The authors report a case of superior ophthalmic vein occlusion in a patient with both Factor V Laiden and MTHFR mutations. I believe this is the first time such case is reported and it definitely deserves to be published in order to expand our knowledge on hypercaogulable blood states affecting the eye. I would like the authors to have their manuscript revised by a native English speaker to correct some language errors. I also suggest detailing the dosage and duration of systemic therapy administered to the patient.

The case study titled “Ophthalmic vein thrombosis associated with Factor V Leiden and MTHFR mutations” by   Teodoru et al. is talking about the Superior ophthalmic vein thrombosis (SOVT), diagnosed in 77--year-old woman, and correlated with the heterozygous mutation in Factor V Leiden and homozygous mutations methylenetetrahydrofolate reductase.  The case study is a well-written and well-presented article.

The point-wise comments are as follows.

1.    There is missing material and methods in the article.

2.    How were the mutations explored? What was the strategy?

3.    Missing an arrow in figure 1 indicating the SOVT.

4.    What does INR stand for? on page 3 line 100.

5.    What was the rationale for exploring Factor V Leiden and MTHFR mutation, and what are the clinical implications?

This manuscript is a case report regarding a 77-year-old woman affected by superior ophthalmic vein thrombosis, a rare clinical entity.

This article could be considered interesting, even if the Figures should be improved, especially that one of the CT scan. In fact, the authors should better show the hyper reflectivity of superior ophthalmic vein and the hypertrophy of the lower right muscle. They should use circles or arrows to better indicate these findings to the readers.

Then, the English should be revised throughout the manuscript.
