Dear Colleagues,
There is an urgent need to optimize diagnosis and therapy according to sex and gender differences.
However, until a few years ago, imaging and drugs were developed and studied without taking into account sex and gender and without any consideration of the context of where the person lives.
Today, we need to consider biological, biochemical, environmental, psychological, and psychosocial factors that may alter gene expression.
The administration of contrast media for imaging methods, such as computed tomography (CT), magnetic resonance (MRI), or contrast-enhanced spectral mammography (CESM), should depend on the different pharmacokinetics in male or female, and not only from the patient weight.
Furthermore, ionizing radiation exposure should be different in sex because of the absorbed dose.
The gender gap is evident in the epidemiology, presentation, mechanisms, outcomes, and treatments of a multitude of diseases and should be the first step to personalized medicine and therapy.
This approach to the patients ensure appropriateness of diagnosis and makes it easier to propose also a personalized therapy.
In "omics" era, “personomics” can support the importance of life experiences and underline their relevance in health and tailored medicine.
This Special Issue aims to present and discuss a new approach to imaging and therapy, considering gender and sex differences, that are essential to arrive at real personalized medicine and a patient oriented medicine.

Submitted manuscripts should not have been published previously, nor be under consideration for publication elsewhere (except conference proceedings papers). All manuscripts are thoroughly refereed through a single-blind peer-review process. Diagnostics is an international peer-reviewed open access semimonthly journal published by MDPI.
