This is an interesting study with conclusions conistent with the epidemiologial data. However, ALA-PDT should be also be included in the analysis or otherwise it should be explained why it has been not evaluated.

Thank you for your comment! ALA-PDT was not included, as the aim was to compare topical treatment modalities that do not require special equipment as lamps. Therefore conventional photodynamic therapy is mentioned as an exclusion criteria. Articles on ALA daylight PDT were not identified by search phrase, therefore also not included.

I think that this article does add to the current literature. Clinical decisions are based on the reactions and need to be tailored to the patients.  Little information is readily available comparing regional and local lesional treatment.  These non surgical options are important alternatives to surgery in clinical practice but there is no standard that allows translation of these LSRs to be compared.


There needs to be a number of clarifications made to some of the statements and interpretation of the literature.  Please see my markups of the document for your review and comment
