Cover Story: Roux-en-Y hepaticojejunostomy is the most commonly used procedure for the establishment of biliary–enteric continuity after any type of resection of the extrahepatic biliary tree. Although the endoscopic approach is preferred for accessing the biliary tree, it is not always feasible, especially in postoperative patients with altered anatomy. The surgical revision of the anastomosis is a highly demanding operation of increased associated morbidity. In this context, an approach of using surgery to facilitate the endoscopic assessment prior to proceeding to the surgical revision of the anastomosis appears justified. In the present report, we present the development of a surgical access loop, avoiding the unnecessary revision of an otherwise flawless hepaticojejunostomy. View this paper
