Cover Story: In this study, we analyzed the characteristics of spongiosis formation in atopic dermatitis (AD) and other skin diseases using immunohistochemical methods. The results showed that infiltration of inflammatory dendritic epidermal cells (IDECs) and T-cells, accompanied by degenerated keratinocytes and aggregated Langerhans cells, was frequently observed in the spongiosis of multiple diseases. However, IgE-expressing IDECs were only detected in AD associated with IgE-mediated delayed-type hypersensitivity. Aggregation of IDECs was prominently observed in adaptive immune-mediated eczematous dermatitis. These IDEC aggregations constituted the major component of epidermal DC clusters seen in eczematous dermatitis, and their presence may serve as a useful distinguishing feature from Pautrier collections seen in cutaneous T-cell lymphoma. View this paper
