Prostate Cancer: Molecular Biology, Prognostication and Treatment Approaches

Dear Colleagues,

Today, prostate cancer is the second most common cancer in the world, with almost 400,000 deaths annually. Recently, screening for prostate cancer using serum prostate-specific antigen has come under considerable criticism due to several trials demonstrating that only screening for PSA serum levels often leads to overdiagnosis and overtreatment, as well as the inability to accurately differentiate between low-, intermediate- and high-risk aggressive disease. Prostate cancer diagnoses are currently based on the gold-standard invasive procedure of conducting a transrectal ultrasound-guided needle biopsy of the prostate. However, in the last ten years, advances in molecular and functional assays, including next-generation sequencing and microarrays, have enabled the creation of profiles of prostate cancer samples from diverse ethnic groups. Through comparative genome-wide analyses of genomic, epigenomic and transcriptomic data from these samples, potential mutations, copy number alterations, DNA methylation changes and gene expression patterns specific to certain ethnicities have been identified. A common feature of prostate cancer is its high degree of heterogeneity, with clinical, morphological and molecular variations ranging from slow progression to highly aggressive and ultimately fatal forms. Genetic mutations, particularly in DNA repair genes such as ATM, BRCA1 and BRCA2, influence prostate cancer. This highlights the importance of family history and genetic testing in assessing risk. Moreover, recent advancements in genetics and molecular studies have identified key genes involved in prostate cancer progression, such as TP53, PTEN and the androgen receptor, revealing new targets for therapy by identifying critical pathways in tumor growth and metastasis. With a deeper understanding of prostate cancer oncogenesis and the personalization of diagnosis and treatment, emerging novel therapeutic approaches hold promise for both primary and metastatic prostate cancer. These advancements have brought about new opportunities for more effective treatment options.

Therefore, this Special Issue will contain a collection of manuscripts focused on prostate cancer research, encompassing investigations into its development, progression, diagnosis, risk assessment, prognosis and treatment. These manuscripts will place particular emphasis on the underlying molecular features of the disease, with the aim of quickly developing more effective therapeutic approaches. This Special Issue welcomes the latest research articles and reviews that clarify concepts, interpret experimental data, suggest specific experiments and organize the vast body of knowledge on prostate cancer.
