This is an interesting case report.

  1. Details of spinal anesthesia and scheme of pain during MRI examination should be provided.
  2. Findings during operation were not enough. What is the reasons for "failed".

Herein the authors report on the case of a vertebral fracture occurred in a patient with ankylosing spondylitis  by position changes  for MRI examination under spinal anesthesia for pain control. Aim of this work is to stress the role of careful positioning in preventing iatrogenic fractures in such clinical setting.

Pertinent literature has been discussed.

Authors should explain practically how this occurrence can practically be prevented. Moreover the role of spinal anesthesia in the complication should be explained more in detail.
