You have touched an important topic and applying as well qualitative methods. Their interpretation I found of great value, though I would advice to discuss about different reasons for not screening for HT in different age groups, which is very evident. More practical suggestions could be given. 

Additionally, I would like to clarify what BP measure was taken as it was written: from 2 measures, but averaged using the arm with higher BP? 

As well I would recommend to change the article name as starting with "Proportion of primary care... " doesn't appealing for the reader
