Special Issue Information

Dear Colleagues,

There have been numerous advances in the area of pancreaticobiliary endoscopy since the turn of this century, leading to improved patient outcomes. This includes advances in access to the pancreaticobiliary tree, as well as diagnostic and therapeutic interventions. The direct access to the bile duct and pancreatic duct, when ERCP fails, using endoscopic ultrasound guidance has become common practice such that IR-guided percutaneous biliary drainage is rarely needed in centers where interventional endoscopists are available. Similarly, gall bladder drainage using EUS guidance is replacing percutaneous cholecystostomy in patients with acute cholecystitis who are not deemed surgical candidates. Access to the ampulla in patients with altered anatomy including gastric bypass has seen dramatic improvements using the EUS-guided transgastric approach utilizing lumen-opposing metal stents and deep enteroscopy techniques. Our diagnostic skills have also improved due to advances in single operator cholangioscopy and pancreatoscopy with diagnostic and interventional tools. We are able to make an immediate diagnosis of pancreatic cysts using confocal laser endomicroscopy and predict the progression risk of these lesions using molecular studies. On the therapeutic side, there are many exciting developments including electrohydrolic and laser lithotripsy for large stones, with the application of RFA, PDT, and other modalities in pancreatic and biliary tumors. This Special Issue focuses on the application of endoscopic technology in pancreaticobiliary diseases.

Pancreatic cystic lesions (PCLs) are increasingly diagnosed owing to the wide use of cross-sectional imaging techniques. Accurate identification of PCL categories is critical for determining the indications for surgical intervention or surveillance. The classification and management of PCLs rely on a comprehensive and interdisciplinary evaluation, integrating clinical data, imaging findings, and cyst fluid markers. EUS (endoscopic ultrasound) has become the widely used diagnostic tool for the differentiation of pancreatic cystic lesions, offering detailed evaluation of even small pancreatic lesions with high sensitivity and specificity. Additionally, endoscopic ultrasound–fine-needle aspiration enhances diagnostic capabilities through cytological analysis and the assessment of fluid viscosity, tumor glycoprotein concentration, amylase levels, and molecular scrutiny. These detailed insights play a pivotal role in improving the clinical prognosis and management of pancreatic neoplasms. This review will focus mainly on the latest recommendations for the differentiation, management, and treatment of pancreatic cystic lesions, highlighting their clinical significance. Full article
