Cover Story: Precise molecular mechanisms underlying the natural history of intracranial aneurysms (IAs) are still debated. The wall shear stress supports IA genesis by triggering endothelial dysfunction, vessel remodeling, myointimal hyperplasia, and smooth muscle cells’ phenotypic switching. Activation of the inflammatory cascade and endothelial oxidative stress, along with cardiovascular risk factors, promotes IA growth and rupture. Recent advances in target therapies against inflammatory mediators may contribute to amending the natural history of IAs. View this paper
