The Effect of Insomnia on the Outcomes of Physical Therapy in Patients with Cervical and Lumbar Pain in Clinical Practice

Background and Objectives: The objective of the study is to determine whether there is a difference in physical therapy outcomes in patients with cervical and/or lumbar pain who have insomnia compared to patients without insomnia during a two-week period of active treatment under the conditions of routine clinical practice. Materials and Methods: The study population consisted of two groups of subjects with chronic back pain, a group with insomnia (“case”) with a total of 38 subjects and a control group without insomnia (“control”) with a total of 41 subjects, who filled out a set of measurement questionnaires: the McGill Pain Questionnaire and its short form (SF-MPQ), the Insomnia Severity Index (ISI) and the European Quality of Life Questionnaire of Life (Euro Qol; EQ-5D). Determination of the biomarkers of structural damage to the nervous tissue, neurofilament polypeptide (NEF—neurofilament polypeptide), neuron-specific enolase (NSE—neuron-specific enolase) and protein S100B was performed by measuring their concentrations in the blood using the ELISA method (enzyme immunosorbent assay). Statistical analysis of the collected data included a descriptive analysis, hypothesis testing methods and univariable and multivariable regression models. Results: At the end of the treatment visits, the level of pain remained higher in some subjects of the experimental group, but the statistical significance of the baseline difference disappeared because of the higher relative treatment response in the controls. Measured with a visual analogue scale, the treatment improved the patients’ quality of life much more in experimental than control subjects, as is proven by the statistically significant difference for the percent change from baseline (~31% vs. ~14%). At baseline, all three neurotropic biomarkers had significantly higher serum values in the subjects of the experimental group than in the control patients, which suggested more damage to the neuronal structures. During the treatment course, their serum concentrations decreased, from 36% to 95%, but for S100B, unlike NES and NEF, there was no statistically significant difference between the study groups at the end of the treatment visits. Conclusions: The results of the study have immediate scientific and practical significance because they contribute to new knowledge about the place and role of insomnia in patients with cervical and/or lumbar pain who are treated with physical medicine methods in the conditions of routine clinical practice. The treatment of insomnia should be an indispensable part of therapeutic treatment for patients with back pain. Full article

Background and Objectives: To compare the oral-health-related quality of life (OHRQoL) outcomes between patients treated with modified Ni-Ti spring-based alignment appliances or conventional fixed appliances using the Oral Health Impact Profile 14 (OHIP-14), as well as the levels of satisfaction with the appliance appearance, treatment progress, and outcomes. Materials and Methods: Thirty-six patients (11 males, 25 females) were randomly divided into two groups: either the modified aligner appliance with Ni-Ti springs group (MAA) or the traditional fixed appliances group (FA). The allocation ratio was 1:1, and the randomization process was carried out by an independent investigator not involved in this study. Mild crowding cases were included in this study. The OHRQoL of patients was evaluated using the short-form Oral Health Impact Profile (OHIP-14) at five time points: before the treatment commencement (T0); 2 weeks (T1), 1 month (T2), and 2 months (T3) after the treatment initiation; and post-treatment (T4). The visual analog scale (VAS) was used to evaluate the patient satisfaction. Blinding was performed only during the outcomes assessment. Results: This randomized controlled trial had no dropouts, and the demographic characteristics of the groups were comparable. The MAA group experienced significantly greater functional limitations compared with the FA group at all three evaluation time points (T1, T2, and T3), as evidenced by the statistically significant p-values (p = 0.004, p = 0.001, and p < 0.001, respectively). The psychological disability in the MAA group was significantly lower than in the FA group at both T2 (p = 0.005) and T3 (p = 0.003). The patient satisfaction with the appliance appearance was significantly higher in the MAA group than in the FA group (p = 0.002). Conclusions: The OHRQoL improved in both the modified aligner appliance with Ni-Ti springs and fixed appliance groups after the treatment. Moreover, the functional limitations during the treatment were less severe in the FA group, while the psychological disability was lower, and the patient satisfaction with the appliance appearance was higher in the MAA group. Background and Objectives: Fibromyalgia is a multifaceted and frequently misunderstood chronic pain disease marked by widespread musculoskeletal pain and cognitive/somatic dysfunction. This trial aims to contribute to the existing knowledge on treating fibromyalgia (FM) with medical cannabis (Cannabis sativa L.) and explore a safer and more effective cannabis administration method. The goal is to provide evidence-based findings that can guide alternative treatment options for FM patients by assessing a pilot study. Materials and Methods: The trial was performed at the pain therapy unit of the San Carlo Hospital (Potenza, Italy) by administrating to 30 FM patients 100 mg/day of Bedrocan® (Bedrocan International, Veendam, The Netherlands) as a decoction. The Numerical Rating Scale (NRS) and SF-12 short-form health questionnaire were used to evaluate pain intensity and the quality of life at the beginning of the study and the 6th-month follow-up. A systematic review of all clinical studies investigating the use of cannabis to reduce FM was also undertaken to place this study in the context of the existing evidence base. Results: Pain intensity evaluated with the NRS lowered from a median of 8 [95% CI 7.66–8.54] at a baseline to a median of 4 (95% CI 3.28–4.79) after 6 months of follow-up (p-value < 0.001; t-test). Similarly, significant physical and mental state improvement, evaluated with the SF-12 questionnaire, was found in 96.67% and 82.33% of patients, respectively (95% CI 44.11–51.13 for the physical state, and 53.48–58.69 for mental state assessed after the 6th-month follow-up; p-value < 0.001; t-test). The systematic analysis of the literature identified 10 clinical trials concerning the treatment of fibromyalgia with cannabis. Conclusions: Considering results from the present pilot study and systematic review, it is possible to assume that medical cannabis may be considered an alternative therapy for FM patients who do not respond to conventional pharmacological therapy. Background and Objectives: Irritable bowel syndrome (IBS) is an invasive and potentially disabling syndrome characterized by a multitude of symptoms capable of reducing the quality of life of patients. Among the most disabling symptoms of IBS is certainly physical pain, which manifests itself mainly at the abdominal level but can also appear in other areas of the body, particularly in the form of chronic low-back pain (CLBP). Among the non-invasive methods of treating organ-specific pathologies and organ-related musculoskeletal problems, the use of Bioresonance Therapy (BT)—based on the administration of self-modulating Extremely Low-Frequency Electromagnetic Fields, capable of determining a rebalance of bio-electrical and metabolic activity in the presence of various functional alterations—is currently gaining acceptance. Therefore, we decided to monitor results obtained from patients suffering from IBS and CLBP subjected to a cycle of treatments with BT. Materials and Methods: We monitored 20 patients (12 women and 8 men, average age of 51 years) suffering from CLBP and other visceral symptoms related to IBS. Patients were monitored through the use of the Bristol Stool Form Scale (BSFS), the Fecal Calprotectin test and the Short-Form Health Survey 36 (SF-36), collected before (T0) and after (T1) the execution of the cycle of treatments. They undertook a treatment protocol consisting of eight sessions of BT carried out over about a month. Results: At the end of the treatments with BT, it was possible to observe a general and significant improvement in all the parameters observed, as well as a close inversely proportional correlation between the Calprotectin values detected and the quality of life experienced by the patients in relation to their perceived IBS symptoms. Conclusions: Overall, our pilot study would seem to suggest a potential beneficial effect of BT in modulating organic and musculoskeletal symptoms derived from IBS. Full article

Background: Materialism is an attitude that considers material goods to be central in life. Nowadays, adolescents appear to have a high level of materialism, which is related to risky behaviors. Nevertheless, there is a lack of measurement tools with adequate psychometric properties to assess materialism in this age group. For this reason, two studies were conducted to investigate the psychometric properties of the original and short Material Values Scale (MVS) in adolescents. Methods: In Study 1, participants were randomly split into two subsamples to compare psychometric properties of the original version of MVS with those of the short one. The first subsample consisted of 1054 adolescents (58% male; Mage = 16.34; SD = 1.15), and the second one of 1058 adolescents (57% male; Mage = 16.26; SD = 1.04). In Study 2, the psychometric properties of a revised version of the short MVS (without item 8) were investigated to confirm its adequacy with a new sample composed of 1896 adolescents (60% male; Mage = 16.40; SD = 2.76). Results: Results of Study 1 showed that the short version appeared to be a better measuring tool with respect to the long form to investigate materialism in adolescents. Nevertheless, problems with item 8 emerged. Results of Study 2 attested to the adequacy of the psychometric properties of the revised version of the short MVS (by excluding item 8) in this age group, in terms of dimensionality, reliability, and validity. Conclusions: Findings show that the revised short version of the MVS could be a valid and reliable tool for measuring the multidimensional construct of materialism in Italian adolescents. Soil amendments play a crucial role in modern agriculture, as they effectively enhance the planting environment. This study innovatively proposes the use of gel as a crosslinking agent to embed biochar and hydroxyapatite (HAP), thereby preparing a novel soil amendment. Furthermore, this study investigates the soil improvement effects of this amendment as well as its influence on plant growth. This study employed a hydrothermal method to combine corn stalk (CB) or sludge (SB) biochar with HAP at different ratios (0–20%). Subsequently, sodium alginate gel (SA) was utilized to encapsulate the biochar and minerals, successfully forming a ternary composite gel material (corn stalk biochar/sludge biochar–sodium alginate gel-hydroxyapatite: CB/SB-SA-HAP). Finally, the practical effectiveness of this amendment was verified through potted soil experiments. The results indicate that the CB/SB-SA-HAP composite materials exhibited a micrometre-scale spherical structure with well-developed micropores and possess the functional groups of CB/SB, SA, and HAP, along with unique mineral properties. Through pot experiments, it was verified that the composite material effectively enhances multiple soil properties. After 21 days of cultivation, the soil pH values stabilized within the neutral range (pH = 7 ± 0.3) across all treatment groups. Except for the CB0 (CB:HAP = 1:0) and CB2.0 (CB:HAP = 1:2) treatments, the remaining treatments significantly reduced the soil EC values by 3.27% to 47.92%. All treatments significantly increased the contents of alkali-hydrolysable nitrogen (AHN) (34.89~57.91%), available phosphorus (AP) (35.93~56.55%), and available potassium (AK) (36.41~56.80%) in the soil. In comparison, although the SB treatment was more effective in regulating the pH and electrical conductivity (EC) of saline–alkali soil than the CB treatment, it was less effective in promoting plant growth in the short term. Through correlation analysis and redundancy analysis, a significant positive correlation was found between soil pH and ryegrass germination rate and plant height, particularly with the most pronounced impact on soil pH observed in the CB1.0 and SB0 (SB:HAP = 1:0) treatments. This study underscores the potential of CB/SB-SA-HAP composite materials in soil improvement and plant growth promotion, providing valuable insights for soil remediation, enhancement, and plant cultivation advancements in the agricultural sector. The formation of materials with negative thermal expansion (NTE) based on a phase transition-type mechanism (NTE-II) in 50 T–x (temperature–composition) RF3-R’F3 (R = La-Lu) systems out of 105 possible is predicted. The components of these systems are “motherRF3 compounds (R = Pm, Sm, Eu, and Gd) with polymorphic transformations (PolTrs), which occur during heating between the main structural types of RF3: β-(β-YF3) → t-(mineral tysonite LaF3). The PolTr is characterized by a density anomaly: the formula volume (Vform) of the low-temperature modification (Vβ-) is higher than that of the high-temperature modification (Vt-) by a giant value (up to 4.7%). In RF3-R’F3 systems, isomorphic substitutions chemically modify RF3 by forming R1−xR’xF3 solid solutions (ss) based on both modifications. A two-phase composite (β-ss + t-ss) is a two-component NTE-II material with adjustable parameters. The prospects of using the material are estimated using the parameter of the average volume change (ΔV/Vav). The Vav at a fixed gross composition of a system is determined by the β-ss and t-ss decay (synthesis) curves and the temperature T. The regulation of ΔV/Vav is achieved by changing T within a “window ΔT”. The available ΔT values are determined using phase diagrams. A chemical classification (ChCl) translates the search for NTE-II materials from 15 RF3 into an array of 105 RF3-R’F3 systems. Phase diagrams are divided into 10 types of systems (TypeSs), in four of which NTE-II materials are formed. The tables of the systems that comprise these TypeSs are presented. The position of Ttrans of the PolTr on the T scale for a short quasi-system (QS) “from PmF3 to TbF3” determines the interval of the ΔTtrans offset achievable in the RF3-R’F3 systems: from −148 to 1186 ± 10 °C. NTE-II fluoride materials exceed known NTE-II materials by almost three times in this parameter. Equilibrium in RF3-R’F3 systems is established quickly. The number of qualitatively different two-component fluoride materials with the giant NTE-II can be increased by more than ten times compared to RF3 with NTE-II. Full article

One of the main objectives of today’s archaeological sites and museums is the development of research, understood as the interpretation and contextualisation of tangible and intangible cultural heritage to broaden the knowledge and accessibility of archaeological parks often unknown to visitors and the public on a large scale. In this perspective, the Appia Antica Archaeological Park aims to support research in digitising infrastructures and archaeological contexts of high historical and cultural value to plan short- and medium-term preservation and maintenance projects. In this context, unmanned aerial vehicles (UAVs) are tools with enormous potential in survey, inspection and digitisation, providing the basis for the subsequent phases of data interpretation, representation and material analysis. Thanks to the photorealistic reconstruction of dense structure from motion (DSfM) in the application of structural inspections, today it is possible to intercept the geometry and material conditions of small, medium and large structures, reducing the costs of inspections, limiting the interruption of the public and providing professionals and visitors with a better volumetric understanding of the system. However, inserting information that gradually accumulates throughout the process requires advanced 3D digital representation techniques, such as HBIM (historic building information modelling), scan-to-BIM approach and interactive forms, such as virtual and augmented reality (VR-AR). For these reasons, this study summarises the experience and lessons learned from the UAV inspection of three research case studies at archaeological, architectural, and infrastructure scales to increase awareness of the Roman-built heritage. In order to avoid the risk of tailing pond failures and to minimize the post-failure losses, it is necessary to analyze the current operation status of tailings ponds, to explore the evolution law of their failure process, to grasp their post-failure impact range, and to propose corresponding effective prevention and control measures. Based on a tailings pond in China, this paper establishes a 1:200 scale indoor model to explore the evolution law of post-failure tailings discharge in a tailings pond under flooded roof conditions; secondly, the finite element difference method and smooth particle fluid dynamics are combined to compare and analyze the post-failure impact area and to delineate the risk prevention and control area. The results of the study show that, during the dam break, the lower tailing sand in the breach is the first to slip, and after forming a steep can, the upper tailing sand in the steep can is pulled to slip, so that the erosion trench mainly develops vertically first, and then laterally. The velocity of the discharged tailing sand will quickly reach its peak value in a short period of time and then decrease to the creeping stage; the front edge of the sand flow is the first to stop moving, and the trailing edge of the tailing sand accumulation depth continues to increase until the end of the dam failure, at which point the initial bottom dam area of the discharge tailing sand flow velocity is the largest. The further the tailings are released from the initial dam, the smaller the accumulation depth and the larger the particle size, and the larger the elevation of the foundation in the same section, the smaller the accumulation depth and the larger the particle size; further, the presence of blocking materials will increase the local tailings accumulation depth. Based on the maximum flow velocity of the discharged tailings and the accumulation depth, the risk area downstream of the tailings pond is divided, so that relocation measures can be formulated. The test results can provide an important reference for the operation and management of similar tailings ponds. Full article

Materials and Methods: Patients were radiographically and clinically evaluated at final follow-up, by using the validated American Orthopedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot score for the clinical–functional assessment, the Short-Form 12 (SF-12) for the physical and psychological domain assessment, and the Visual Analog Scale (VAS) for pain. Results: In total, 38 patients (42 CF) met the inclusion criteria and were retrospectively evaluated and divided into two groups (Kirschner group and plate group) consisting of 19 patients each. The overall mean follow-up was 59.4 ± 11.8 months. The average values of the post-operative clinical outcomes of the two groups KG and PG were, respectively, 70.7 ± 11.9 and 70.1 ± 10.9 (AOFAS), 45.7 ± 6.8 and 46.5 ± 10.8 (SF-12 PCS), 54.7 ± 9.9 and 50.9 ± 11.8 (SF-12 MCS) at the final follow-up. Conclusions: The present study showed that in the cases analyzed, the two surgical approaches used for the treatment of CF achieved comparable clinical outcomes. The only substantial difference found between the two groups of patients was the re-intervention rate that afflicted them. Full article

Background and objectives: Quality of life and sleep quality of college students were extensively studied. The present study evaluated sleep quality and quality of life of college students in Taiwan by using the Pittsburgh Sleep Quality Index (PSQI) and Short-Form Health Survey (SF-36), respectively. Materials and Methods: Data of 1756 college students aged 20–24 years were collected in this study. Association rule analysis was also used to provide a graphics-based visualization of the relationships between data, enabling the rapid identification of data correlations. Results: The results showed that the average physical component scale (PCS) and average mental component scale (MCS) scores were 52.9 and 44.1, respectively. Based on their body mass index (BMI), participants were divided into underweight, normal, overweight, and obese groups. The results of one-way analysis of variance showed that the p values for the PSQI, PCS, and MCS scores were 3.5 × 10−5, 1.7 × 10−5, and 0.671, respectively. The normal and overweight groups had the lowest PSQI scores. The PCS score of the obese group was lower than that of normal and overweight groups. The p values of the t-test result among PSQI, BMI, PCS, and MCS groups were 0.002, <2 × 10−16, and <2 × 10−16, respectively. The good sleep quality group had higher PCS and MCS scores. Conclusions: In this study, the results of association rule analysis indicated two distinct groups: Group 1, with the characteristics of good sleep quality as revealed by the high MCS and PCS scores, and Group 2, with the characteristics of poor sleep quality as revealed by low MCS and PCS scores and underweight BMI. Arsenic contamination of groundwater occurs due to both geogenic and anthropogenic processes. Conventional arsenic remediation techniques require extraction of groundwater into pump-and-treat systems, which are expensive and require long operational times. Hence, there is a need for cost-effective remediation. In this study, we assessed and validated the in situ remediation of arsenic contamination in groundwater resources using permeable reactive barriers (PRBs) made of injectable, colloidal iron oxide nanoparticles in the laboratory and in field-scale pilot tests. Sand-packed, flow-through column studies were used in order to assess the sorption behavior of the iron oxide nanoparticles using field materials (sand, groundwater) in the laboratory. The breakthrough curves were analyzed using a reactive transport model considering linear and nonlinear adsorption isotherms and were fitted best with a chemical nonequilibrium consideration. The results were used to design a pilot-scale field test. The injected 28 m3 of nanoparticles (ca. 280 kg dry weight of iron oxide) were successfully delivered to the aquifer via an injection well. No mobile iron was detected downstream, confirming that a stable in situ barrier was formed that did not move with the groundwater flow. Arsenic concentrations in groundwater were reduced to the aimed 50% of the background value, despite the relatively short contact time between arsenic and the iron oxide in the barrier, due to the high flow velocity of 1.21 m/day. We compared the results of the laboratory and field tests and concluded that the single-parameter models based on retardation factor and/or adsorption capacity fail to predict the longevity of the barrier and the evolution of arsenic breakthrough with time, most likely because they do not consider the chemical nonequilibrium effects. Therefore, we propose that upscaling the laboratory findings to field design must be carried out with care and be coupled with detailed reactive transport models. Full article

Nutrient enrichment of lakes from anthropogenic activities is a significant and increasing issue globally, impairing the health, biodiversity and service provisioning from lakes, with impacts on cultural, recreational, economic and aesthetic values. Internal nutrient loads from lakebed sediment releases are a primary cause of lake eutrophication and have necessitated geoengineering methods to mitigate releases and speed up recovery from eutrophication. Our objective in this review was to evaluate the use of oxygen nanobubbles as a geoengineering technology to remediate low oxygen conditions at the lake sediment/water interface, as a precursor to alleviating eutrophication linked to high internal nutrient loads. Oxygen nanobubbles (NBs) are bubbles < 1000 nm formed at the interface of solid surfaces and aqueous solutions. These bubbles have higher density than water, persist for longer and facilitate greater oxygen solubility than larger bubbles. Methods have been developed to enable NB formation at the surface of carrier materials, which are then used in conjunction with modified local soils (MLSs), to ‘floc, lock and oxygenate’ to strip nutrients from the water column, locking them in lakebed sediments and oxygenating the sediments to prevent re-release of nutrients. Most studies of NBs for lake restoration have thus far only demonstrated their potential for this purpose, using short-term, small-scale core incubations conducted mainly in laboratory settings. Work is required to (1) address scalability, including procurement and cost, (2) extend laboratory incubation studies to large outdoor enclosures and pond/lake trials, (3) examine longevity of the effects in the natural environment, including potential for MLSs to smother benthos and/or have toxic effects, and (4) extend to a range of lake environments and MLS types. Legal, cultural and social acceptance of the technology is another prerequisite of applications in the natural environment and requires individualised analysis. Until these issues are addressed in a systematic way that addresses scalability and recommends suitable carrier materials and MLSs, NBs may continue to remain largely untried as a geoengineering method to address lake eutrophication. Background and Objectives: Knee osteoarthritis (OA) is a frequent cause of pain, functional limitations, and a common reason for surgical treatment, such as joint replacement. Conservative therapies can reduce pain and improve function; thus, delaying or even preventing surgical intervention. Various individual conservative therapies show benefits, but combination therapies remain underexplored. The aim of this prospective case-study was to assess the effect of a conservative combination therapy in patients with painful varus knee OA. Materials and Methods: With strong inclusion and exclusion criteria, nine patients with painful varus knee OA (mean age 56 years (range 51–63 years) were selected and monitored over six months, using the following clinical outcome scores: pain visual analog scale (VAS), Western Ontario and McMaster Universities osteoarthritis index (WOMAC score), short-form–36 items (SF-36) quality of life score, and the sports frequency score. All patients received a standardized conservative trio-therapy with varus-reducing hindfoot shoe-insoles with a lateral hindfoot wedge, oral viscosupplementation, and physiotherapy for six months. Results: The pain was reduced significantly from initial VAS values of 5.4 points (range, 3–10) to values of 0.6 points (range, 0–3; p < 0.01), at the end of treatment. After six months, seven out of nine patients reported no pain at all (VAS 0). The WOMAC score improved significantly, from initial values of 35 (range, 10–56) to values of 2 (range, 0–9; p < 0.01). The SF-36 score showed significant improvement after six months in all four domains of physical health (p < 0.01) and in two of the four domains of mental health (p < 0.05). The sports frequency score increased by at least one level in six out of nine patients after six months. Conclusions: The conservative trio-therapy in patients with varus knee OA showed positive initial clinical results: less pain, higher function, better quality of life, and higher sport activity. Further studies are required to evaluate the long-term effect. Full article

Background and Objectives: The aim of this study was to evaluate short-term continuous positive air pressure (CPAP) treatment for health-related quality of life (HRQL) in patients with obstructive sleep apnea. Materials and Methods: Our subjects were 18–65 years old, diagnosed with moderate-to-severe obstructive sleep apnea and treated with CPAP between January 2020 and June 2021 in Hospital of Lithuanian University of Health Sciences Kaunas clinics. All the patients completed the Epworth Sleepiness Scale (ESS), the 36-Item Short Form Health Survey (SF-36), the and Pittsburgh Sleep Quality Index (PSQI) before and after 3 months of treatment. Polysomnography was also repeated. Statistical analyses were performed using SPSS 27.0 software. The value of p < 0.05 was considered as statistically significant. Results: The active-treatment group comprised 17 subjects with a mean age of 51.9 ± 8.9 years. The total SF-36 questionnaire score improved from 499.8 ± 122.3 to 589.6 ± 124.7 (p = 0.012). The SF-36 role limitations due to emotional problems (p = 0.021), energy (fatigue) (p = 0.035), and general health (p = 0.042) domains score significantly improved after CPAP treatment for 3 months. The PSQI mean score at baseline was 12.6 ± 2.9 and in the post-treatment group, it was −5.5 ± 2.3 (p = 0.001). The ESS also changed significantly from a pretreatment mean score of 10.9 ± 5.7 to −5.3 ± 3.2 (p = 0.002) after 3 months. Conclusions: Improvement in HRQL is seen even after a short treatment period with CPAP. Questionnaires are a good tool to evaluate CPAP treatment efficacy. Full article
