This is an appropiated well done review about current state of knowledge on SCAD with focusing in clinical findings and multimodality imaging techniques.

The options of coronary baypass grafting and post-procedural management in SCAD general patients, not only in pregnant or lactating patients should be described.

Also, the pivotal points of medical theraphy after SACD should be described.

The paper " Multimodalitu Imaging approach to sponatneous coronary artery dissection" is an interesting review about an extremely insidious issue.

It is a well written paper and appropriately addresses the relevant issues.

I have just a couple of suggestions to propose:

1 Shortening  a little bit the paper

2 Adding a brief comment about the risk of relapse and on how to deal with the follow-up of these patients after the acute phase, taking for granted the usefulness of a period of  cardiac rehabilitation
