Epidural Oxycodone for Acute Pain
Round 1
Reviewer 1 Report
I have read this review article thoroughly with great interest.
The study group has been working on the preclinical and clinical studies of epidural oxycodone for long time and has published several papers.
In this manuscript, the authors have done comprehensive review of available evidences about epidural oxycodone and provided concise summary.
In general, the manuscript is well written and organized thus can provide readers a quick reference and facilitate further clinical use of epidural oxycodone.
I have only some minor inquiries.
In abstract, the authors mentioned that the only 2 opioids are approved for "intrathecal" use which are morphine and sufentanyl.
But in the introduction, second paragraph, the authors stated that "The two opioids approved for epidural administration are morphine and sufentanil, but off-label use of neuraxial fentanyl is also common"
The major focus of this review article is epidural oxycodone, so please correct it.
Also, the reference 15 and 16 are irrelevant to the statement.
Please cite proper articles to document what opioids are "approved" for epidural use!

Very nicely written review about the emphasis of oxycodone as an epidural drug. It definitely emphasizes the preference of oxycodone over morphine as we know oxycodone's pharmacology and pharmacokinetics is different to morphine.
I would suggest to include a figure about the glymphatic pathway. Its a very important pathway but it hasnt been given much attention and that particular section looks like it would attract more interest if a figure could be presented as well.
