Prevalence of Hearing Impairment in Saudi Arabia: Pathways to Early Diagnosis, Intervention, and National Policy

Background: Hearing impairment is a significant public health issue globally, yet national data for Saudi Arabia remain sparse. Methods: Using data from the 2017 Disability Survey, we analysed 12 hearing-related indicators across 13 administrative regions. Descriptive statistics, logistic regression, cluster analysis, and residual mapping were applied to identify socio-demographic disparities and service gaps. Findings: Among 20,408,362 Saudi nationals, about 1,445,723 (7.1%) reported at least one functional difficulty. Of these, 289,355 individuals (1.4%) had hearing impairment, either alone or with other difficulties—229,541 (1.1%) had hearing impairment combined with other disabilities, while 59,814 (0.3%) had only hearing impairment. Females and males were equally affected. Notably, educational attainment and marital status significantly influenced device uptake; less-educated and divorced individuals were particularly underserved. Regionally, southern provinces (Al-Baha, Jazan, and Najran) demonstrated the highest unmet need due to geographic barriers, limited audiological resources, and socioeconomic constraints, reflecting compounded risks from consanguinity and rural isolation. Cluster analyses identified provinces requiring urgent attention, recommending mobile audiology units, tele-audiology services, and means-tested vouchers to enhance coverage. Conclusions: Despite Saudi Arabia’s existing public audiology services and a National Newborn Hearing Screening programme achieving 96% coverage, substantial gaps remain in follow-up care and specialist distribution, underscoring the necessity for systematic workforce tracking and enhanced rural incentives. International evidence from India and Brazil underscores the feasibility and cost-effectiveness (approximately USD 5200/QALY) of these recommended interventions. Implementing targeted provincial strategies, integrating audiological screening into routine healthcare visits, and aligning resource allocation with the WHO and Vision 2030 benchmarks will significantly mitigate hearing impairment’s health, social, and economic impacts, enhancing the quality of life and societal inclusion for affected individuals. Platinum-induced ototoxicity constitutes a significant adverse effect in pediatric oncology, frequently resulting in permanent hearing impairment with profound implications for quality of life, language acquisition, and scholastic performance. This comprehensive review critically evaluates contemporary ototoxicity monitoring practices across various pediatric oncology settings, analyzes current guideline recommendations, and formulates strategies for implementing standardized surveillance protocols. Through examination of recent literature—encompassing retrospective cohort investigations, international consensus recommendations, and functional outcome assessments—we present an integrated analysis of challenges and opportunities in managing chemotherapy-associated hearing loss among childhood cancer survivors. Our findings demonstrate marked heterogeneity in monitoring methodologies, substantial implementation obstacles, and considerable impact on survivors’ functional status across multiple domains. Particularly concerning is the persistent absence of an evidence-based consensus regarding the appropriate duration of audiological surveillance for this vulnerable population. We propose a structured framework for comprehensive ototoxicity management emphasizing prompt detection, standardized assessment techniques, and integrated long-term follow-up care to minimize the developmental consequences of platinum-induced hearing impairment. This approach addresses critical gaps in current practice while acknowledging resource limitations across diverse healthcare environments. Full article

Waardenburg syndrome (WS) is a rare genetic disorder that affects both hearing and pigmentation. The wide divergence of WS poses significant diagnostic and management challenges. This study investigated type 4 WS within an underrepresented Mongolian population. Whole-exome sequencing revealed that two unique heterozygous variants were identified in the SOX10 gene: c.393C>G (p.Asn131Lys) in a five-year-old female patient presenting with profound sensorineural hearing loss (SNHL), dystopia canthorum, and a white forelock; and c.535A>T (p.Lys179Ter) in a nine-year-old male patient presenting with profound SNHL, dystopia canthorum, and Hirschsprung’s disease. Temporal bone imaging revealed abnormalities in the inner ear structure in both patients. The genotypic and phenotypic characteristics were meticulously delineated, incorporating the deleterious effects of these variants, as evaluated by multiple predictive tools and the American College of Medical Genetics and Genomics (ACMG) criteria. In addition, structural characterizations were also presented using AlphaFold. The findings of this study contribute valuable genetic data to the limited literature on type 4 WS within this ethnic group and highlight the importance of genetic testing and multidisciplinary care for this rare disorder in settings with limited resources. Hearing loss (HL) is a major global health concern, with drug-induced ototoxicity contributing significantly, particularly in patients undergoing treatment for drug-resistant tuberculosis (DR-TB). In South Africa, where both TB and HIV are prevalent, the risk of treatment-related auditory damage is especially high. This study aimed to assess the prevalence and predictors of hearing impairment among DR-TB patients in rural Eastern Cape, South Africa. A retrospective analysis was conducted on 438 DR-TB patients treated between 2018 and 2020, using pure tone audiometry (PTA) to assess hearing status post-treatment. Demographic, clinical, and lifestyle data were extracted from patient records and analyzed using logistic regression. The overall prevalence of hearing loss was 37.2%. Risk was significantly associated with an older age, a male gender, DR-TB classification (MDR, pre-XDR, and XDR), unsuccessful treatment outcomes, and substance use. Prevalence of HL increased notably in patients aged 70 and older. Lifestyle factors, particularly combined use of tobacco, alcohol, and drugs, were linked to higher odds of HL. These findings underscore the need for routine audiometric screening and personalized treatment monitoring in DR-TB care, especially for high-risk populations. Early identification of ototoxicity risk factors can inform safer treatment regimens and improve patient outcomes in resource-limited settings. Full article

Background/Objectives: Cochlear implantation is an advantageous procedure for individuals with severe to profound hearing loss in many aspects related to auditory performance, social communication and quality of life. As machine learning applications have been used in the field of Otorhinolaryngology and Audiology in recent years, signal processing, speech perception and personalised optimisation of cochlear implantation are discussed. Methods: A comprehensive literature review was conducted in accordance with the PRISMA guidelines. PubMed, Scopus, Web of Science, Google Scholar and IEEE databases were searched for studies published between 2010 and 2025. We analyzed 59 articles that met the inclusion criteria. Rayyan AI software was used to classify the studies so that the risk of bias was reduced. Study design, machine learning algorithms, and audiological measurements were evaluated in the data analysis. Results: Machine learning applications were classified as preoperative evaluation, speech perception, and speech understanding in noise and other studies. The success rates of the articles are presented together with the number of articles changing over the years. It was observed that Random Forest, Decision Trees (96%), Bayesian Linear Regression (96.2%) and Extreme machine learning (99%) algorithms reached high accuracy rates. Conclusions: In cochlear implantation applications in the field of audiology, it has been observed that studies have been carried out with a variable number of people and data sets in different subfields. In machine learning applications, it is seen that a high amount of data, data diversity and long training times contribute to achieving high performance. However, more research is needed on deep learning applications in complex problems such as comprehension in noise that require time series processing. A significant communication gap persists between the deaf and hearing communities, often leaving deaf individuals isolated and marginalised. This challenge is especially pronounced for Arabic-speaking individuals, given the lack of publicly available Arabic Sign Language datasets and dedicated recognition systems. This study is the first to use the Temporal Convolutional Network (TCN) model for Arabic Sign Language (ArSL) recognition. We created a custom dataset of the 30 most common sentences in ArSL. We improved recognition performance by enhancing a Recurrent Neural Network (RNN) incorporating a Bidirectional Long Short-Term Memory (BiLSTM) model. Our approach achieved outstanding accuracy results compared to baseline RNN-BiLSTM models. This study contributes to developing recognition systems that could bridge communication barriers for the hearing-impaired community. Through a comparative analysis, we assessed the performance of the TCN and the enhanced RNN architecture in capturing the temporal dependencies and semantic nuances unique to Arabic Sign Language. The models are trained and evaluated using the created dataset of Arabic sign gestures based on recognition accuracy, processing speed, and robustness to variations in signing styles. This research provides insights into the strengths and limitations of TCNs and the enhanced RNN-BiLSTM by investigating their applicability in sign language recognition scenarios. The results indicate that the TCN model achieved an accuracy of 99.5%, while the original RNN-BiLSTM model initially achieved a 96% accuracy but improved to 99% after enhancement. While the accuracy gap between the two models was small, the TCN model demonstrated significant advantages in terms of computational efficiency, requiring fewer resources and achieving faster inference times. These factors make TCNs more practical for real-time sign language recognition applications. Full article

Background/Objectives: Hearing aid fitting is critical for hearing loss rehabilitation but involves complex, interdependent parameters, while AI-based technologies offer promise, their reliance on large datasets and cloud infrastructure limits their use in low-resource settings. In such cases, expert knowledge, manufacturer guidelines, and research findings become the primary sources of information. This study introduces DHAFES (Dynamic Hearing Aid Fitting Expert System), a personalized, ontology-based system for hearing aid fitting. Methods: A dataset of common patient complaints was analyzed to identify typical auditory issues. A multilingual self-assessment questionnaire was developed to efficiently collect user-reported complaints. With expert input, complaints were categorized and mapped to corresponding hearing aid solutions. An ontology, the Hearing Aid Fitting Ontology (HAFO), was developed using OWL 2. DHAFES, a decision support system, was then implemented to process inputs and generate fitting recommendations. Results: DHAFES supports 33 core complaint classes and ensures transparency and traceability. It operates offline and remotely, improving accessibility in resource-limited environments. Conclusions: DHAFES is a scalable, explainable, and clinically relevant solution for hearing aid fitting. Its ontology-based design enables adaptation to diverse clinical contexts and provides a foundation for future AI integration. Full article

Background: Hearing-related disease is a significant cause of disability worldwide. In resource-limited settings, prevention and early detection are critical for preventing severe disease. Understanding what a population knows and believes with regards to hearing health can be critical in identifying knowledge gaps and developing targeted interventions. Objective: To assess community awareness of hearing health and otitis media (OM) treatment, both modern and traditional, to inform educational programs. Methods: A retrospective review of clinical records from 52 patients (aged 1–79 years) diagnosed with OM during a 3-day hearing health clinic in Kasungu district, Malawi was conducted. Patients diagnosed with OM during the clinic were invited to provide additional details about their hearing health. Surveys contained open-ended questions to assess knowledge and beliefs regarding the cause of their infection and therapies they had previously used for treatment, including home remedies and prescribed medications from allopathic providers or traditional healers. A WHO adapted survey on hearing knowledge was also administered. Results: Hearing loss was identified in 60% of participants. Otoscopy revealed either bilateral or unilateral drainage in 69% of participants and perforation in 73%. Confidence in understanding the causes and treatments of OM was voiced by 60% of participants and 54% had used home remedies as treatment. Of the 11 home remedies used, none aligned with modern medical practice, and only two were recommended by local herbalists. Conclusions: Hearing-related disease contributes significantly to global disability, particularly in resource-limited settings. Educational campaigns to improve hearing health knowledge offer low-cost yet impactful solutions and implementation via partnerships with community leaders and traditional healers can be critical to addressing hearing health challenges. The use of nonantibiotic antimicrobials should be explored further, as these are low-cost and readily available. However, therapeutic alliance between patients and healthcare providers remains crucial. Full article

Hearing loss remains a significant but underexplored health challenge in individuals with HIV/AIDS, particularly those exposed to occupational noise. The ototoxic effects of antiretroviral therapy (ART) and comorbid conditions like tuberculosis (TB) further compound the risk. This narrative review examines the intersection of HIV/AIDS, ART, and occupational noise-induced hearing loss (ONIHL), emphasizing the South African and broader African contexts. The aim of the study was to map the existing literature on the association between HIV/AIDS, its treatments, and ONIHL, and to identify research gaps to inform policy and clinical practice. A narrative review approach was adopted, systematically searching databases including PubMed, Scopus, and Web of Science. Studies published between 2000 and 2024 were included, focusing on the effects of HIV/AIDS, ART, and occupational noise exposure on hearing health. Data extraction and thematic synthesis were performed to identify key findings and gaps. Twenty studies were included, covering diverse settings such as South Africa, Cameroon, Tanzania, and the USA. Three key themes emerged: (1) dual burden of HIV and occupational noise exposure: HIV-positive individuals in noise-intensive industries, such as mining, face amplified risks of hearing loss due to immunological compromise and ototoxic TB treatments; (2) ototoxicity of ART: older ART regimens, widely used in resource-limited settings, are associated with a higher prevalence of sensorineural hearing loss (SNHL); and (3) immunological susceptibility to ONIHL: HIV-related immune suppression exacerbates cochlear damage from noise and ototoxic agents, contributing to both peripheral and central auditory dysfunction. This review highlights the urgent need for integrated hearing health interventions in HIV care and occupational health frameworks, particularly in high-prevalence regions like South Africa. Routine audiological assessments, access to safer ART regimens, and enhanced workplace protections are essential to mitigate the dual burden of HIV/AIDS and ONIHL. Future research should prioritize longitudinal studies and innovative, low-cost solutions for resource-limited settings. Full article

Background: Microtia is a congenital outer ear deformity that causes the auricle to be absent or underdeveloped. It is frequently associated with external auditory canal atresia and causes hearing and psychosocial problems. Objectives: We thoroughly investigate the aspects of microtia and explore both current and innovative therapies. Methods: A systematic literature review was conducted following PRISMA guidelines, focusing on microtia and reconstruction methodologies. This review utilized three databases: PubMed, Scopus, and Web of Science. Results: The etiology involves both genetic and environmental factors and can occur as part of a syndrome or as an isolated condition. Clinically, it has esthetic and functional implications, potentially leading to conductive hearing loss. A multidisciplinary approach is essential for treatment, which includes surgical reconstruction using autologous cartilage or synthetic prostheses. Advances in bioengineering and 3D printing offer new, less invasive solutions. Conclusions: This review aims to synthesize current knowledge on microtia, focusing on tissue engineering for auricular reconstruction, identifying research gaps, evaluating techniques, and providing a resource for clinicians to improve decision-making and foster further research. Congenital CMV (cCMV) is the leading cause of hearing loss and neurodevelopmental disabilities in children, affecting 15–18% of births in the United States (U.S.). Despite its strong presence, the lack of routine prenatal screening limits its detection and prevention, especially in low-resource communities. The aim of this scoping review is to describe the scope of the CMV interventions and the policies on CMV screening in the United States, identify successful strategies for the implementation of improved CMV screening rates, and make recommendations for future efforts aimed at improving CMV screening in U.S. healthcare settings. The Joanna Briggs Institute recommendations for scoping reviews and the Arksey and O’Malley (2005) York methodology guided the different review steps. These included (1) identifying research questions; (2) searching for relevant studies; (3) selecting studies relevant to the research questions; (4) charting the data; (5) collating, summarizing, and reporting results. A total of six studies were retained for analysis. A common barrier reported across studies was the lack of generalizability of findings due to small sample size. Challenges encountered by patients often centered around awareness, emotional resistance, and cultural norms. Providers also faced significant barriers, particularly related to time constraints, knowledge gaps, and the prioritization of CMV education in prenatal care. Furthermore, a recurrent theme across included studies was the critical role of education in increasing awareness and reducing CMV transmission risks. Findings will contribute to the advancement of CMV prevention through the identification of gaps in education, policy, and practice, thus assuring better health outcomes for mothers and infants. Family-centred early hearing detection and intervention (FC-EHDI) is an established framework globally recognized for its emphasis on family involvement in supporting children who are deaf and hard of hearing (DHH). In the African context, unique sociocultural and systemic challenges necessitate tailored approaches to ensure effective implementation. This narrative review explores the relevance of FC-EHDI in Africa, highlights barriers to its implementation, and offers recommendations for creating sustainable and culturally aligned interventions. A narrative review methodology synthesizing evidence from African countries to examine the intersection of FC-EHDI with cultural practices, systemic barriers, and opportunities for innovation was adopted. Databases including PubMed, Scopus, Web of Science, and Google Scholar were searched for peer-reviewed journal articles, books, and reports published between 2000 and 2024. Keywords included “family-centred care”, “EHDI”, “Africa”, “cultural responsiveness”, and “early hearing detection and intervention”. Studies were included if they addressed EHDI in African contexts, explored family-centred approaches, or provided barriers and recommendations specific to the region. Thematic analysis was employed to synthesize findings into barriers, evidence, and strategies for FC-EHDI implementation. Data were extracted and analysed thematically to identify patterns and gaps in knowledge. Key challenges identified include resource limitations, economic constraints, linguistic and cultural diversity, and fragmented healthcare systems. Evidence highlights the effectiveness of community-based care, linguistic inclusivity, and culturally tailored interventions in enhancing family engagement and programme outcomes. Recommendations focus on leveraging technology, interdisciplinary collaboration, and policy advocacy. FC-EHDI offers a transformative approach to addressing DHH African children by integrating family involvement, cultural responsiveness, and systemic innovations. Future efforts should emphasize technology, scalable models, and family empowerment to create sustainable and equitable services. Full article

Hearing loss significantly impacts daily communication, making accurate speech audiometry (SA) assessment essential for diagnosis and treatment. However, SA testing is time-consuming and resource-intensive, limiting its accessibility in clinical practice. This study aimed to develop a multi-class classification model that predicts SA results using pure-tone audiometry (PTA) data, enabling a more efficient and automated assessment. To achieve this, we implemented and compared MLP, RNN, gradient boosting, and XGBoost models, evaluating their performance using accuracy, F1 score, log loss, and confusion matrix analysis. Experimental results showed that gradient boosting achieved the highest accuracy, 86.22%, while XGBoost demonstrated a more balanced classification performance. The MLP achieved 85.77% and the RNN achieved 85.41%, exhibiting relatively low accuracy, with the RNN showing limitations due to the low temporal dependency of PTA data. Additionally, all models faced challenges predicting class 2 (borderline hearing levels) due to overlapping data distributions. These findings suggest that machine learning models, particularly gradient boosting and XGBoost, outperform deep learning models in SA prediction. Future research should focus on feature engineering, hyperparameter optimization, and ensemble approaches to enhance performance and validate real-world applicability. The proposed model could contribute to automating SA prediction and improving hearing assessment efficiency and patient care. Full article

(1) Background: The safe execution of heavy machinery operations and high-risk construction tasks requires operators to manage multiple tasks, with a constant awareness of coworkers and hazards. With high demands on visual and auditory resources, vibrotactile feedback systems offer a solution to enhance awareness without overburdening vision or hearing. (2) Aim: This study evaluates the impact of vibrotactile feedback regarding proximity to hazards on multitasking performance and cognitive workload in order to support hazard awareness in a controlled task environment. (3) Method: Twenty-four participants performed a joystick-controlled navigation task and a concurrent mental spatial rotation task. Proximity to hazards in the navigation task was conveyed via different encodings of vibrotactile feedback: No Vibration, Intensity-Modulation, Pulse Duration, and Pulse Spacing. Performance metrics, including obstacle collisions, target hits, contact time, and accuracy, were assessed alongside perceived workload. (4) Results: Intensity-Modulated feedback reduced obstacle collisions and proximity time, while lowering workload, compared to No Vibration. No significant effects were found on spatial rotation accuracy, indicating that vibrotactile feedback effectively guides navigation and supports spatial awareness. (5) Conclusions: This study highlights the potential of vibrotactile feedback to improve navigation performance and hazard awareness, offering valuable insights into multimodal safety systems in high-demand environments. This paper delves into the development and implications of the Hearing Dog Evaluation and Reporting Overview (HERO) tool, which is a novel initiative built on the Five Domains animal welfare model to comprehensively assess the wellbeing of Hearing Dogs. The HERO’s creation involved two phases. In Phase One, an expert panel was surveyed using an electronic Delphi technique to gather consensus on items for the tool. This phase culminated in 28 items, exhaustively covered the Five Domains, and offered a holistic evaluation of Hearing Dog welfare. Phase One complemented existing assistance animal standards and emphasised the importance of a quality environment and continuous wellbeing monitoring. Phase Two involved 23 Hearing Dog owners, to assess the tool’s usability, particularly its user-friendliness and perceived value. Concerns about handlers’ ability to identify welfare indicators were raised, highlighting the need for additional development of resources and guidance. This preliminary study has shown that the HERO is a promising tool for understanding and promoting Hearing Dog welfare. Future research will look to validate the tool by drawing from expert observations and physiological measures, tailoring survey questions, and creating a user-sharing platform. Full article
