPa (Flexible alloy), 100 GPa (Titanium alloy) and 200 GPa (Hard material). The results were evaluated by means of Maximum Principal Stress (in the tooth and bone), Microstrain (periodontal ligament) and Displacement (mouthguard) criteria. Results: The elastic modulus of the material inside the MG influenced the stress distribution on the enamel buccal face. However, it did not affect the bone tissue stress, periodontal ligament strain or root dentin tissue stress. Conclusion: The use of reinforcement inside the custom-made mouthguard can modify the stress generated in the enamel buccal surface without improvement to the root dentin, periodontal ligament or bone tissue. The present study examined the influence of a customized mouthguard on body alignment and balance performance in professional basketball players. Twenty-three professional male basketball players, aged 25.8 ± 8.6 years old, were voluntarily assigned to participate in three treatments, including no treatment (no mouthguard), acute treatment (wearing a mouthguard), and repeated treatments (8 weeks follow-up). Body alignment status, such as spinal and pelvic posture and balance performance, were measured at each time point using a 3D Formetric III (Germany) and a postural control device (Posturomed 202, Germany), respectively. A repeated MANOVA analysis with a Bonferroni post hoc test was applied, and the adjusted p-value was set at 0.02. No significant treatment effect was observed in body alignment (p = 0.302). However, univariate analysis showed a significant difference in pelvic torsion, where it was decreased after acute and repeated mouthguard treatments compared to no treatment (p < 0.001). Kyphotic angle also increased significantly following 8 weeks of treatment compared to no treatment (p < 0.001) and acute treatment (p < 0.002). There was a significant treatment effect on balance performance (p < 0.001). Both static and dynamic balance performance improved following 8 weeks of treatment (p < 0.001). Our study revealed that a customized mouthguard provides a benefit to balance performance. Notably, repeated treatment impacts on balance performance more than acute treatment. Although our findings did not show a significant effect on body alignment, some positive results, such as pelvic torsion and kyphotic angle, may provide substantial information for developing future longitudinal studies with large sample sizes. The aim of this study is to consider the hypothesis that wearing mouthguards (MGs) from a young age helps to shape players’ habit of continuing to wear MGs as adults. In addition, we consider the mandatory rule of wearing mouthguards in the youth category on the future wearing rates of mouthguards. Eighty-five Japanese male amateur rugby players were included in this study. We examined the relationship between the wearing rates of mouthguards when participants were high school students and as adults (in 2018) using a questionnaire. Collected data were classified into categories (age, mouthguard type, and applied mandatory rule or not), and their relationships were analyzed. The wearing rate of mouthguards in high school was significantly related to the wearing rate of mouthguards in adulthood (χ2 = 12.1, p < 0.01). The mandatory rule to wear mouthguards at high school was related to the wearing rates of mouthguards as adults, but not significantly (χ2 = 3.42, p = 0.06). Participants’ mouthguard wearing rates as adults were affected by their high school wearing rates. The mandatory rule of wearing mouthguards in the youth category may be somewhat effective for improving mouthguard wearing rates; however, it is more important to provide knowledge about their importance. This in-vitro study compares the shock absorption qualities of five mouthguard designs measured with a triangulation laser sensor during small hard object collisions. The aim was to investigate the impact of different labial designs on mouthguard performance. Methods: Five different custom-fabricated ethylene vinyl acetate (EVA) types of mouthguards with varying thickness and different labial inserts (polyethylene terephthalate glycol-modified (PETG), nylon mesh, air space) were tested with a triangulation laser sensor during different energy blows, generated with a pendulum testing device. The pendulum hits were applied to the center of a pivoted tooth crown in a custom-built upper jaw model. Measurements were executed with the mouthguards on the model and with no mouthguard as a negative control. Results: Tooth deflection was reduced with all mouthguards in comparison to no mouthguard. Increasing mouthguard thickness improved the mouthguards’ shock absorption capacities. Also, adding labial inserts increased their preventive qualities in ascending order: Mouthguard with a soft insert (nylon mesh), a hard insert (PETG), air space plus a hard insert (PETG). Conclusion: Increasing EVA foil thickness of a mouthguard, increasing labial thickness, and adding labial inserts (soft, stiff and air space) improve mouthguard shock absorption capabilities during small hard object collisions, thereby improving dental trauma prevention.
