Automated Detection, Localization, and Severity Assessment of Proximal Dental Caries from Bitewing Radiographs Using Deep Learning

Background/Objectives: Dental caries is a widespread chronic infection, affecting a large segment of the population. Proximal caries, in particular, present a distinct obstacle for early identification owing to their position, which hinders clinical inspection. Radiographic assessments, particularly bitewing images (BRs), are frequently utilized to detect these carious lesions. Nonetheless, misinterpretations may obstruct precise diagnosis. This paper presents a deep-learning-based system to improve the evaluation process by detecting proximal dental caries from BRs and classifying their severity in accordance with ICCMSTM guidelines. Methods: The system comprises three fundamental tasks: caries detection, tooth numbering, and describing caries location by identifying the tooth it belongs to and the surface, each built independently to enable reuse across many applications. We analyzed 1354 BRs annotated by a consultant of restorative dentistry to delineate the pertinent categories, concentrating on the detection and localization of caries tasks. A pre-trained YOLOv11-based instance segmentation model was employed, allocating 80% of the dataset for training, 10% for validation, and the remaining portion for evaluating the model on unseen data. Results: The system attained a precision of 0.844, recall of 0.864, F1-score of 0.851, and mAP of 0.888 for segmenting caries and classifying their severity, using an intersection over union (IoU) of 50% and a confidence threshold of 0.25. Concentrating on teeth that are entirely or three-quarters presented in BRs, the system attained 100% for identifying the affected teeth and surfaces. It achieved high sensitivity and accuracy in comparison to dentist evaluations. Conclusions: The results are encouraging, suggesting that the proposed system may effectively assist dentists in evaluating bitewing images, assessing lesion severity, and recommending suitable treatments. Adulteration detection or authentication is considered a type of one-class classification (OCC) in chemometrics. An effective OCC model requires representative samples. However, it is challenging to collect representative samples from all over the world. Moreover, it is also very hard to evaluate the representativeness of collected samples. In this study, we blazed a new trail to propose an authentication method to identify adulterated edible oils without building a prediction model beforehand. An authentication method developed by real-time one-class classification modeling, and model population analysis was designed to identify adulterated oils in the market without building a classification model beforehand. The underlying philosophy of the method is that the sum of the absolute centered residual (ACR) of the good model built by only authentic samples is higher than that of the bad model built by authentic and adulterated samples. In detail, a large number of OCC models were built by selecting partial samples out of inspected samples using Monte Carlo sampling. Then, adulterated samples involved in the test of these good models were identified. Taking the inspected samples of avocado oils as an example, as a result, 6 out of 40 avocado oils were identified as adulterated and then validated by chemical markers. The successful identification of avocado oils adulterated with soybean oil, corn oil, or rapeseed oil validated the effectiveness of our method. The goal of “double carbon” puts forward higher requirements for the control of transport carbon emissions, and the exploration of transport carbon emission modelling driven by big data is an important attempt to reduce carbon accurately. Based on the land Vehicle Miles Traveled data (VMT) and the sea Automatic Identification System (AIS) data, this study establishes a refined, high-resolution carbon emission measurement model that incorporates the use of motor vehicles and ships from a bottom-up approach and analyzes the spatial distribution characteristics of land and sea transport carbon emissions in Tianjin using geospatial analysis. The results of the study show that (1) the transportation carbon emissions in Tianjin mainly come from land road traffic, with small passenger cars contributing the most to the emissions; (2) high carbon emission zones are concentrated in economically developed, densely populated, and high road network density areas, such as the urban center Binhai New Area, and the marine functional zone of Tianjin; (3) carbon emission values are generally higher in the segments where ports, airports, and interchanges are connected. The transportation carbon emission measurement model developed in this study provides practical, replicable, and scalable insights for other coastal cities. Full article

Background: Advancements in surgical care have made it possible to offer cardiac surgery to an older and frailer patient cohort. Frailty has been recognised as a prognostic indicator that impacts post-operative recovery and patient outcomes. The aim of this study is to identify frailty assessment tools, evaluate the impact of frailty on post-operative outcomes, and explore strategies to optimise care for frail patients undergoing cardiac surgery. Methods: A comprehensive literature search was performed across PubMed, MEDLINE, and SCOPUS to identify articles reporting post-operative outcomes related to frail patients undergoing cardiac surgery. Results: Measurement tools such as gait speed, the Clinical Frailty Scale, Fried frailty phenotype, deficit accumulation frailty index and the Short Physical Performance Battery can be used to assess frailty. Frailty has been reported to increase the risk of post-operative morbidity and mortality. Multiple studies have also reported the association between frailty and an increased length of intensive care unit and hospital stays, as well as an increased risk of post-operative delirium. It is important to perform a comprehensive frailty assessment and implement perioperative optimisation strategies to improve outcomes in this patient population. Pre-operative strategies that can be considered include adequate nutritional support, cardiac prehabilitation, and assessing patients using a multidisciplinary team approach with geriatric involvement. Post-operatively, interventions such as early recognition and treatment of post-operative delirium, nutrition optimisation, early planning for cardiac rehabilitation, and occupational therapy can support patients’ recovery and reintegration into daily activities. Conclusions: The early identification of frail patients during the perioperative period is essential for risk stratification and tailored management strategies to minimise the impact of frailty on outcomes following cardiac surgery. The development of robust, real-time optical methods for the detection and tracking of particles in complex, multiple-scattering media is a problem of practical importance in a number of fields, including environmental monitoring, air quality assessment, and homeland security. In this paper, we develop a holographic, optical-theorem-based method for the detection of particles embedded in complex environments where wavefronts undergo strong multiple scattering. The proposed methodology is adaptive to a complex medium, which is integral to the sensing apparatus and thereby enables constant monitoring through progressive adaptation. This feature, along with the holographic nature of the developed approach, also renders (as a byproduct) real-time imaging capabilities for the continuous tracking of particles traversing the region under surveillance. In addition, the proposed methodology also enables the development of customized sensors that leverage a controllable complex multiple-scattering medium and the derived holographic sensing technology for real-time particle detection and tracking. We demonstrate, with the help of realistic computer simulations, holographic techniques capable of detecting and tracking small particles under such conditions and analyze the role of multiple scattering in enhancing detection performance. Potential applications include the identification of aerosolized biological substances, which is critical for biosecurity, and the rapid detection of hazardous airborne particles in confined or densely populated areas. The advancement of multi-omics technologies is crucial to deepen knowledge on tick biology. These approaches, used to study diverse phenomena, are applied to experiments that aim to understand changes in gene transcription, protein function, cellular processes, and prediction of systems at global biological levels. This review addressed the application of omics data to investigate and elucidate tick physiological processes, such as feeding, digestion, reproduction, neuronal, endocrine systems, understanding population dynamics, transmitted pathogens, control, and identifying new vaccine targets. Furthermore, new therapeutic perspectives using tick bioactive molecules, such as anti-inflammatory, analgesic, and antitumor, were summarized. Taken together, the application of omics technologies can help to understand the protein functions and biological behavior of ticks, as well as the identification of potential new antigens influencing the development of alternative control strategies and, consequently, the tick-borne disease prevention in veterinary and public health contexts. Finally, tick population dynamics have been determined through a combination of environmental factors, host availability, and genetic adaptations, and recent advances in omics technologies have improved our understanding of their ecological resilience and resistance mechanisms. Future directions point to the integration of spatial omics and artificial intelligence to further unravel tick biology and improve control strategies. Full article

Ulcerative colitis (UC) is a chronic inflammatory bowel disease closely associated with dysbiosis of the gut microbiome, encompassing not only bacterial communities but also fungal populations. Despite the growing recognition of the gut microbiome’s role in UC pathogenesis, the contribution of intestinal fungi has only recently garnered significant attention. In this review, we comprehensively examine the characteristics of intestinal fungi in both healthy individuals and UC patients, elucidating their role in disease pathogenesis and their interactions with bacterial communities. Additionally, we explore the impact of intestinal fungi on disease severity and therapeutic responses in UC. Furthermore, we evaluate the therapeutic potential of antifungal agents, probiotics, and fecal microbiota transplantation (FMT) in UC management, emphasizing the critical role of fungi in these treatment modalities. Future research should prioritize elucidating the multifunctional roles of fungi in UC pathogenesis and their implications for treatment strategies. Moreover, the identification of fungal biomarkers associated with FMT efficacy could pave the way for precision medicine approaches in FMT, offering novel insights into personalized therapeutic interventions for UC. Pancreatic ductal adenocarcinoma (PDAC) is a highly lethal malignancy, often associated with new-onset diabetes. The relationship between PDAC and diabetes, particularly type 3c diabetes, remains poorly understood. This study investigates whether PDAC-associated diabetes represents a distinct subtype by integrating transcriptomic and histological analyses. Whole-tumour RNA sequencing data from The Cancer Genome Atlas (TCGA) were analysed to compare gene expression profiles between PDAC patients with and without diabetes. Cell-type Identification By Estimating Relative Subsets Of RNA Transcripts (CIBERSORT) deconvolution was employed to assess immune cell populations. Histopathological evaluations of pancreatic tissues were conducted to assess fibrosis and islet morphology. Histological analysis revealed perivascular fibrosis and islet basement membrane thickening in both PDAC cohorts. Transcriptomic data indicated significant downregulation of islet hormone genes insulin (INS) and glucagon (GCG) but not somatostatin (SST) in PDAC-associated diabetes, consistent with a type 3c diabetes phenotype. Contrary to previous reports, no distinct immunogenic signature was identified in PDAC with diabetes, as key immune checkpoint genes (Programmed Cell Death Protein 1 (PDCD1), Cytotoxic T-Lymphocyte Associated Protein 4 (CTLA4), Programmed Death-Ligand 1(PD-L1)) were not differentially expressed. The findings suggest that PDAC-associated diabetes arises through neoplastic alterations in islet physiology rather than immune-mediated mechanisms. The observed reductions in endocrine markers reinforce the concept of PDAC-driven β-cell dysfunction as a potential early indicator of malignancy. Given the poor response of PDAC to PD-L1 checkpoint inhibitors, further research is needed to elucidate alternative therapeutic strategies targeting tumour–islet interactions. Full article

Background/Objectives: Epithelial basement membrane dystrophy (EBMD) is a common corneal dystrophy characterized by recurrent corneal erosions and visual impairments due to surface irregularities and opacities. This study aims to evaluate the effectiveness of alcohol-assisted delamination (ALD) of the corneal epithelium in patients with EBMD affecting the visual axis, who experience decreased vision quality due to higher-order aberrations (HOAs) and irregular astigmatism. Methods: Eleven eyes of nine patients (four males and five females) were treated with ALD, with a mean age of 51.3 ± 19.7 years. All patients underwent refraction, best-corrected visual acuity (BCVA) assessment, a comprehensive slit-lamp examination for EBMD pattern identification, anterior segment imaging with and without fluorescein, tear break-up time (BUT) testing, corneal topography, corneal aberrometry (Zernike coefficients (Zmn) were calculated for a 5.0 mm simulated pupil), and anterior segment optical coherence tomography preoperatively and at 1-day, 14-day, 1-month, 3-month, 6-month, and 12-month intervals. Results: All patients demonstrated improvements in BCVA and visual quality, ocular comfort, and BUT results. The mean root mean square (RMS) value of total corneal aberrations decreased from 1.72 ± 0.90 μm to 0.90 ± 0.62 μm, while the mean RMS value of HOAs reduced from 0.92 ± 0.48 μm to 0.53 ± 0.28 μm. Astigmatism and trefoil were the aberration components that exhibited the most significant reductions. Conclusions: Alcohol-assisted delamination of the corneal epithelium is a safe and effective treatment for central EBMD patients experiencing visual quality deterioration. Astigmatism and trefoil appear to be the primary aberrations contributing to visual disturbances in this patient population. Full article

Background: Metabolic-associated fatty liver disease (MASLD) and metabolic syndrome (MetS) are increasingly prevalent among children and adolescents with obesity, posing significant long-term metabolic and cardiovascular risks. Non-invasive identification of at-risk individuals is crucial for a timely intervention. This study aimed to evaluate the diagnostic performance of the triglyceride-glucose (TyG) index and its modified versions, TyG-body mass index (TyG-BMI) and TyG-waist circumference (TyG-WC), in predicting MASLD and MetS in a large cohort of children and adolescents with obesity. Methods: A total of 758 children and adolescents with obesity (454 females, 304 males; mean age 14.8 ± 2.1 years; mean BMI 37.9 ± 6.2 kg/m2) were included. MASLD was diagnosed via ultrasonography, while MetS was defined using International Diabetes Federation criteria. TyG, TyG-WC, and TyG-BMI were calculated for all participants. Receiver operating characteristic (ROC) curves were generated to assess the diagnostic accuracy of these indexes, including sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV). Results: MASLD was detected in 38.9% of participants, with a higher prevalence in males (p < 0.0001). MetS was present in 27.8% of the cohort, with higher prevalence in males (p < 0.0001). Among the indexes, TyG-WC exhibited the highest sensitivity for MASLD (77.6%), whereas TyG-BMI had the highest specificity (63.3%). In predicting MetS, all three indexes performed better than for MASLD, with TyG demonstrating the highest PPV (54.5%) and TyG-BMI the highest NPV (87.5%). Predictive performance was lower in males than females, potentially due to sex-specific differences in fat distribution and metabolic response. Conclusions: TyG, TyG-WC, and TyG-BMI are promising, non-invasive tools for identifying children and adolescents with obesity at risk for MASLD and MetS. The superior sensitivity of TyG-WC and the high specificity of TyG-BMI highlight the value of incorporating anthropometric parameters into metabolic screening. Integrating these indexes into routine clinical practice may enhance early detection, allowing for timely intervention and personalized management strategies, ultimately reducing the long-term burden of metabolic and liver diseases in pediatric populations. Background: Steatotic liver disease (SLD) is a growing global health concern and may progress to more advanced liver diseases (i.e., fibrosis, cirrhosis, and hepatocellular carcinoma). Early identification of individuals at risk through effective screening strategies is crucial for timely intervention and management. The aim of this population-based study was to evaluate the feasibility of mean/large-scale screening and its importance by analyzing key risk factors, such as metabolic and lifestyle-related determinants. Methods: This cross-sectional study involved 387 subjects aged 18 to 89 years in a remote rural area that stretches among the valleys at the foot of the Apennines and the Apuan Alps. Anthropometric and demographic data were recorded, together with the measurement of blood pressure and cardiac rhythm. Furthermore, US-based liver stiffness (LS) and the ultrasound attenuation parameter (UAP) using the ILivTouch (Hisky Medical, Wuxi, China) device were performed. All data were analyzed with SPSS version 28. Univariate and multivariate analyses were conducted to identify significant predictors of both LS and UAP. Results: Significant associations are observed between elevated LS and UAP values and risk factors, such as BMI and waist circumference (BMI and waist with R = 0.45 and R = 0.34, R = 0.29 and R = 0.28; respectively, for UAP and LS; all with p < 0.001). The presence of hypertension is associated with a high value of LS (p < 0.05), and high UAP with alcohol consumption and sugary coffee intake habit (p < 0.001 and, p < 0.05, respectively). Conclusions: General population screening for SLD is feasible, sustainable, and useful to identify both individuals at risk and patients with progressive liver disease. Full article

Candida auris has emerged as a multidrug-resistant yeast implicated in healthcare-associated invasive infections and hospital outbreaks. The aim of the current 38-month period observational study in a multidisciplinary Intensive Care Unit (ICU) was to analyze the epidemiology, potential risk factors, management strategies, and patient outcomes of patients with C. auris. During the study period, 32 patients were identified with C. auris infection (6 patients) or colonization (26 patients) and their clinical characteristics and treatment-related factors were compared. Identification of C. auris isolates was confirmed by MALDI-TOF spectrometry. According to our results, regarding patient-related factors, no significant differences were identified. Regarding treatment-related factors, the proportion of patients already receiving corticosteroids (34.6% vs. 83.3%, p = 0.064) or being on renal replacement treatment (7.7% vs. 33.3%) was higher in infected patients. Median time elapsed from ICU admission to first positive culture was 7 (1–21) days and half of cases were ICU-imported. All strains were resistant to fluconazole and susceptible to echinocandines and amphotericin B. Crude mortality of the study population was 43.75%, similar to other previously reported candidemias. Rapid identification of C. auris, continued surveillance, and infection control practices are important elements for controlling successfully its spread in the hospital setting and for establishing promptly its transition from commensalism to infection. The conversion of tropical forests into urban and agriculture landscapes may alter insect populations through habitat disturbance and impact the diets of aerial insectivores. Most dietary studies on aerial insectivores have limitation on identifying prey at higher taxonomic levels in broad landscapes, restricting species-level identification and thus making a detailed dietary comparison impossible. This study examines the dietary changes through adaptation of house-farm swiftlets (Aerodramus sp.) and Pacific swallows (Hirundo tahitica) across three distinct habitats in Peninsular Malaysia: mixed-use landscapes, oil palm plantations, and paddy fields. High-throughput DNA metabarcoding with ANML primers targeting mitochondrial CO1 gene, identified 245 arthropod prey species, with six dominant orders: Coleoptera, Diptera, Blattodea, Hemiptera, Hymenoptera, and Lepidoptera. Mixed-use landscapes supported the highest dietary diversity and niche breadth, reflecting their ecological complexity. Paddy fields exhibited moderate diversity, while oil palm plantations demonstrated the lowest diversity, influenced by simplified vegetation structures and limited prey availability. The consumption of agricultural pests and vector species highlights the critical ecological role of aerial insectivorous birds in natural pest management and mitigating vector-borne disease risks. This research emphasizes the importance of conserving habitat heterogeneity to sustain the ecological services provided by these birds, benefiting both agricultural productivity and public health. Great progress has been made in the treatment of pulmonary arterial hypertension (WHO group 1 PAH) over the past two decades, which has significantly improved the morbidity and mortality in this patient population. Likewise, the more recent availability of antifibrotic medications for interstitial lung disease (ILD) have also been effective in slowing down the progression of disease. There is no known cure for either of these disease states. When this combination coexists, treatment can be challenging. Interstitial lung disease is a heterogenous group of chronic inflammatory and/or fibrotic parenchymal lung disorders. A subset of patients with ILD, not related to connective tissue disease, can initially present with inflammatory-predominant disease which progresses to irreversible fibrosis. This population of patients is also at risk for developing pulmonary hypertension (PH) or World Health Organization (WHO) group 3 PH. This coexistence of ILD and PH is associated with early morbidity and mortality. The early identification, diagnosis, and treatment of this combination of ILD and PH is vital. Medications available for both ILD and PH require an individualized approach with the intention of slowing down disease progression. Referral to expert centers for clinical trials and transplant evaluation is recommended. The combination of PH-ILD can be challenging to diagnose and treat effectively. Patients require a thorough clinical evaluation to enable the most accurate diagnosis. A vital part of that evaluation is the early recognition of PH. Medications can help improve disease progression along with clinical trials that will further improve our gaps in knowledge. Full article

Nanchuan Dachashu (Camellia nanchuanica), an arboreal tea species from Chongqing, China, exhibits valuable germplasm characteristics and tea production quality. However, the morphological diversity and genetic basis of key traits, such as tree architecture, leaf anatomy, and ovary trichomes, within this natural population remain to be elucidated. In this study, we conducted a survey on 90 wild individuals from this population, with a special focus on ovary trichomes—an important taxonomic trait. Considerable variations were observed in tree architecture, leaf size and shape, and anatomical structures. Through association analysis, we identified the SNP locus Chr9_89939207 to be associated with the glabrous/hairy ovary trait. A KASP marker was subsequently developed based on this locus, which could accurately distinguish between glabrous and hairy ovary individuals of Nanchuan Dachashu, as well as differentiate this species from C. sinensis or other hairy ovary species. The SNP locus Chr9_89939207 resides in the exon of a predicted protein phosphatase 2C (PP2C) gene, CSS0003297, which potentially regulates ovary trichome development in tea plants. These results reveal extensive morphological variation within the Nanchuan Dachashu population, establish a molecular tool for the identification of valuable interspecific hybrids, and provide insights into the breeding and industrial applications of this germplasm.
