Infrared Thermography and Deep Learning Prototype for Early Arthritis and Arthrosis Diagnosis: Design, Clinical Validation, and Comparative Analysis

Arthritis and arthrosis are prevalent joint diseases that cause pain and disability, and their early diagnosis is crucial for preventing irreversible damage. Conventional diagnostic methods such as X-ray, ultrasound, and MRI have limitations in early detection, prompting interest in alternative techniques. This work presents the design and clinical evaluation of a prototype device for non-invasive early diagnosis of arthritis (inflammatory joint disease) and arthrosis (osteoarthritis) using infrared thermography and deep neural networks. The portable prototype integrates a Raspberry Pi 4 microcomputer, an infrared thermal camera, and a touchscreen interface, all housed in a 3D-printed PLA enclosure. A custom Flask-based application enables two operational modes: (1) thermal image acquisition for training data collection, and (2) automated diagnosis using a pre-trained ResNet50 deep learning model. A clinical study was conducted at a university clinic in a temperature-controlled environment with 100 subjects (70% with arthritic conditions and 30% healthy). Thermal images of both hands (four images per hand) were captured for each participant, and all patients provided informed consent. The ResNet50 model was trained to classify three classes (healthy, arthritis, and arthrosis) from these images. Results show that the system can effectively distinguish healthy individuals from those with joint pathologies, achieving an overall test accuracy of approximately 64%. The model identified healthy hands with high confidence (100% sensitivity for the healthy class), but it struggled to differentiate between arthritis and arthrosis, often misclassifying one as the other. The prototype’s multiclass ROC (Receiver Operating Characteristic) analysis further showed excellent discrimination between healthy vs. diseased groups (AUC, Area Under the Curve ~1.00), but lower performance between arthrosis and arthritis classes (AUC ~0.60–0.68). Despite these challenges, the device demonstrates the feasibility of AI-assisted thermographic screening: it is completely non-invasive, radiation-free, and low-cost, providing results in real-time. In the discussion, we compare this thermography-based approach with conventional diagnostic modalities and highlight its advantages, such as early detection of physiological changes, portability, and patient comfort. While not intended to replace established methods, this technology can serve as an early warning and triage tool in clinical settings. In conclusion, the proposed prototype represents an innovative application of infrared thermography and deep learning for joint disease screening. With further improvements in classification accuracy and broader validation, such systems could significantly augment current clinical practice by enabling rapid and non-invasive early diagnosis of arthritis and arthrosis. Background: Acromioclavicular joint (ACJ) injuries frequently result from trauma to the shoulder girdle and are particularly common among young, physically active individuals. These injuries account for approximately 9% of all traumatic shoulder girdle injuries and often lead to functional impairment and pain. The TightRope® system, LARS™ band, and Bosworth screw are among over 160 currently described surgical techniques for managing ACJ dislocations. However, there is no consensus regarding the optimal surgical approach, particularly for the management of moderate Rockwood Type III ACJ dislocations. Materials and Methods: In this retrospective study, data from 246 patients who underwent surgery for ACJ dislocation between 2010 and 2018 at the Department of Orthopedics and Trauma Surgery, Medical University of Vienna, were analyzed. Patients were divided into four cohorts based on the surgical technique used: Bosworth screw, LARS (acute), LARS (chronic), and TightRope. Clinical and radiological outcomes were assessed pre- and postoperatively using the Visual Analog Scale (VAS), Constant, Disability of the Arm, Shoulder and Hand Score (DASH), Simple Shoulder Test (SST), University of California—Los Angeles Shoulder Score (UCLA), Short Form Health Survey (SF-36), and American Shoulder and Elbow Surgeons score (ASES), as well as radiographic analysis. Radiological measurements of the acromioclavicular (AC) and coracoclavicular (CC) joint spaces were taken on both the injured and uninjured shoulders to analyze and compare the reduction in joint gaps. Results: All surgical methods resulted in significant reductions in AC and CC joint gaps. The TightRope and LARS acute groups showed the greatest reductions, with minimal complication rates. Complication analysis revealed significant differences in clavicular elevation (p < 0.001) and CC-ligament ossification (p = 0.006), which were most frequent in the Bosworth group and least common in TightRope® patients, with LARS showing intermediate values. AC joint arthrosis was uncommon in all four groups and did not differ significantly (p = 0.13). Overall, TightRope® was associated with the most favorable complication profile. The postoperative VAS score in the TightRope group was 1.52 ± 2.06, and the Constant score was 96.83 ± 5.41, reflecting high patient satisfaction. Conclusions: All systems led to satisfactory radiological and clinical outcomes, with the LARS™ band showing particular effectiveness in chronic ACJ dislocations. While all techniques provided good results, the TightRope® system demonstrated the most favorable overall profile in our cohort and may therefore be considered a promising contemporary option. Further studies are needed to determine the optimal treatment for moderate ACJ dislocations and to assess the cost-effectiveness of these surgical techniques. Full article

Background/Objectives: Avascular necrosis of the femoral head is a debilitating condition that, if left untreated, leads to progressive arthritis necessitating total hip replacement (THR). In the younger adult population, there is a drive towards joint-preserving procedures, particularly where alternative techniques such as core decompression or vascularised bone grafting are anticipated to fail. Proximal femoral osteotomy is a technique that aims to remove the necrotic segment from the weight bearing area. The presented review aims to examine the efficacy of this technique in the management of avascular necrosis of the femoral head, reporting both rates of conversion to total hip replacement and patient reported outcomes. Methods: This systematic review was conducted according to PRISMA guidelines. A search was conducted of PubMed, Ovid Medline, EMBASE, and the Cochrane Library using pre-defined search terms. Data were extracted, and descriptive data presented. Quality of each study was assessed using the NIH quality assessment tool for case series studies. Results: Fifty-three studies with data for 2686 osteotomies are presented. Progression of radiological arthrosis was present in 40% of cases, with 20.3% of patients having undergone conversion to THR at a mean of 75.4 months (range 20–132 months). Patient-reported outcome measures were recorded in 1416 patients, of which the Harris Hip Score was the most commonly utilised. This score improved from a mean of 58.3 to 84.4 at a mean follow-up of 102 months. Conclusions: Osteotomy represented a valid head-preserving technique in the armamentarium against avascular necrosis of the femoral head, with conversion to THR required in 20.3% of patients at 7 years. In those patients who did not require THR, PROMS were similar to the arthroplasty population. Full article

Background: Total hip arthroplasty (THA) is a well-established surgical procedure for end-stage hip arthrosis. Innovations such as minimally invasive approaches and new technologies have improved outcomes and reduced invasiveness. The introduction of short-stem prostheses, which offer potential benefits in bone preservation, has been a significant development in recent years. This prospective case series study aims to compare invasiveness of the short-stem (SS) and conventional-stem (CS) prostheses in THA with a posterolateral approach (PLA) by assessing perioperative serum markers. Methods: A prospective case series was conducted involving consecutive patients who underwent primary THA from January 2022 to December 2023. Demographics and preoperative, postoperative day 1 (POD1), and postoperative day 2 (POD2) serum levels of C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), procalcitonin (PCT), and white blood cells (WBCs) were measured. Results: The study included 21 patients with CS and 19 with SS, with no significant differences between groups in demographic. No statistically significant differences were found in serum markers between SS and CS groups at any time point. Both groups showed significant increases in ESR, CRP, and PCT from preoperative levels to POD2 (p < 0.001), while WBC values increased from preoperative to POD1 but decreased between POD1 and POD2. Conclusion: The short-stem prosthesis does not exhibit significantly different perioperative serum marker profiles compared to the conventional stem, suggesting similar levels of surgical invasiveness between the two implants. Further studies with larger sample sizes are needed to validate these findings and explore other aspects of short-stem THA. Full article

Some musculoskeletal disorders, including osteoarthritis; arthrosis; post-traumatic injuries; and other inflammatory tendon, joint and muscular afflictions, still represent unmet medical needs. Cetylated fatty acids (CFAs) are key components of widely distributed over-the-counter products, especially for topical use, which are intended to reduce symptoms associated with these conditions. Nevertheless, the mechanism of action of CFAs’ analgesic and anti-inflammatory properties has not yet been clearly established. Endocannabinoids, such as 2-arachidonoylglycerol (2-AG) and anandamide (AEA), are known to produce analgesic and anti-inflammatory effects. These compounds undergo physiological inactivation operated by several enzymes, including monoacylglycerol lipase (MAGL). We herein demonstrate for the first time that the therapeutic effects of CFAs may be attributable, at least in part, to their MAGL inhibition activities, which induce a local increase in analgesic/anti-inflammatory endocannabinoids in close proximity to the site of administration. These findings pave the way for the development of new potent local analgesic agents, whose action is based on an indirect cannabinoid effect. Full article

Background: Fractures of the distal radius are common, particularly among young men and elderly women, often leading to painful wrist arthritis, especially if the joint surface has been affected. Traditional treatments of the wrist, such as full or partial wrist fusion, limit movement, and common wrist prostheses have high complication rates. Regenerative medicine and 3D bioprinting offer the potential for personalized joint replacements. Methods: This study evaluates using the contralateral radius as a template for creating customized distal radius prostheses. Bilateral CT scans of healthy wrists were analyzed to assess the shape and symmetry of the distal radius using a landmark-free morphometric method. Instead of comparing defined landmarks, the entire surface of the radius is analyzed employing dense point- and deformation-based morphometry to detect subtle morphological differences, providing an unbiased and more accurate comparison of the overall deformations in the distal radii. Results: results show strong intraindividual symmetry in joint surfaces. Interindividual comparisons revealed significant morphological variations, particularly gender-specific differences. Conclusions: These findings support the use of the contralateral radius as a template for the replaced side. At the same time, the interindividual results endorse the approach of pursuing personalized prostheses as the optimal replacement for distal joint surfaces. The increasing improvement of 3D-printed prostheses promises new methods for better outcomes in distal radius arthrosis after intraarticular fractures. Further research into clinical applications and biocompatible 3D printing materials is recommended. Full article

Background: For many years, advancements in hand joint replacement (JR) were relatively minor compared to those for large joints. However, the caution previously exercised due to high complication rates is gradually being replaced by the expanding use of JR therapies for small joints in the hand. Despite this progress, there is a lack of comprehensive data on the outcomes of hand JR and on the optimal infrastructure required to meet the growing demand for these therapies. Methods: This study examined trends and revision rates of JR for thumb carpometacarpal (CMC-1) and finger (MCP and PIP) joints in both inpatient and outpatient settings in Germany. Data from the Federal Statistical Office of Germany (Destatis) and the Central Institute for Statutory Health Insurance Physicians (ZI) were analyzed, focusing on the incidence, demographics, and outcomes of these procedures. Results: This study found a substantial national increase in prosthetic treatments specifically for CMC-1, with a 2.18-fold rise in the outpatient sector compared to a 1.65-fold increase in inpatient treatments. Despite this shift, 83.7% of JR procedures were still performed in an inpatient setting. Conclusions: The overall complication rates appear to be declining, suggesting that while the management of these procedures is shifting towards outpatient care, the quality remains stable. Lower-limb alignment deformities constitute a significant clinical concern, as they can lead to serious complications, including progressive degenerative diseases and disabilities. Rotational deformities may give rise to conditions such as joint arthrosis, patellar instability, and the degeneration of the patellofemoral cartilage. Therefore, a comprehensive evaluation of lower-limb alignment is essential for the effective patient management, preoperative planning, and successful correction of these deformities. The primary assessment method employs full-length standing radiographs in the anteroposterior (AP) projection, which facilitates accurate measurements of the anatomical and mechanical axes of the lower limb, including angles and deviations. The outcomes of this analysis are vital for the meticulous planning of osteotomy and total knee arthroplasty (TKA). In addition, computed tomography (CT) provides a specialized approach for the precise evaluation of femoral and tibial rotation. Background/Objectives: A properly functioning musculoskeletal system is imperative for human well-being at every stage of life, including at an older age. This study’s aim was to assess the relationship between sociodemographic and physical functioning variables and the presence of individual musculoskeletal disorders (MSDs), MSD comorbidity, and multimorbidity, as well as to determine factors that are independent predictors of the presence of MSDs in people over 65 years old. Methods: This population-based study included 3701 participants aged 65 years and older. Data on individual MSDs addressed cervical and lumbosacral spine regions and degenerative joint disease (arthrosis). The subjects were categorized into four groups: those without any diseases; those with one MSD; those with two MSDs (comorbidities); and those with three MSDs (multimorbidities). The sociodemographic and physical functioning variables were analyzed. Results: Females were more likely to have MSDs (two: OR 1.95 and three: OR 2.25) than men. Elderly people aged 75 and above were 1.49 times more likely to have three MSDs. Elderly people with elementary school education were more likely to have MSDs (two: OR 1.34 and three: OR 2.06) than those with high school/university education. The low-income population was 2.47 times more likely to have three MSDs. Individuals with partial activity limitations because of health problems had greater chances of having one, two, or three MSDs (OR 1.60, 1.59, and 1.94, respectively), and elderly individuals with severe limitations had an OR of 1.43, 2.17, and 4.12, respectively. Individuals with some/many difficulties in walking up or down 12 steps were more likely to have MSDs (two: OR 2.26 and three: OR 2.28). Conclusions: The significant predictors of experiencing a single MSD, MSD comorbidity, or MSD multimorbidity include residing in the Serbian capital city and having limitations in activities due to health problems. A significant predictor of having a single MSD or MSD comorbidity is residing in the northern region of Serbia. A significant predictor of MSD comorbidity is residing in the southeastern region of Serbia. Significant predictors of MSD comorbidity or MSD multimorbidity include female gender, an elementary school educational level, and experiencing difficulty in walking up or down 12 steps. Significant predictors of MSD multimorbidity are being 75 years of age and above and having a lower income. Full article

Objective: This study aimed to evaluate the distribution and trends of masseter muscle tension in patients with temporomandibular joint (TMJ) pain, examining gender-specific differences and the impact of various TMJ disorders. Methods: From January 2020 to June 2024, a total of 734 patients presenting with facial pain radiating to the head and neck, localized around and extending from the TMJ, were referred for ultrasonographic examination. After applying exclusion criteria, 535 patients (72.9%) were included in the study. The patient cohort consisted of 343 females (64.1%) and 192 males (35.9%), with muscle tension measured using the Aixplorer ultrasound system equipped with a shear wave device. Data were collected and analyzed across different age groups and TMJ conditions, including “no changes”, “exudate”, “arthrosis”, and “disc displacement”. Results: The study found that males exhibited higher muscle tension across all conditions, particularly in the “no changes” (40.4 kPa vs. 32.1 kPa, 25.9% higher) and “exudate” (38.5 kPa vs. 29.7 kPa, 29.6% higher) categories, indicating increased muscle strain and inflammation during middle age. In females, a trend of decreasing muscle tension with age was observed, with a significant reduction from 36.2 kPa in the 20–30 age group to 24.3 kPa in the 60–70 age group (32.9% reduction), suggesting a reduction in muscle mass or strength due to aging. Both genders showed high muscle tension in the presence of exudate, with females peaking in the 40–50 age group at 37.1 kPa and males peaking earlier in the 20–30 age group at 41.2 kPa (10.9% higher in males), highlighting potential gender differences in inflammatory response. In the arthrosis group, males displayed a consistent increase in muscle tension with age, peaking at 37.5 kPa in the 50–60 age group (50.7% increase from the 20–30 age group), while females showed high tension, particularly in the 40–50 age group at 31.0 kPa (82.4% higher compared to the 20–30 age group), indicating the need for targeted joint health interventions in middle-aged women. Conclusions: This study reveals significant gender-specific differences in masseter muscle tension among patients with TMJ pain. Males were found to be more affected by muscle strain and inflammation during middle age, whereas females showed a significant decrease in muscle tension with age. The presence of exudate significantly impacted muscle tension across all age groups for both genders. These findings underscore the importance of tailored clinical interventions and preventive strategies to manage TMJ disorders effectively. Full article

Introduction: Osteoarthritis is a degenerative condition of the cartilage, often common among the population and occurs frequently with aging. Many factors are decisive for the development of its pathogenesis such as age, obesity, trauma, mechanical load, and modification of synovial biology. The main features of osteoarthritis are chondrocytes and cartilage matrix loss, which lead to pain, loss of function of the whole joint, and disability, representing a relevant health problem. Recently, a new therapeutic approach based on cell therapy has been studying the regenerative ability of mesenchymal stem cells for osteoarthritic chondrocytes. Aim: This in vitro study clarifies the regenerative effects of multipotent adipose-derived stem cells and the pluripotent amniotic epithelial stem cells on arthrosis chondrocytes by performing co-culture experiments. Methods: We studied the regenerative potential of secretome (soluble factors and extracellular vesicles), mesenchymal stem cells, and the adipose stromal vascular fraction. The regenerative effects were evaluated by gene and protein expression analysis of articular cartilage-specific genes and proteins like col2a1, acan, and sox9. Results: Mesenchymal stem cells, secretome, and adipose stromal vascular fractions influenced the cartilage genes and protein expression. Conclusions: The results indicate that the treatment with mesenchymal stem cells could be the best biological approach for cartilage regenerative medicine. Full article

Socioeconomic Gender Variables Impact the Association between Hypertension and Chronic Health Issues: Cross-Sectional Study

Our aim is to investigate if sex and gender influence the association of hypertension and their comorbidities. We investigated how gender differences in five socioeconomic factors impact the relation between hypertension and ten comorbidities including diabetes mellitus, renal disease, and chronic pulmonary disease in European countries grouped by their gender inequality index using representative survey data from the European Health Interview Survey. Using logistic regressions, we compute the ratio of odds ratios in females versus males. Therefore, an ORR > 1 is associated with a higher odds ratio for females than for males while an ORR < 1 means the opposite. To account for multiple hypothesis testing, we applied the Bonferroni correction. Hypertension in both sexes was associated with lower educational level, being unemployed, and lower income. In males, being divorced/widowed (OR1.12, p < 0.001) had an association to hypertension, whereas in females, being common-law/married (OR1.30, p < 0.001) and being divorced/widowed (OR1.17, p < 0.001) was associated with a higher risk for hypertension. Moreover, in hypertension, females who worked had an association with myocardial infarction (OR1.39, p < 0.001) and having post-secondary education had an association with arthrosis (OR 1.35, p < 0.001) compared to males. Our findings show that gender variables influence the association of hypertension and comorbidities, especially in females. These results can be used to inform targeted prevention measures taking gender-specific contextual factors into account. Full article

Nifuroxazide Prevents Chikungunya Virus Infection Both In Vitro and In Vivo via Suppressing Viral Replication

Chikungunya virus (CHIKV) is a reemerging arbovirus causing disease on a global scale, and the potential for its epidemics remains high. CHIKV has caused millions of cases and heavy economic burdens around the world, while there are no available approved antiviral therapies to date. In this study, nifuroxazide, an FDA-approved antibiotic for acute diarrhea or colitis, was found to significantly inhibit a variety of arboviruses, although its antiviral activity varied among different target cell types. Nifuroxazide exhibited relatively high inhibitory efficiency in yellow fever virus (YFV) infection of the hepatoma cell line Huh7, tick-borne encephalitis virus (TBEV) and west nile virus (WNV) infection of the vascular endothelial cell line HUVEC, and CHIKV infection of both Huh7 cells and HUVECs, while it barely affected the viral invasion of neurons. Further systematic studies on the action stage of nifuroxazide showed that nifuroxazide mainly inhibited in the viral replication stage. In vivo, nifuroxazide significantly reduced the viral load in muscles and protected mice from CHIKV-induced footpad swelling, an inflammation injury within the arthrosis of infected mice. These results suggest that nifuroxazide has a potential clinical application as an antiviral drug, such as in the treatment of CHIKV infection. Full article

Knee osteoarthritis (KOA) is a chronic degenerative disease characterized by progressive joint damage leading to significant disability. Although rehabilitative treatment methods for KOA have been widely implemented, the optimal integrated instrumental physical therapy approach remains unclear. Therefore, this study aimed to analyze the effect of Quantum Molecular Resonance (QMR) on pain reduction as the primary outcome and the functional improvement in activity daily living (ADL) as a secondary outcome. The study was designed as a double-blind, randomized, controlled trial in an outpatient setting. Fifty-four (N = 54) patients were enrolled and then randomized into three groups according to a simple randomization list: Group 1 (intensive protocol, N = 22), Group 2 (extensive protocol, N = 21), and a Sham group (N = 11). Patients were evaluated over time with scales assessing pain and function. Treatment was performed with the QMR model electro-medical device, which generates alternating electric currents characterized by high frequency (4–64 MHz). The results showed that QMR had a positive effect with respect to the Sham group in terms of pain and function (p < 0.01), and intensive treatment was more effective than the extensive treatment in terms of “speed of response” to the treatment (p < 0.05). In conclusion, QMR in KOA could be effective in slowing the progression of clinical symptoms and improving patients’ pain and functionality and thus quality of life. Future studies will be necessary to investigate further treatment algorithms and therapeutic associations with rehabilitative exercise. Full article

Total knee arthroplasty (TKA) is a gold standard surgical procedure to improve pain and restore function in patients affected by moderate-to-severe severe gonarthrosis refractory to conservative treatments. Indeed, millions of these procedures are conducted yearly worldwide, with their number expected to increase in an ageing and more demanding population. Despite the progress that has been made in optimizing surgical techniques, prosthetic designs, and durability, up to 20% of patients are dissatisfied by the procedure or still report knee pain. From this perspective, the introduction of robotic TKA (R-TKA) in the late 1990s represented a valuable instrument in performing more accurate bone cuts and improving clinical outcomes. On the other hand, prolonged operative time, increased complications, and high costs of the devices slow down the diffusion of R-TKA. The advent of newer technological devices, including those using navigation systems, has made robotic surgery in the operatory room more common since the last decade. At present, many different robots are available, representing promising solutions to avoid persistent knee pain after TKA. We hereby describe their functionality, analyze potential benefits, and hint at future perspectives in this promising field. Full article
